[Descemet membrane endothelial keratoplasty in complex eyes].

BACKGROUND: Descemet membrane endothelial keratoplasty (DMEK) has become the standard surgical treatment for Fuchs' endothelial corneal dystrophy (FECD) in Germany. In addition to classical FECD, there are endothelial disorders amenable to DMEK associated with complex changes in the anterior segment of the eye, such as anterior synechia of the iris, larger iris defects, after glaucoma surgery, after vitrectomy and after trauma. OBJECTIVE: This article provides an overview of the possibilities to treat patients with complex anterior segment situations in the presence of corneal endothelial dec

Histopathology of Failed Descemet Membrane Endothelial Transfer.

OBJECTIVES: To describe the postmortem histologic features after an unsuccessful Descemet membrane endothelial transfer (DMET) and assess any potential clinical implications. METHODS: Postmortem, an eye from a patient who previously underwent unsuccessful DMET for pseudophakic bullous keratopathy (PPBK) was harvested and processed for morphologic evaluation. RESULTS: Clinically and histologically, the host cornea showed evidence of diffuse stromal edema. Although the edges of the surgical descemetorhexis were well visualized, there was no evidence of endothelial migration or repopulation of

Chandelier Illumination for Descemet Membrane Endothelial Keratoplasty.

PURPOSE: To describe a simple technique that uses posterior chandelier illumination during Descemet membrane endothelial keratoplasty in cases of severe bullous keratopathy (BK). METHODS: Five eyes of 4 patients with advanced BK undergoing Descemet membrane endothelial keratoplasty were retrospectively analyzed. The pupil of the host eye was not treated with mydriatic or miotic agents. The chandelier illuminator was inserted transconjunctivally into the vitreous cavity from the pars plana. RESULTS: In all eyes, BK was secondary to laser iridotomy, which was performed for prevention or treatm

Hemorrhagic descemet membrane detachment after classic canaloplasty.

PURPOSE: Canaloplasty is a safe and effective alternative in glaucoma surgery, avoiding the risk for hypotony and bleb-related complications. Two cases of hemorrhagic Descemet membrane detachment (DMD) after canaloplasty are reported in patients who did not have previous surgery. RESULTS: Two patients with primary open-angle glaucoma underwent canaloplasty because of medically uncontrolled intraocular pressure (IOP). Canaloplasty was performed using a flexible microcatheter, viscoelastic material and a tensioning suture. The day after surgery, hemorrhagic DMD was observed in the inferior quad

[Refractive changes after Descemet membrane endothelial keratoplasty].

Penetrating keratoplasty has different refractive disadvantages in contrast to posterior lamellar keratoplasty. For example, a decentered corneal trephination and a tilted trephination or unevenly tightened corneal sutures can cause an uncontrolled high astigmatism as well as a refractive change. Also the postoperative refraction may change over time as a result of wound healing, suture loosening or suture removal. The aim of this retrospective study was to investigate a possible refractive change after Descemet membrane endothelial keratoplasty (DMEK). A total of 139 pseudophakic eyes from 12

[Clinical results after Descemet membrane endothelial keratoplasty].

BACKGROUND: Diseases of the endothelial cell layer represent a common indication for perforating keratoplasty. In recent years posterior lamellar keratoplasty techniques have undergone a revival. The latest and most promising advancement is the isolated transplantation of Descemet's membrane (DM) with the endothelial layer - also known as Descemet's membrane endothelial keratoplasty (DMEK). This study was conducted to evaluate the clinical results of our DMEK patients and to assess the perioperative management. PATIENTS AND METHODS: 70 patients (75 eyes) with endothelial cell decompensation (

Management of Descemet Membrane's Folds After Deep Anterior Lamellar Keratoplasty: Descemet Membrane-Tucking Technique.

PURPOSE: To describe a surgical maneuver that allows for correction of central Descemet membrane (DM) folds at the end of a deep anterior lamellar keratoplasty (DALK) procedure. We term the present technique "DM tucking." METHODS: A blunt tip spatula is introduced vertically into the trephination cut, 90 degrees away from the main direction of the DM folds, and advanced until it touches the host layer. Gentle pressure is applied, resulting in tucking of the redundant host layer toward the periphery. The tucking maneuver is repeated at different clock hours until a regular graft-host interface

Descemet stripping only and ripasudil for the treatment of traumatic Descemet's membrane ruptures.

PURPOSE: Descemet´s membrane ruptures (with a discontinuation of Descemet´s membrane and double detached coiled edges) in the context of complicated anterior segment surgery have rarely been described and its management can be challenging. We report a modified Descemet stripping only (DSO) technique associated with ripasudil drops to treat these cases when other techniques fail. METHODS: We describe two cases of large Descemet´s membrane detachments associated with Descemet´s ruptures after cataract surgery that did not respond to two SF6 intracameral injections. As the detached Descemet's me

Surgical repair of Descemet's membrane detachment.

The aim of the study was to demonstrate a successful repair of Descemet membrane detachment (DMD). A 73-year-old woman with pseudoexfoliative glaucoma underwent phacoemulsification with flexible posterior chamber intraocular lens implantation. Because of inadequate pupillary dilatation flexible iris retractors were used. Four days after cataract surgery, a large DMD was recognized. One day later, the patient underwent surgical repair. Three long full-thickness 10-0 nylon sutures were used to fixate DM to the cornea. The next day, DM was completely attached and the cornea was clear. Final best-

Stripping of Descemet's membrane: a clinicopathologic correlation.

We describe a case of accidental injection of sodium hyaluronate (Healon) anterior to Descemet's membrane that caused the membrane to become almost completely detached. The patient eventually underwent penetrating keratoplasty, permitting histopathologic study of the detachment. Sodium hyaluronate globules in the space between the posterior stroma and Descemet's membrane were demonstrated by light and electron microscopy. In addition, 111 consecutive cases of Descemet's membrane detachment identified pathologically were reviewed to identify potential predisposing risk factors and determine cor

Iatrogenic Descemet membrane detachment in the donor lenticule during Descemet stripping automated endothelial keratoplasty.

A female patient in her 60s presented with an 8-month history of decreased vision in her right eye. She was diagnosed with pseudophakic bullous keratopathy (PBK), and Descemet stripping automated endothelial keratoplasty (DSAEK) surgery was planned. Intraoperative Descemet membrane detachment (DMD) occurred while inserting the lenticule using the Sheets glide technique. The detachment of the Descemet membrane was detected immediately, and air descemetopexy was performed intraoperatively. Postoperatively, the donor lenticule was attached properly to the stromal bed, except for a focal DMD appro
