Oxidative stress in diabetic retinopathy.

Diabetic retinopathy (DR) remains a leading cause of blindness, with oxidative stress as a central pathogenic driver. However, the translation of this mechanistic knowledge into clinical diagnostics and targeted therapies has been hampered by inconsistent biomarkers and a simplistic view of redox balance. This critical review moves beyond cataloging oxidative markers to evaluate their biological specificity, analytical robustness, and compartmentalization between systemic circulation and the ocular microenvironment. We dissect the technical challenges of direct reactive species measurement and

Animal models of diabetic retinopathy.

The retina is the posterior neuro-integrated layer of the eye that conducts impulses induced by light to the optic nerve for human vision. Diseases of the retina often leads to diminished vision and in some cases blindness. Diabetes mellitus (DM) is a worldwide public health issue and globally, there is an estimated 463 million people that are affected by DM and its consequences. Diabetic retinopathy (DR) is a blinding complication of chronic uncontrolled DM and is the most common cause of blindness in the United States between the ages 24-75. It is estimated that the global prevalence of DR w

Diabetic Retinopathy: Pathophysiology and Treatments.

Diabetic retinopathy (DR) is the most common complication of diabetes mellitus (DM). It has long been recognized as a microvascular disease. The diagnosis of DR relies on the detection of microvascular lesions. The treatment of DR remains challenging. The advent of anti-vascular endothelial growth factor (VEGF) therapy demonstrated remarkable clinical benefits in DR patients; however, the majority of patients failed to achieve clinically-significant visual improvement. Therefore, there is an urgent need for the development of new treatments. Laboratory and clinical evidence showed that in addi

Management of proliferative diabetic retinopathy.

Proliferative diabetic retinopathy is characterized by neovascularization originating from the retina and/or optic disk in patients with diabetes mellitus. The role of vascular endothelial growth factor appears to be central in the pathogenesis of proliferative diabetic retinopathy. Advanced glycation end products are important in the development of vitreous abnormalities in proliferative diabetic retinopathy. The majority of the neovascular membranes are adherent to the posterior vitreous cortex. When the posterior hyaloid exerts traction, the edges of the neovascular complex are pulled forwa

Cataract surgery and diabetic retinopathy.

Diabetes is a risk factor for the development of cataracts. Studies have shown an increased risk of ocular complications in diabetics after cataract surgery, but modern surgical techniques have minimized them, leading to an overall good visual outcome. Macular edema before surgery is the most common condition that limits post-operative visual recovery. Thus, pre-operative laser treatment is needed. Photocoagulation of preproliferative or early proliferative diabetic retinopathy is also advisable, due to the increased risk of iris neovascularization or retinopathy progression after surgery.

[Medical treatment of diabetic retinopathy].

The pathogenesis of diabetic retinopathy (DR) consists of a complex patch-work of factors correlated and independent on one another at the same time. It is therefore difficult to find pharmaceuticals able to produce different, or even mutually contrasting effects. Another difficulty are the intrinsic characteristics of the disease which after developing to a "critical" stage, evolves with independent mechanism to the most advanced stages. In spite of that, a great deal of medicaments has been suggested so far, all providing therapeutical logic to interfere with one or more pathogenetical stage

Exogenous estrogen exposures and changes in diabetic retinopathy. The Wisconsin Epidemiologic Study of Diabetic Retinopathy.

OBJECTIVE: To investigate whether the use of exogenous estrogen is associated with changes in the severity of diabetic retinopathy and the incidence of macular edema. RESEARCH DESIGN AND METHODS: The study design involved observation of two well-defined cohorts of women with diabetes. One group was diagnosed with diabetes at < 30 years of age and used insulin (younger-onset group), and the other group was diagnosed at > or = 30 years of age with no criteria regarding therapy (older-onset group). Subjects received standard examinations, medical interviews, and retinal photography in 1980-1982.

Serum cystatin C as a biomarker for diabetic retinopathy and its role in diabetic retinopathy-diabetic kidney disease comorbidity.

BACKGROUND: To investigate whether serum cystatin C (CysC) can serve as a biomarker for diabetic retinopathy (DR), and explore the comorbidity between DR and diabetic kidney disease (DKD). METHODS: This study used a nested case-control design with Mendelian randomization (MR) analysis. A total of 128 type 2 diabetic patients were categorized into no DR (DM), non-proliferative DR (NPDR), and proliferative DR (PDR) groups with age- and sex- matched controls. Biochemical and renal biomarkers were assessed, including the absolute difference (abdiff) eGFRabdiff (eGFRCysC-eGFRScr) and relative diff

Current epidemiology of diabetic retinopathy and diabetic macular edema.

With increasing global prevalence of diabetes, diabetic retinopathy (DR) is set to be the principle cause of vision impairment in many countries. DR affects a third of people with diabetes and the prevalence increases with duration of diabetes, hyperglycemia, and hypertension-the major risk factors for the onset and progression of DR. There are now increasing data on the epidemiology of diabetic macular edema (DME), an advanced complication of DR, with studies suggesting DME may affect up to 7 % of people with diabetes. The risk factors for DME are largely similar to DR, but dyslipidemia appea
