Cyclosporin (cyclosporin A) is a unique immunosuppressant used to prevent the rejection of transplanted organs and to treat diseases of autoimmune origin. Therapeutic drug monitoring of cyclosporin is essential for several reasons: wide variability in cyclosporin pharmacokinetics has been observed after the oral or intravenous administration of the drug. The variability in the kinetics of cyclosporin is related to a patient's disease state, the type of organ transplant, the age of the patient and therapy with other drugs that interact with cyclosporin; maintaining a blood concentration of cycl

For several years cyclosporin A has been used as an immunosuppressive agent in transplantation medicine and to avoid graft-versus-host disease. Some case reports concerning beneficial effects on different dermatoses have aroused speculation about the application of this drug in dermatology. The mode of action, a summary of the dermatologic case reports, and a discussion of the side effects of cyclosporin A are presented in this article.

Cyclosporine is a new immunosuppressive drug that acts early in the exposure of a host to allogeneic stimulation. It is a peptide of fungal origin. It has selective action on T cells, leaving the other cells of the immune system intact. It acts by preventing the function of the early activation signals of T cells, such as the acquisition of receptors for Il 2 and Il 1. It is lipophilic, moderately well absorbed by the gut, and metabolized by the liver. Factors affecting absorption or hepatic metabolism alter the amount of cyclosporine available in the circulation. Circulating levels can be mea

Erythromelalgia is a rare syndrome characterised by recurrent erythema, heat and burning pain in the extremities. There are two types: primary (genetic) and secondary (toxic, drug-related or associated with other diseases). We report a 42-year-old woman who developed erythromelalgia after taking cyclosporine for myasthenia gravis. Although exact mechanism for this rare adverse effect is unclear, it is reversible, and so clinicians should be aware of the association . Additional use of corticosteroids could aggravate cyclosporine's toxic effects.

UNLABELLED: Cholinergic itch is part of symptom complex that also includes cholinergic erythema and cholinergic urticaria. It mostly occurs during the winters among young adults. It is characterized by onset of severe itching or burning sensation all over body, mostly, on exposure to sunlight, warm atmosphere and in some cases after hot and spicy food intake. In most of the cases, it is poorly responsive to antihistamine therapy.
MATERIALS AND METHODS: This was a prospective, open labeled, clinical study done in patients of cholinergic itch, refractory to both sedating and non sedating anti-h

Cyclosporine began to be used as one of the immunosuppressive agents in transplantation in the beginning of the 1980s, and in the treatment of nephrotic syndrome in the pediatric nephrology. Therapeutic area of cyclosporine is narrow and its side effects limit its usage. Preference for cyclosporine and tacrolimus as a calcineurin inhibitor is left for the choice of the department. There are still countries with preferred-cyclosporine use because of economic reasons. Cyclosporine is currently being used in the treatment of nephrotic syndrome, but due to its high relapse rates in a short-term us

Acitretin alone is efficient (PASI 90: 40%). In responders, it is the best long-term maintenance treatment (up to 29 years of continuous treatment). The main side effect is its teratogenicity in females. It is necessary to begin retinoid treatment at low doses (10 mg/day), increasing the dose step by step, looking for the maximum well-tolerated dose (usually defined as a mild cheilitis). Doses higher than the highest well-tolerated dose are frequently responsible for the Kobner phenomenon. In children, retinoids are very efficient and nearly always well tolerated, but it seems important to nev

Cyclosporin is a potent immunosuppressive drug and is frequently used in the therapy of autoimmune diseases, including systemic lupus erythematosus (SLE). Few large studies have been performed using this drug in SLE patients. However, small uncontrolled studies of patients with SLE have shown favourable results with a significant improvement in disease activity, a fall in anti ds DNA titres and proteinuria and an improvement in complement levels, leucopaenia and thrombocytopaenia. Interestingly, a consistent reduction in corticosteroid dosage often by as much as 50% is seen. Toxicity, especial

The efficacy of cyclosporine is well documented in multiple clinical trials. Patients with severe psoriasis who require systemic treatment should be considered for this therapeutic agent. The need for both a careful evaluation prior to therapy and strict monitoring of the patient cannot be over emphasized because untreated hypertension, renal dysfunction, and other disorders may lead to serious, nonreversible morbidity. Physicians are encouraged to follow the published guidelines for controlling psoriasis in their patients and lowering the risk of potential drug toxicity.

Cyclosporin is an immunosuppressant used in organ transplantation and selected autoimmune diseases such as rheumatoid arthritis. In both these indications, the elderly represent an important and growing segment of the patient population. Cyclosporin is primarily eliminated via biotransformation by cytochrome P450 (CYP)3A in the gut wall and liver. Additionally, P-glycoprotein (mdr-1) located in the gastrointestinal epithelium can affect affect blood drug concentrations after oral administration of cyclosporin, presumably by counter-transporting the drug from the systemic circulation back into

The aim of the study was to find out whether an extended subacute toxicity study (additional organ weights, histopathology and immune functional tests), routinely employed in testing of chemicals, would shown indications of (adverse) effects on the immune system below general toxicity. Therefore, a five laboratory ring study on the basis of an oral 28-day repeated dose study in rats (OECD guideline 407) was carried out with 1, 5 and 25 mg/kg of cyclosporin A (CsA) per day by gavage. Besides some toxic effects such as reduced body weight gain and increased kidney calcification in the high-dose

Cyclosporin A (CSA) is an immunosuppressive agent that has provided new approaches in transplantation medicine and in the treatment of autoimmune diseases. One of the most common dermatological side effects of oral CSA is dose-dependent hypertrichosis. This stimulating effect on hair growth encouraged a number of investigators to use CSA not only in the treatment of alopecia areata (AA), but also in male pattern alopecia (MPA). While oral application proved successful, the beneficial effect of topical application was very limited in both AA and MPA.

A case of pyoderma gangrenosum that occurred in the absence of underlying illness and was initially misdiagnosed is described. Hemorrhagic pustules and ulcers appeared over vein-graft sites after open heart surgery and were treated with cyclosporine. The result was rapid and complete resolution. It is the purpose of this report to emphasize pyoderma gangrenosum as a cause of ulceration in healthy individuals and to highlight the addition of cyclosporine to the therapeutic armamentarium.

Atopic dermatitis is a chronic pruritic dermatosis that requires individualized treatment. Recent studies have shown that low-dose oral treatment with cyclosporine A may be useful in more severe forms. We report our experience with two patients who had severe atopic dermatitis. Both had high IgE concentration and satisfied Hanifin and Rajka's diagnostic criteria for atopic dermatosis. We began oral treatment at a daily dosage of 5 mg/kg divided into two doses. We monitored cyclosporine concentration in whole blood, as well as kidney and liver function. Contraindications for cyclosporine A were
