The atrophic edentulous ridge represents one of the greatest challenges faced in general practice. An estimated 20 million adults in the U.S. are edentulous in at least one arch and a significant number of these patients experience problems with their prostheses from alveolar bone loss. Many of these patients, for various reasons, are not candidates for endosseous dental implants. Alveolar ridge augmentation is a viable option in many of these cases; however, this procedure has been unpredictable because of particle migration. Presented here is a method of ridge augmentation with dense hydroxy

OBJECTIVE: The aim of this prospective pilot study was to evaluate the 15-year clinical performance of mandibular overdentures supported by a single midline implant in edentulous patients.
METHODS: Eleven eligible patients were enrolled. A single implant was inserted in the midline of the mandible (Root Line; Promote, Camlog Biotechnologies, diameter 3.8 mm, length 11-13 mm). In 6 patients, a non-submerged protocol with moderate immediate loading on healing abutments was applied. The submerged healing protocol was followed in the other five patients with uncovering taking place two months aft

The purpose of this study was to examine if a modified direct relining method could result in excellent denture adaptation compared to a conventional method. Direct relining was done using heat-polymerized acrylic resin base on a maxillary cast with two times of three seconds of cooling using dichloro-difluoro-methane coolant. Discrepancy between cast and intaglio surface at residual ridge and center of palate was measured at 12 minutes after mixing of resin (Time 0) and at 24 hours or seven days after water immersion (36 degrees C). At Time 0 (with three seconds of cooling), the discrepancy a

Background and Objectives: Despite the identified benefits of early implant loading, studies have questioned its advantages compared to delayed loading in edentulous patients. This study aimed to evaluate clinical peri-implant parameters and marginal bone loss around early placed and loaded mandibular implant overdentures with a 60-month follow-up. Materials and Methods: In this prospective cohort study, 43 patients were enrolled to receive 86 early loading sub-crestal dental implants through prosthetic guides. Implant overdentures were supported by two isolated implant locator attachments bet

The purpose of this article is to identify the types of complications that have been reported in conjunction with endosseous root form implants and associated implant prostheses. A Medline and an extensive hand search were performed on English-language publications beginning in 1981. The searches focused on publications that contained clinical data regarding success/failure/complications. The complications were divided into the following 6 categories: surgical, implant loss, bone loss, peri-implant soft tissue, mechanical, and esthetic/phonetic. The raw data were combined from multiple studies

STATEMENT OF PROBLEMS: Research-based assessments of the quality of removable partial dentures (RPDs), especially within a large patient population, are lacking.
PURPOSE: This study evaluated the prevalence and quality of RPDs by use of the National Health and Nutrition Survey (NHANES III) data set.
MATERIAL AND METHODS: Estimates of the health and nutritional status of the American population were obtained from the NHANES III data set (1988-1994). Calibrated dentists performed dental examinations on 17,884 adults. If the subject wore an RPD, its quality was assessed with 5 criteria: integri

STATEMENT OF PROBLEM: The clinical success of implant-retained, tissue-supported mandibular overdenture prostheses has been documented. However, few studies have evaluated the return visits and associated cost required to maintain these prostheses.
PURPOSE: The purpose of this investigation was to evaluate the amount of maintenance required to provide acceptable and satisfactory implant-retained mandibular overdentures in a prospective clinical trial.
MATERIAL AND METHODS: Fifty-eight patients received new maxillary and mandibular complete dentures followed by placement of microthreaded/TiOB

This study evaluated the treatment needs of 44 overdenture patients who had worn their dentures for at least 5 years. There were 135 abutment teeth involved, 35 in the maxillae and 100 in the mandible. Recall rates varied from a low of 27.6% to a high of 73.7%. The attrition rate was 20.5% after 5 years. The caries rate varied from a low of 2.7% to a high of 20.6%, and periodontal treatment was required by 94% of the subjects after 5 years. Only 39.4% of the subjects required adjustments, repairs, relines, or remakes of their overdentures during this time. The study showed that the success of

Clinicians reline or replace dentures to improve occlusion, stability, retention and facial support. They believe that well-fitting dentures will maintain supporting tissues in good health and satisfy patients by improving oral function and self-esteem. In this study, satisfaction of 21 patients with poorly fitting dentures was assessed before and after the dentures were modified to improve occlusion, vertical dimension of occlusion, and retention and stability or the dentures were replaced with new ones. More than 55% of the patients were moderately to fully satisfied with their poorly fittin

The hypothesis of this 5-y randomized clinical trial was that there would be no significant difference in the satisfaction of edentulous participants with removable complete overdentures attached to 1 or 2 mandibular implants. Secondary aims were to test changes in satisfaction between and within the groups from baseline to 5 y and differences between the groups in implant survival and prosthodontic maintenance over 5 y. Each of the 86 participants (mean age, 67 y) was randomly allocated to receive either 1 implant in the midline (group 1) or 2 implants in the canine areas (group 2) attached t

