Background/Objectives: The management of locally advanced rectal cancer has evolved significantly, shaped by advances in multimodal neoadjuvant therapy and a growing emphasis on organ preservation through the watch-and-wait approach. These advancements, however, introduce complex treatment decisions that require careful consideration by both patients and clinicians. Methods: This narrative review explores the evolution of the management of locally advanced rectal cancer and the role of shared decision-making in guiding treatment decisions, particularly for patients facing decisions between sur

The increasing use of antidepressants, escalating rates of unintended pregnancy, and disproportionately high rate of maternal suicide in the United States, as compared with other high-income countries, all highlight the urgent need for more clarity on available treatment options for perinatal depression. Misrepresentative data obtained from animal experiments and observational human studies, many of which do not account for underlying mental illness as a confounder, largely overestimate the teratogenicity of antidepressants. These outdated data-coupled with a lack of clear guidance from leadin

'Elderly' is most commonly defined as an individual aged 65 years or older. However, this definition fails to account for the differences in genetics, lifestyle and overall health that contribute to significant heterogeneity among the elderly beyond chronological age. As the world population continues to age, the prevalence of chronic diseases, including chronic kidney disease (CKD), is increasing and CKD frequently progresses to kidney failure. Moreover, frailty represents a multidimensional clinical entity highly prevalent in this population, which needs to be adequately assessed to inform a

PURPOSE: To quantify the results of shared decision making in pediatric strabismus surgery from the parent perspective using the nine-item Shared Decision Making (SDM) Questionnaire and the associations of SDM score with parental education level; secondarily, to evaluate postoperative parental satisfaction as a function of child age, parental education level, SDM scores, and motor outcomes.
METHODS: Consecutive parents of children 2-14 years of age with concomitant manifest strabismus that consented to strabismus surgery were surveyed using the SDM Questionnaire. A four-point Likert scale was

Shared decision-making (SDM) is a collaborative approach in which clinicians educate, support, and guide patients as they make informed, value-congruent decisions. SDM improves patients' health-related outcomes through increasing knowledge, reducing decisional conflict, and enhancing experience of care. We measured SDM in genetic counselling appointments with 27 pregnant women who were at increased risk to have a baby with a genetic abnormality. The eight experienced genetic counsellors who participated had no specific SDM training and were unaware that SDM was being assessed. Audio transcript

Office insertion of tympanostomy tubes in young children without anesthesia is facilitated through shared decision making with families, including a candid discussion about anticipated pain (which is usually brief and transient) and how their child might react to being restrained. This commentary elaborates on the rationale for office tube insertion as an alternative to general anesthesia, including a clinical decision aid and suggestions for optimizing outcomes.

We assessed the extent to which parents of children with autism spectrum disorder report that they are engaged in shared decision making. We measured the association between shared decision making and (a) satisfaction with care, (b) perceived guidance regarding controversial issues in autism spectrum disorder, and (c) perceived assistance navigating the multitude of treatment options. Surveys assessing primary medical care and decision-making processes were developed on the basis of the U.S. Department of Health and Human Service's Consumer Assessment of Healthcare Providers and Systems survey

In this article, an adjusted model of shared decision-making is presented for the sector of medical rehabilitation. This model of interaction describes shared decision-making as a process between physician and patient on the basis of shared information and with both parties equally and actively involved. The typical elements of this interaction model can be transferred to other professionals in the health care sector (i. e. therapists, nurses, social workers, etc.) as well as to the interaction between these different groups of health professionals working as a team. The idea of patient orient

People with diabetes often live with other chronic conditions and lead complicated lives. Determining what is the best management decision for a patient requires consideration of each individual's personal, social and biomedical context, what he or she values, the reasons he or she has to value the available options, and the relative contribution of each option in terms of benefits, harms, costs and inconveniences. Empathic conversations between patients and clinicians to diagnose the patient situation that necessitates action and the range of evidence-based actions that best address the situa

IMPORTANCE: Experts recommend a shared decision-making (SDM) process before percutaneous left atrial appendage occlusion (pLAAO) in patients with atrial fibrillation, and the Centers for Medicare & Medicaid Services (CMS) require SDM with the use of a patient decision aid (DA) as a condition for reimbursement. However, little is known about how these guidelines and policies have influenced practice.
OBJECTIVE: To describe overall trends in reported SDM and use of DAs for pLAAO and to identify key patient, operator, and institutional factors associated with their use.

BACKGROUND: Decision making for adult tracheostomy and prolonged mechanical ventilation is emotionally complex. Expectations of surrogate decision makers and physicians rarely align. Little is known about what surrogates need to make goal-concordant decisions. Currently, little is known about the decisional needs of surrogates and providers, impeding efforts to improve the decision-making process.
METHODS: Using a thematic analysis approach, we performed a qualitative study with semistructured interviews with surrogates of adult patients receiving mechanical ventilation (MV) being considered

BACKGROUND: Shared decision-making (SDM) aligns treatment recommendations with patient values and has been shown to positively affect surgical outcomes. We aimed to characterize the variation in levels of SDM among surgeons using CollaboRATE, a validated patient-reported tool for assessing SDM in clinical care.
STUDY DESIGN: This multicenter retrospective cohort study analyzed American College of Surgeons NSQIP patient-reported outcome data from 30,957 surgical patients treated by 2,921 surgeons across 65 hospitals between 2020 and 2023. Patient-reported scores were analyzed to (1) characteri

OBJECTIVES: Relatives often participate in medical decision-making, yet many report feeling excluded rather than meaningfully involved. Research indicates that when relatives are actively engaged in patient care - aligned with their abilities and needs - it can enhance patient adherence, safety, and the overall experience for both patients and their relatives. Despite this, their role in Shared Decision-Making (SDM) remains underexplored. This study examines perceptions of meaningful partner involvement in medical decision-making during hospital consultations with prostate cancer patients.

Shared decision-making (SDM) is recognised as a cornerstone of patient-centred care, yet progress in embedding SDM in urology has been inconsistent. The European Association of Urology (EAU) Patient Office convened urologists, nurses, and patient advocates at two consensus meetings (Paris, France, March 2024; Rome, Italy, November 2024) to define core SDM principles and identify barriers to implementation. The outcome was the SHINE-URO framework, comprising six principles: open discussion of harms and benefits; exploration of patient values and goals; provision of clear and guideline-based inf

Sex differences have been reported to influence the clinical symptoms of Parkinson's disease and the pharmacokinetics and side effects of anti-Parkinsonian drugs. However, these aspects have rarely been considered in clinical practice. Information regarding drug therapy during pregnancy is insufficient owing to the high proportion of elderly patients. Device-aided therapies are used to treat motor complications in patients with advanced Parkinson's disease. As it is a highly invasive therapy, shared decision-making is important for its introduction.
