Shifting the Paradigm: CAMBRA Adoption and Medical-Dental Integration Leveraging an Electronic Health Record.

PURPOSE/OBJECTIVES: This study describes the integration of Caries Management by Risk Assessment (CAMBRA) into a medical-dental integrated Epic electronic health record (EHR) at Columbia University's College of Dental Medicine (CDM), with the goal of incorporating risk-based caries management into predoctoral clinical care. METHODS: A multi-phase, mixed-methods approach was used. CAMBRA protocols were embedded into the Epic EHR, including clinical decision support, automated scoring, integration with prescribing and documentation workflows, and patient education after visit summary (AVS). Weaning, parturitions and illnesses are recorded in rhesus macaque (Macaca mulatta) dental cementum microstructure.

Many open questions in evolutionary studies relate to species' physiological adaptations, including the evolution of their life history and reproductive strategies. There are few empirical methods capable of detecting and timing physiologically impactful events such as weaning, parturition and illnesses from hard tissue remains of either extant or extinct species. Cementum is an incremental tissue with post eruption annual periodicity, which covers the tooth root and functions as a recording structure of an animal's physiology. Here we test the hypothesis that it is possible to detect and time

Prediction of Sjögren's disease diagnosis using matched electronic dental-health record data.

BACKGROUND: Sjögren's disease (SD) is an autoimmune disease that is difficult to diagnose early due to its wide spectrum of clinical symptoms and overlap with other autoimmune diseases. SD potentially presents through early oral manifestations prior to showing symptoms of clinically significant dry eyes or dry mouth. We examined the feasibility of utilizing a linked electronic dental record (EDR) and electronic health record (EHR) dataset to identify factors that could be used to improve early diagnosis prediction of SD in a matched case-control study population. METHODS: EHR data, including

Documented caries treatment and prevention among highly caries-active adults: a retrospective observational study of public dental records.

BACKGROUND: Evidence-based caries management emphasises identification of disease drivers and causal, preventive, and behaviour-oriented care. However, limited evidence is available on how such care is documented for adults with high caries activity in routine public dental care. This study examined documentation of caries treatment and preventive measures consistent with evidence-based caries management, and whether documented care addressed underlying causes of caries disease. METHODS: This retrospective observational record-review study included 418 adults aged 25-65 years from 19 Public D

Ease and practicability of the 2017 classification of periodontal diseases and conditions: a study of dental electronic health records.

BACKGROUND: A new classification for Periodontal and Peri-implant Diseases and Conditions was introduced in the 2017 World Workshop. In the past the 1999 Armitage Classification was commonly used in practice. This study aimed to assess the ease and practicability of retroactively diagnosing a subset of patients formerly diagnosed using the 1999 AAP/CDC classification with the 2017 AAP/EFP disease classification. METHODS: A random subset of 10% of all patients referred over a 7-year period (2011-2018) to the Post-Doctoral Periodontics Clinic at Columbia University College of Dental Medicine we

Recording compliance of dental splint use in obstructive sleep apnoea patients by force and temperature modelling.

Fibre-optic sensors are used to monitor the force and temperature of dental splints worn by patients suffering from sleep apnoea. Owing to the small size of the sensors, they can be easily embedded within the splint in a way that does not affect the effectiveness of the splint, and, at the same time, are able to indicate whether the splint has been properly worn by the patient. The overall dimensions of the sensor are approximately 0.375 mm thickness, 1 cm length and 3 mm width. The force and temperature sensors are calibrated and found to have sensitivities of better than 0.5 N and 0.1 degree

Comparing gingivitis diagnoses by bleeding on probing (BOP) exclusively versus BOP combined with visual signs using large electronic dental records.

The major significance of the 2018 gingivitis classification criteria is utilizing a simple, objective, and reliable clinical sign, bleeding on probing score (BOP%), to diagnose gingivitis. However, studies report variations in gingivitis diagnoses with the potential to under- or over-estimating disease occurrence. This study determined the agreement between gingivitis diagnoses generated using the 2018 criteria (BOP%) versus diagnoses using BOP% and other gingival visual assessments. We conducted a retrospective study of 28,908 patients' electronic dental records (EDR) from January-2009 to De

Analysis of oxygen saturations recorded during dental intravenous sedations: a retrospective quality assurance of 3500 cases.

The death of a patient under sedation in New South Wales, Australia, in 2002 has again raised the question of the safety of dental sedation. This study sought answers to 2 questions: Can safe oxygen saturation levels (≥ 94%) be consistently maintained by a single operator/sedationist? Does the additional use of propofol, in subanesthetic doses, increase the risk of exposure to hypoxemia? Three thousand five hundred cases generated between 1996 and 2006 were randomly examined and divided into 2 subcohorts: 1750 patients were sedated with midazolam and fentanyl, and 1750 patients received propof

Fabrication of a reverse-engineered custom scan body as a digital solution for recording implant position: A dental technique.

A technique for the reverse engineering of the implant-abutment connection to fabricate a custom scan body is described. The implant-abutment connection was designed using the exocad software program, the scan body with screw channel was designed with the Blender software program, and the file was either 3-dimensionally printed in definitive tooth-colored resin with ceramic filler material or milled in polyetheretherketone (PEEK). This technique offers an accurate, cost-effective digital solution for implant optical scanning that can replace prefabricated scan bodies that may not be available

Caries occurrence in a fluoridated and a nonfluoridated town in Finland: a retrospective study using longitudinal data from public dental records.

The tap water of Kuopio, Finland, was fluoridated from 1959 to 1992. In the first decade of fluoridation, children in Kuopio had lower DMF values than children in Jyväskylä, a nearby low-fluoride town, but later differences between the towns have been small and inconsistent. The present study aimed to gain further insight into caries occurrence in Kuopio and Jyväskylä using longitudinal tooth-specific data from public health records on cohorts born in 1970/71 and 1980/81 (total n = 1,503). Survival analyses were used to summarize the tooth-specific times elapsed between eruption and the first

Developing and testing a prediction model for periodontal disease using machine learning and big electronic dental record data.

Despite advances in periodontal disease (PD) research and periodontal treatments, 42% of the US population suffer from periodontitis. PD can be prevented if high-risk patients are identified early to provide preventive care. Prediction models can help assess risk for PD before initiation and progression; nevertheless, utilization of existing PD prediction models is seldom because of their suboptimal performance. This study aims to develop and test the PD prediction model using machine learning (ML) and electronic dental record (EDR) data that could provide large sample sizes and up-to-date inf
