Improving the quality of workers' compensation health care delivery: the Washington State Occupational Health Services Project.

This article has summarized research and policy activities undertaken in Washington State over the past several years to identify the key problems that result in poor quality and excessive disability among injured workers, and the types of system and delivery changes that could best address these problems in order to improve the quality of occupational health care provided through the workers' compensation system. Our investigations have consistently pointed to the lack of coordination and integration of occupational health services as having major adverse effects on quality and health outcome

Women's health priorities and perceptions of care: a survey to identify opportunities for improving preventative health care delivery for older women.

BACKGROUND: women live longer than men but consistently report poorer health status and health-related quality of life. Information is scarce on what components of care women desire to help them meet the challenges of their later years. OBJECTIVES: to identify older women"s priorities for maintaining physical, emotional and social well-being in later years and to measure the extent to which women perceive that these priorities are being met by the health care system. METHODS: a cross-sectional survey of community-dwelling women aged 55-93 years old (n = 609, mean age 70). Women attendees of

Validation of the Finnish Care register for Health Care diagnoses for preeclampsia, gestational diabetes and preterm delivery.

BACKGROUND: Centrally collected Finnish national health register data on adverse pregnancy outcomes are available for research, but the validity of the data is largely unknown. Our aim was to compare the diagnoses of preeclampsia (PE), gestational diabetes (GDM), and preterm delivery from hospital records with the registry based diagnoses from the Finnish Care Register for Health Care (FCR). Data on gestational age at delivery from the Medical Birth Registry (MBR) was also studied. METHODS: The Finnish Genetics of Pre-eclampsia Consortium (FINNPEC) Study cohort was used as a data source. Each

Kidney donor profile index and post-transplant health care utilization: Implications for value of transplant care delivery.

BACKGROUND: Centers discard high kidney donor profile index (KDPI) allografts, potentially related to delayed graft function and prolonged hospital use by kidney transplant recipients (KTR). We sought to determine whether high KDPI KTRs have excess health care utilization. METHODS: We conducted a retrospective cohort study from a high-volume center analyzing KTRs from January 3, 2011 to April 12, 2015 (n = 652). We measured differences in hospital use, emergency visits, and outpatient visits within the first 90 days between low (≤85%) versus high KDPI (>85%) KTRs, as well as long-term graft f

Delivery of mental health care for Black university students: a scoping review protocol.

OBJECTIVE: The objective of this review is to map the delivery of mental health care to Black university students formally enrolled in undergraduate and postgraduate courses at public or private higher-education institutions. INTRODUCTION: The prevalence of common mental disorders among Black university students is higher than their non-Black peers. Although there is evidence of the influence of ethnic-racial issues on mental health care for this group, mapping and identifying existing gaps will support actions and programs aimed at reversing racial inequities in mental health. ELIGIBILITY C

Patient and Health Care Provider Factors Associated With Prescription of Opioids After Delivery.

OBJECTIVE: To identify patient and health care provider characteristics associated with receipt of a high amount of prescribed opioids at postpartum discharge. METHODS: This was a retrospective case-control study of all opioid-naïve women delivering at a single, high-volume tertiary care center between December 1, 2015, and November 30, 2016. Inpatient, outpatient, pharmacy, and billing records were queried for clinical, prescription, and health care provider (training, age, gender) data. The discharging health care provider, whether an opioid prescription was provided, and the details of any

Health care delivery, zidovudine use, and survival of women and men with AIDS.

The aim of this study was to define predictors of survival for women and men after AIDS diagnosis. We examined health care delivery and drug therapy in the year before AIDS diagnosis for continuously enrolled New York State Medicaid beneficiaries with AIDS in 1988-1990. We examined the association of these factors with survival after AIDS diagnosis. Of 1,077 women and 1,871 men, 60% of both gender groups were drug users. In both risk groups, women had more outpatient visits than men but were equally likely to visit an AIDS specialist. In those who were not drug users, men were twice as likely

The Impact of Hurricane Harvey on Home Health Care Delivery for Medicare Beneficiaries.

Hurricanes can profoundly affect the delivery of home health (HH) services. We examined the impact of Hurricane Harvey on HH care in Texas, linking 2017-2018 patient-level Medicare HH claims with county-level data on disaster declarations from the Federal Emergency Management Agency. We used segmented interrupted time series to estimate monthly HH use changes among 142,416 beneficiaries in affected versus non-affected counties immediately after landfall (level change) and in its continued aftermath (trend change). HH nursing use dropped 16% immediately following hurricane and continued to decr

Effects of a Standardized Community Health Worker Intervention on Health Care Utilization Within an Integrated Delivery System.

BACKGROUND: Community health worker (CHW) interventions can improve health outcomes and reduce acute care utilization. Few prior studies have examined the association of CHW interventions with health care utilization among patients within an integrated health system. OBJECTIVE: To evaluate the effects of Individualized Management for Patient Centered Targets (IMPaCT), a standardized CHW intervention originally developed within a single health system in Philadelphia, PA, on acute care utilization and primary care engagement among low-income patients at two clinics within an integrated health s

Behavioral Health Care Delivery Through Street Medicine Programs in California.

Mental health and substance use disorders are prevalent among people experiencing homelessness. Street Medicine can reach unhoused people who face barriers to accessing healthcare in more traditional medical settings including shelter-based clinics. However, there is little guidance on best practices for mental health and substance use treatment through Street Medicine. The aim of the study was to describe behavioral health care through Street Medicine by analyzing data from the California Street Medicine Landscape survey and follow-up qualitative interviews. Most street medicine programs util

Interventional Mental Health: A Transdisciplinary Approach to Novel Psychiatric Care Delivery.

Mental health disorders are among the most common health conditions in the United States. Traditional clinical treatments rely on psychiatric counseling and, in many cases, prescription medications. We propose an innovative model, Interventional Mental Health, which employs a combination of modalities through a multifaceted approach to treat conditions that have exhibited limited responsiveness to traditional methods and individuals afflicted with multiple comorbidities simultaneously. We hypothesize that creating a unique treatment algorithm combining current therapeutic modalities such as St

Subspecialty emergency room as alternative model for otolaryngologic care: implications for emergency health care delivery.

PURPOSE: A dedicated otolaryngology emergency room (ER) represents a specialized surgical evaluation and treatment setting that may be an alternative triage pathway for acute otolaryngologic complaints. We aim to characterize practice patterns in this setting and to provide insight into the epidemiology of all-comer, urgent otolaryngologic complaints in the United States. METHODS AND METHODS: Electronic medical records were reviewed for all patients who registered for otolaryngologic care and received a diagnosis in the Massachusetts Eye and Ear Infirmary ER between January 2011 and September

Children's health outcomes in six different ambulatory care delivery systems.

This study assesses health outcomes across six widely varying delivery settings within one population sample, describing differences between actual and expected illness prevalences among children using each type of setting as a regular source. Illness measures are from independent clinical examinations of the children for anemia, acute and chronic ear problems, hearing loss and vision disorders. Comparisons of actual with expected prevalences based on each setting's patient clientele reveal three systematic patterns: children using solo practitioners had generally higher-than-expected illness

Organisation of health services for the delivery of primary health care in the WHO African region: a future perspective.

Despite decades of investment in health systems across the WHO African region, population health outcomes remain suboptimal. The region faces evolving challenges, including demographic shifts, emerging health threats, and persistent inequalities. Current health-service delivery models are misaligned with anticipated future health demands, necessitating a reimagined operational framework grounded in a revitalised primary health-care approach. In this Viewpoint, we draw on expert consensus from professionals across 19 countries using the nominal group technique and Delphi-style rounds. Experts w
