Retrobulbar pressures measured during surgical decompression of the orbit.

AIMS/BACKGROUND: In Graves' ophthalmopathy the increase in volume of intraocular muscles and fat will cause elevated intraorbital pressure. In order to investigate the pressure levels involved, intraorbital pressure, or retrobulbar pressure (RBP) was measured continuously in orbits of patients with Graves' ophthalmopathy during surgical decompression. METHODS: Retrobulbar pressure was measured before and during surgical decompression using an intraorbitally applied pressure transducer. RESULTS: In eight patients with dysthyroid optic neuropathy (DON) RBPs between 17 and 40 mm Hg were recorde

Bone regrowth after surgical decompression for lumbar spinal stenosis.

We reviewed 40 patients treated surgically for lumbar stenosis at an average time of 8.6 years after operation. In 32, total laminectomy had been performed and in eight bilateral laminotomy, both at one or more levels. Of the 16 patients with degenerative spondylolisthesis, ten had had a concomitant spinal fusion. Patients were assigned to one of four groups according to the amount of bone regrowth: group 0 had no regrowth and groups I, II, and III, had mild, moderate or marked regrowth, respectively. Only 12% of the patients showed no bone regrowth; 48% were assigned to group I, 28% to group

[Surgical treatment of spasmodic torticollis: effectiveness of microvascular decompression].

We report two patients of spasmodic torticollis who underwent different types of surgery according to their pathogenetic mechanisms. The first case was a 42-year-old man who had two years history of right-sided antecollis. EMG of the sternocleidal muscle on the affected side showed pathological findings. Characteristic findings on the vertebral angiography were a low position of the branching of tortuous PICA. At operation, we confirmed sandwich-like compression of left accessory nerve between the PICA and vertebral artery. Decompression of the nerve from these arteries led to a remarkable rel

Functional Outcomes after Surgical Decompression for Tarsal Tunnel Syndrome.

OBJECTIVES: To evaluate the functional outcomes of surgical decompression for tarsal tunnel syndrome (TTS) and to explore the clinical relevance of diagnostic tests such as electromyography (EMG), magnetic resonance imaging (MRI), and ultrasound. METHODS: We performed a retrospective single-center study of 15 patients with clinically diagnosed TTS who underwent open decompression (2015-2022). All had failed conservative management and completed ≥ 12 months of follow-up. Preoperative evaluation combined clinical exams with at least one confirmatory test (EMG, MRI, or ultrasound). Outcomes incl

Surgical technique management of microvascular decompression for trigeminal neuralgia.

Trigeminal neuralgia is a severe neuropathic disorder, affecting the distribution area of the trigeminal nerve and often impairs the quality of life of patients. More and more scholars agree that one of the pathogenesis of trigeminal neuralgia is due to the demyelinating lesion caused by vascular compression or arachnoid bundle wrapping on the root exit zone of trigeminal nerve. In this regard, the most effective method is microvascular decompression, which can relieve the compression of the offending vessels and the thickened arachnoid on the trigeminal nerve. However, it still has some disad

Surgical decompression for malignant cerebral oedema after ischaemic stroke.

BACKGROUND: Large territory middle cerebral artery (MCA) ischaemic strokes account for around 10% of all ischaemic strokes and have a particularly devastating prognosis when associated with malignant oedema. Progressive cerebral oedema starts developing in the first 24 to 48 hours of stroke ictus with an associated rise in intracranial pressure. The rise in intracranial pressure may eventually overwhelm compensatory mechanisms leading to a cascading secondary damage to surrounding unaffected parenchyma. This downward spiral can rapidly progress to death or severe neurological disability. Early

Imaging appearance following surgical decompression of the ulnar nerve.

Ulnar neuropathy at the elbow is the second most common entrapment neuropathy of the upper extremity. Yet, there is a paucity of literature focusing on the imaging appearance following surgical decompression of the ulnar nerve at the elbow. Diagnostic imaging studies obtained after surgical decompression at The University of Michigan were reviewed and imaging findings were documented. We aim to describe the various techniques of ulnar nerve decompression and corresponding post-operative appearance on imaging. Potential complications following decompression will also be described with imaging a

Surgical Freedom Evaluation During Optic Nerve Decompression: Laboratory Investigation.

BACKGROUND AND OBJECTIVE: Various surgical routes have been used to decompress the intracanalicular optic nerve. Historically, a transcranial corridor was used, but more recently, ventral approaches (endonasal and/or transorbital) have been proposed, individually or in combination. The present study aims to detail and quantify the amount of bony optic canal removal that may be achieved via transcranial, transorbital, and endonasal pathways. In addition, the surgical freedom of each approach was analyzed. METHODS: In 10 cadaveric specimens (20 canals), optic canals were decompressed via pterio

Delayed spinal cord infarction following anterior cervical surgical decompression.