Patients afflicted with congenital/developmental anomalies of the oral/head and neck regions present the prosthodontist with unique esthetic and functional restorative challenges. This article reviews the various pediatric patient categories that would benefit from placement of "vital" overdentures. A long-term follow-up of patients seen over 25 years of practice is presented with a focus on complications such as temporomandibular joint stability, caries, soft tissue and bony support changes, and denture adaptation. Longitudinal follow-up of these patients indicates that, although problems occ

The present study aims to investigate changes in the occlusion of complete dentures after their insertion. A total of 85 edentulous patients were provided with new complete dentures. Their individual hinge axes were determined using mechanical axiography and the upper finished dentures were transferred by facebows to Dentatus articulators. The lower dentures were mounted according to an intraoral central bearing point (CBP) registration and equilibrated in terminal hinge position. An articulator specially modified for measurements in the condylar area was used. The differences between the posi

Numerous articles report on the unmet dental needs of the older patient, yet few have suggested strategies to overcome barriers to care for this unique demographic. At a time when prosthodontic services, including dental implants, offer improved outcomes, the elderly remain the least served of all patient groups. Obstacles to treatment include ageism, medical and dental complexity, financial limitations, and patients' resistance to treatment. Expanding prosthodontic care of the elderly requires a departure from traditional patient management techniques. The attempt to reverse ageism in your pr

PURPOSE: The purpose was to assess and compare self-reported oral health and oral and general health related quality of life (OHRQoL and HRQoL) in two groups of edentulous adults who reported dissatisfaction with their mandibular dentures, and who were treated with a conventional relining of this denture or by having it converted into an implant-retained one.
MATERIALS AND METHODS: Sixty subjects were randomly allocated into two equal groups, a relined conventional denture (RCD) group and an implant-retained overdenture (IOD) group. Data on demographics, oral health, OHRQoL, and HRQoL were re

BACKGROUND: The present study evaluated the efficacy of a treatment consisting of placing and immediately loading implants with a bar-retained overdenture in edentulous maxillae.
METHODS: Twenty-two consecutive patients were treated with four or five implants rigidly connected with a bar, which were then loaded with a maxillary overdenture within 48 hours post-surgery. The patients were followed clinically and radiographically for 1 year after loading. The implant outcome with regard to survival and success was analyzed. Visual analog scale questionnaires were used to record patient function

PURPOSE: The aim of this retrospective study was to evaluate implant survival/success rates and peri-implant parameters as well as patient satisfaction for uniformly designed implant-supported anterior maxillary or mandibular fixed partial dentures (FPDs).
MATERIALS AND METHODS: A retrospective study was conducted on patients with maxillary or mandibular anterior partial edentulism (all incisors missing) treated between 2002 and 2006 with a two-implant-supported four-unit FPD. All FPDs were of the same design: two implant abutments in the lateral incisor positions and two ovate pontics in the

OBJECTIVES: To evaluate the outcome of immediately loaded distally cantilevered mandibular full-arch prostheses according to the 'all-on-four' concept supported by implants placed in both fresh extraction and healed sites.
MATERIAL AND METHODS: A prospective study was conducted in 24 patients with extraction of all remaining mandibular teeth and placement of 4 implants per patient (2 mesial axial and 2 distal tilted) for full-arch mandibular restorations. Implants were inserted in fresh extraction sockets 2.3 ± 1.0 per patient and 1.7 ± 1.0 implants in healed sites. Implants placed in fresh e

Medication-related osteonecrosis of the jaw (MRONJ) is a severe adverse drug reaction, consisting of progressive bone destruction in the maxillofacial region of patients. ONJ can be caused by two pharmacological agents: Antiresorptive (including bisphosphonates (BPs) and receptor activator of nuclear factor kappa-B ligand inhibitors) and antiangiogenic. MRONJ pathophysiology is not completely elucidated. There are several suggested hypothesis that could explain its unique localization to the jaws: Inflammation or infection, microtrauma, altered bone remodeling or over suppression of bone resor

Medication-related osteonecrosis of the jaw (MRONJ) is an uncommon complication of antiresorptive therapy (ART) in patients receiving higher and more frequent doses of osteoclast inhibitors. The jaws are the most common site, as they have high bone turnover. The oral structures are exposed to various types of stresses, like mastication and dental diseases, which lead to microtrauma and increased bone remodeling. The hallmark feature of MRONJ is the area of exposed, necrotic, nonhealing, asymptomatic bone for more than eight weeks. Objective signs are pain in the jaw and oral cavity, loose teet
