Bilateral Decompressive Craniectomy in Malignant Cerebral Venous Thrombosis.

INTRODUCTION: Despite the extremely favourable prognosis of patients with cerebral venous thrombosis (CVT), death occurs in 10-15% of patients. In severe cases of malignant CVT with supratentorial haemorrhagic lesions, cerebral oedema and brain herniation, decompressive surgery may be the only life-saving treatment. PATIENT AND METHODS: We present the case of a puerperal young woman with progressive headache, seizures and decreased alertness. Thrombosis of the entire superior sagittal sinus with bifrontal venous infarcts and midline shift was confirmed by magnetic resonance imaging with venog

Emergency decompressive craniectomy after removal of convexity meningiomas.

BACKGROUND: Convexity meningiomas are benign brain tumors that are amenable to complete surgical resection and are associated with a low complication rate. The aim of this study was to identify factors that result in acute postoperative neurological worsening after the removal of convexity meningiomas. METHODS: Clinical evaluation and neuroradiological analysis of patients who underwent removal of a supratentorial convexity meningioma were reviewed. Patients were selected when their postoperative course was complicated by acute neurological deterioration requiring decompressive craniectomy.

Decompressive Craniectomy and Traumatic Brain Injury: A Review.

Intracranial hypertension is the largest cause of death in young patients with severe traumatic brain injury. Decompressive craniectomy is part of the second level measures for the management of increased intracranial pressure refractory to medical management as moderate hypothermia and barbiturate coma. The literature lack of concepts is their indications. We present a review on the state of the art.

[Decompressive craniectomy in acute stroke - The different perspective].

Extensive space occupying strokes occur in about 1-10% of all ischaemic supratentorial infarctions. Both the high mortality and morbidity primarily result from secondary brain damage due to an accompanying brain edema. Therefore, the primary therapeutic target in patients with space occupying strokes is the control of the brain edema and the consecutively elevated intracranial pressure. If intracranial pressure cannot be controlled by conservative treatment methods, a decompressive craniectomy (DC) is a possible treatment option in selected patients to reduce intracranial pressure. In this rev

Introduction: decompressive craniectomy for trauma and cerebrovascular disease.

Elevated intracranial pressure is one of the most common causes of death and disability following severe traumatic brain injury and ischemic stroke. Unfortunately, there have been no new medical treatments for cerebral edema and elevated intracranial pressure in more than 80 years. Decompressive craniectomy may be an appropriate surgical option in the face of elevated intracranial pressure that is refractory to medical treatment. When performed correctly, this procedure can reduce intracranial pressure and prevent cerebral herniation and death. The last decade has seen a renewed interest in th

Functional recovery after decompressive craniectomy for cerebral infarction.

There is continuing controversy about the benefits of decompressive craniectomy in the treatment of lesions causing increased intracranial pressure (ICP) and brain edema. Laboratory work has shown a decrease in ICP after craniectomy, but also a paradoxical enhancement in the formation of underlying cerebral edema, which may act to the detriment of the patient. Since Rengachary et al. advocated craniectomy for massive cerebral infarction and reported their group of three patients, we have managed five patients with acute supratentorial cerebral infarction who progressed to uncal herniation and

Malignant Middle Cerebral Artery Infarction Decompressive Craniectomy Score: A Prognostic Model for Malignant Middle Cerebral Artery Infarction Patients Treated with Decompressive Craniectomy.

BACKGROUND: Malignant middle cerebral artery infarction (MMI) is a severe condition caused by space-occupying brain edema due to ischemia. While decompressive hemicraniectomy (DC) improves outcome, many survivors experience lasting disabilities, and tools to predict outcome are lacking. We aimed to identify independent predictors of outcome following DC in MMI and to create a pragmatic, bedside tool for patient prognostication. METHODS: This retrospective, multicenter study analyzed 318 MMI patients treated with DC at 4 Swedish university hospitals (2008-2022). Demographic, preoperative, and

Bilateral decompressive craniectomy in pediatric patients: A systematic review.

Traumatic brain injury (TBI) and other acute neurological insults remain leading causes of pediatric morbidity and mortality. In cases where elevated intracranial pressure (ICP) is refractory to maximal medical therapy, decompressive craniectomy (DC) may be considered as a salvage intervention. Although bilateral, bifrontal, bitemporal, and cruciate decompressive approaches are used in selected patients with diffuse cerebral edema or bihemispheric swelling, their role, timing, complications, and outcomes remain poorly defined in pediatric populations. Refractory traumatic intracranial hypertension: the role of decompressive craniectomy.

Traumatic brain injury according to the World Health Organization estimates that by 2020 will be the third leading cause of morbidity and mortality worldwide. Intracranial hypertension refractory to medical management is the cause of increased mortality in neurotrauma. There are various measures to control intracranial hypertension, including surgical. Decompressive craniectomy has been routinely used to treat intracranial hypertension secondary to cerebral infarction, subarachnoid hemorrhage, intracerebral hemorrhage and trauma. We review the literature to describe the mechanisms, types and i

Risk factors for post-traumatic hydrocephalus following decompressive craniectomy.