Anterior cervical discectomy and fusion (ACDF) for cord compression is a safe and effective procedure with good outcomes. However, worsening of myelopathy is the most feared adverse event of the surgery. We report the case of a 36-year-old male patient who presented with an acute non-traumatic C5-6 cervical disc herniation causing incomplete quadriparesis. He underwent an uncomplicated ACDF at C5-6, and after an initial period of improvement, he developed a delayed onset of an anterior cord syndrome on day 3, without any discerning cause. We have reviewed similar cases reported in the literatu

Surgical decompression for cerebral oedema in acute ischaemic stroke.

BACKGROUND: Large cerebral infarction has a high case fatality. Despite the use of conventional medical treatments such as hyperventilation, mannitol, diuretics, corticosteroids and barbiturates, the outcome of this condition remains poor. Decompressive surgery to relieve intracranial pressure is performed in some cases, although evidence of any clinical benefits has not been available until recently. This is an update of a Cochrane review first published in 2002. OBJECTIVES: To examine the effects of decompressive surgery in patients with massive acute ischaemic stroke complicated with cereb

Outcome after surgical decompression of severe traumatic brain injury.

One of the factors that affects outcome following severe traumatic brain injury is development and progression of cerebral oedema with associated increase in intracranial pressure (ICP). Uncontrolled elevations of ICP may compromise energy metabolism of the injured brain and lead to secondary injury, affecting neurological outcome of the patient. Decompressive craniectomy has been used for over a century as a treatment of refractory brain swelling in a variety of neurological conditions. However, conclusive evidence of whether it has a beneficial or adverse affect on outcome is lacking. This a

Surgical decompression for cerebral oedema in acute ischaemic stroke.

BACKGROUND: The high mortality that follows a large cerebral infarction is in part due to brain oedema. Oedema causes mass-effect with raised intracranial pressure and herniation. Medical therapies are used to reduce intracranial pressure but outcome is poor in spite of treatment. Decompressive surgical techniques that attempt to relieve high intracranial pressure due to oedema have been described, but their efficacy in reducing case fatality and disability is uncertain. OBJECTIVES: To compare medical therapy plus decompressive surgery with medical therapy alone on the outcomes death and 'dea

Surgical treatment of benign intracranial hypertension--subtemporal decompression revisited.

BACKGROUND: Subtemporal decompression, first advocated by Dandy for the treatment of benign intracranial hypertension or pseudotumor cerebri, has been replaced as a treatment mainstay by medical management using diuretics, steroids, and lumbar puncture. Failure of these forms of treatment has frequently led to insertion of cerebrospinal fluid shunts. METHODS: We have retrospectively reviewed the long term outcome of eight patients who were treated by subtemporal decompression (STD) for classical presentations of refractory benign intracranial hypertension. The follow-up period ranged from 8 t

Meralgia paresthetica: etiology, diagnosis, and outcome of surgical decompression.

Entrapment of the lateral femoral cutaneous (LFC) nerve as a source of anterolateral thigh dysesthesias has been recognized for 100 years. Despite this historic recognition, its diagnosis today is often delayed, and definitive treatment of refractory cases by surgical decompression rarely reported. This study describes 26 LFC entrapments in 23 patients whose etiologies include iliac crest bone graft harvesting, seat belt injury associated with motor vehicle accident, and diabetes. Twenty-two of the 23 patients (25 of the 26 entrapments) achieved good to excellent outcomes following surgical de

Cauda equina syndrome treated by surgical decompression: the influence of timing on surgical outcome.

A prospective longitudinal inception cohort study of 33 patients undergoing surgery for cauda equina syndrome (CES) due to a herniated lumbar disc. To determine what factors influence spine and urinary outcome measures at 3 months and 1 year in CES specifically with regard to the timing of onset of symptoms and the timing of surgical decompression. CES consists of signs and symptoms caused by compression of lumbar and sacral nerve roots. Controversy exists regarding the relative importance of timing of surgery as a prognostic factor influencing outcome. Post-operative outcome was assessed at 3

[The significance of surgical anatomy in lateral orbital wall decompression].

The article reviews data on the anatomical and topographic features of the lateral wall in the context of performing orbital surgery and presents a description of the surgical areas of the lateral wall that are most important for increasing the volume of the orbit: the lacrimal keyhole, the sphenoid door jamb, and the basin of the inferior orbital fissure. Particular attention is given to the topographic anatomy of the sphenoid door jamb as the most promising area for orbital decompression and transorbital access to the skull base.

On the Surgical Implications of Peritrigeminal Perforating Vessels in Microvascular Decompression.

BACKGROUND: Perforating branches arising from the superior cerebellar artery (SCA) or anterior inferior cerebellar artery (AICA) that pierces the brainstem within 5 mm of the trigeminal root may limit offending vessel transposition during microvascular decompression for trigeminal neuralgia. OBJECTIVE: To investigate the microsurgical anatomy of peritrigeminal perforators and evaluate their effect on the mobility of the SCA and AICA. Additionally, we propose strategies for mitigating the potential complications caused by the presence of short peritrigeminal perforators. METHODS: Retrosigmoid