BACKGROUND: Post-traumatic hydrocephalus (PTH) is one of the main complications of decompressive craniectomy (DC) after traumatic brain injury (TBI). Then, the recognition of risk factors and subsequent prompt diagnosis and treatment of PTH can improve the outcome of these patients. The purpose of this study was to identify factors associated with the development of PTH requiring surgical treatment in patients undergoing DC for TBI. METHODS: In this study, we collected the data of 190 patients (149 males and 41 females), who underwent DC for TBI in our Center. Then we analyzed the type of sur

Bilateral Decompressive Craniectomy for Subdural Empyema: A Case Report.

Intracranial subdural empyema (SDE) is a rare but life-threatening condition characterized by the rapid spread of purulent infection within the subdural space and a high risk of neurological deterioration. Despite significant improvements in neuroimaging, antimicrobial therapy, and surgical techniques, severe cases may progress to diffuse cerebral edema and refractory intracranial hypertension, challenging conventional management strategies. Surgical evacuation remains the cornerstone of treatment; however, the role of decompressive craniectomy in infection-related intracranial hypertension is

Decompressive Craniectomy Decreases Neuroinflammation after Ischemic Stroke in Mice.

In human, decompressive craniectomy (DC) reduces morbi-mortality of malignant stroke. Because the cranial vault carries hematopoietic niches hosting inflammatory cells, we used here a preclinical model of stroke and DC in mice to evaluate whether the removal of the cranial vault may influence neuroinflammation after malignant stroke. 99 C57/BL6 mice were used, and stroke was induced using a filament model. Mice were randomized in 4 groups: Sham (n = 15), Stroke (n = 40), DC (n = 18) and Stroke + DC (n = 26). The inflammatory response was evaluated at D1 and D5 by using both non-invasive molecu

Ultrasound-Guided External Ventricular Drain Insertion After Decompressive Craniectomy.

BACKGROUND: Ultrasound guidance offers real-time visualization of patient-specific anatomy during external ventricular drain (EVD) insertion. A craniectomy defect provides a sonolucent window, enabling the use of a large, low-frequency probe with deep penetration and a wide field of view. While specialized burr-hole probes exist, use of a curvilinear probe through a craniectomy defect for bedside EVD placement has not been previously described. METHODS: Using a curvilinear probe, we performed ultrasound-guided bedside insertion of a left frontal EVD through a hemicraniectomy flap. RESULTS: B

Decompressive craniectomy in trauma: What you need to know.

Decompressive craniectomy (DC) is a surgical procedure in which a large section of the skull is removed, and the underlying dura mater is opened widely. After evacuating a traumatic acute subdural hematoma, a primary DC is typically performed if the brain is bulging or if brain swelling is expected over the next several days. However, a recent randomized trial found similar 12-month outcomes when primary DC was compared with craniotomy for acute subdural hematoma. Secondary removal of the bone flap was performed in 9% of the craniotomy group, but more wound complications occurred in the cranie

Sunken Skin Flap Syndrome: Neurological Dysfunction After Decompressive Craniectomy.

Sunken Skin Flap Syndrome (or Syndrome of the Trephined) following a head trauma is rare, but most often results from complications after decompressive craniectomy. This syndrome is most often characterized by neurological dysfunction that improves with cranioplasty. Early diagnosis and treatment are critically important to long term neurological improvement. This is a case report of a 49-year-old male who fell down a flight of stairs and was found unresponsive. Initial imaging revealed extensive head trauma. Neurosurgery performed an emergency decompressive craniectomy, but his post-operative

Intracranial Hypertension After Primary Decompressive Craniectomy for Head Trauma.

BACKGROUND: Primary decompressive craniectomy (DC) is commonly performed for patients with traumatic brain injury (TBI). Some, but not all patients, will benefit from invasive monitoring of intracranial pressure (ICP) after surgery. We intended to identify risk factors for elevated ICP after primary DC to treat TBI. METHODS: A retrospective chart review study identified all patients at our institution who underwent primary DC for TBI during the study period and who had ICP monitors placed at the time of surgery. Various preoperative and intraoperative variables were assessed for correlation w

Decompressive Craniectomy: the Right Call at the Right Moment.

Introduction:Massive intracerebral hemorrhages and large internal carotid artery infarcts may cause early death due to severe cerebral edema with elevated intracranial pressure, despite maximal medical therapy. Decompressive craniectomy may be of benefit to these patients in terms of survival and even functional outcome. The aim of our paper is to present two cases that illustrate the use of decompressive craniectomy both in ischemic and hemorrhagic stroke, followed by a discussion on the indication and right timing of the intervention, but also on the outcome of these patients. Management of decompressive craniectomy defects: modern military treatment strategies.

BACKGROUND: Decompressive craniectomy has become a critical and standard life-saving maneuver in the theater of war. The high number of patients returning with large cranial defects and complex craniofacial injuries often involving the upper orbits or associated with the sunken skin flaps are a reconstructive challenge. We present a review of our treatment protocol highlighting the evolution of decompressive craniectomy and the development of a modern cranial defect treatment algorithmic approach to reconstruct these difficult clinical cases. METHODS: A review of previously published data and
