The DSM-5 Section III alternative model of personality disorder (AMPD) and the International Classification of Diseases - 11th Edition's (ICD-11) personality disorder classification allow clinicians to identify individual trait domains in which people score highly. However, how these domains relate to constructs associated with efficacious treatment approaches is unclear. The current study aimed to determine whether constructs from two evidence-based treatments (schema therapy [ST] and dialectical behavior therapy [DBT]) were associated with maladaptive personality traits in a way consistent w

OBJECTIVE: This study focused on the implementation of dialectical behavior therapy (DBT), an evidence-based treatment for youths with nonsuicidal self-injury and suicidal ideation, in a psychiatric residential treatment center (RTC) for children and adolescents who have complex psychological, psychiatric, substance use, and developmental disorders.
METHODS: In a pilot hybrid effectiveness-implementation trial, the authors compared functioning scores of adolescent patients who received DBT (N=113) with the scores of those who received treatment as usual (N=114) in an RTC. Data were collected

OBJECTIVE: There is limited research examining factors associated with Dialectical Behavior Therapy (DBT) treatment retention among veterans, who are of critical interest given higher rates of suicide and borderline personality disorder (BPD). This study examined factors related to DBT treatment retention and BPD symptom trajectories among veterans.
METHODS: Participants were veterans enrolled in a comprehensive DBT program. 145 veterans presented to the program, and 115 veterans were deemed eligible and enrolled. The relationships between mental health care utilization, suicide-related facto

OBJECTIVE: We examined whether the emotions that clients experience within session are associated with treatment outcome in dialectical behavior therapy (DBT) for borderline personality disorder (BPD).
METHOD: Participants were 52 adults who met criteria for BPD and were enrolled in a 12-month DBT treatment. The Classification of Affective-Meaning States, an observer-rated measure of discrete emotions, was used to code videos of individual DBT sessions. Raters coded three psychotherapy sessions for each participant: one session from each of the early, working, and late phases of psychotherapy

Existing literature on the effects of borderline personality disorder (BPD) and eating disorder (ED) comorbidity in terms of clinical presentation and treatment outcome has been limited and inconclusive. The present study examined whether clients with EDs and varying levels of BPD symptoms presented with more severe ED symptoms at admission, and whether they responded to dialectical behavior therapy (DBT)-based treatment. Participants (N = 176) were adults in a DBT-based partial hospitalization program for EDs at an academic medical center. Participants completed self-report measures at admiss

A comprehensive Dialectical Behavior Therapy (DBT) program was created within a VA Health Care System for patients with recent psychiatric hospitalization, suicidality and/or significant emotion dysregulation. The program was notable for being one of a relatively small number of comprehensive DBT programs in the VA system, and for including patients with psychosis and psychotic disorder, with a majority of patients (58%) having a documented history of psychosis or endorsing psychotic symptoms in assessments. We describe the process of creating this program at a VA medical center and present pr

Neural underpinnings of emotion dysregulation in borderline personality disorder (BPD) are characterized by limbic hyperactivity and disturbed prefrontal activity. It is unknown whether neural correlates of emotion regulation change after a psychotherapy which has the goal to improve emotion dysregulation in BPD, such as dialectical behavioral therapy (DBT). We investigated distraction as a main emotion regulation strategy before and after DBT in female patients with BPD. Thirty-one BPD patients were instructed to either passively view or memorize letters before being confronted with negative

Although one of the main aims of dialectical behavior therapy (DBT) for borderline personality disorder (BPD) is to increase the retention rates, premature termination rates for DBT inpatient programs were found to be over 30%. The aim of the study was to identify the reasons for, and to analyze, patient characteristics that are associated with premature termination. We studied 541 inpatients with BPD, who were consecutively admitted for an open-door 3-month DBT inpatient treatment in Berlin, Germany. All participants completed several self-rating measures and participated in clinical intervie

OBJECTIVE: At present, the most frequently investigated psychosocial intervention for borderline personality disorder (BPD) is dialectical behavior therapy (DBT). We conducted a meta-analysis to examine the efficacy and long-term effectiveness of DBT.
METHOD: Systematic bibliographic research was undertaken to find relevant literature from online databases (PubMed, PsycINFO, PsychSpider, Medline). We excluded studies in which patients with diagnoses other than BPD were treated, the treatment did not comprise all components specified in the DBT manual or in the suggestions for inpatient DBT pr

There are few published randomized controlled trials examining treatment for symptoms of bulimia nervosa (BN) in adolescents. Additionally, many adolescents presenting for treatment for BN symptoms endorse co-occurring mood disturbances, suicidality, and nonsuicidal self-injury (NSSI), and may not meet full Diagnostic and Statistical Manual-IV-Text Revision (DSM-IV-TR) diagnostic criteria for BN. In addition to the limited number of randomized controlled trials, published treatment studies of BN symptoms in adolescence do not specifically address the multiple comorbid symptoms that these adole

Obsessive-compulsive personality disorder (OCPD) is commonly comorbid with major depressive disorder, and their comorbidity is associated with worsened outcomes and treatment refraction. Radically open dialectical behavior therapy (RO DBT) is a novel transdiagnostic psychosocial intervention. RO DBT targets OCPD-related traits (e.g., maladaptive perfectionism, inflexibility) and has been theorized as an OCPD treatment. This study examined RO DBT efficacy for treatment-refractory depression (TRD), with and without comorbid OCPD. RefraMED was a multisite randomized controlled trial of treatment

Bipolar disorder and borderline personality disorder commonly co-occur. Each disorder is associated with substantial morbidity and mortality, which are worsened by co-occurrence of the disorders. Emotional dysregulation, suicidality, and disrupted circadian rhythm are key aspects of psychopathology associated with both conditions. A novel psychotherapy combining elements of two evidence-based treatments (i.e., dialectical behavior therapy [DBT] for borderline personality disorder and social rhythm therapy [SRT] for bipolar disorder) is described. Unlike either treatment alone, the new therapy,

Objective: Research has shown that dialectical behavior therapy (DBT) is an effective treatment modality to decrease suicidal ideation, incidents of nonsuicidal self-injurious behavior, and suicide attempts in an adolescent outpatient population. However, research on the impact of DBT on depressive and manic symptoms for adolescents and within an inpatient setting is limited. The purpose of this study is to examine whether DBT significantly decreases depressive and manic symptoms compared with treatment as usual (TAU) for inpatient adolescents. Method: We conducted a retrospective chart review

Although binge eating disorder (BED) is a recent diagnostic category, research for efficacious and effective treatment is well underway. This case study describes a dialectical behavior therapy (DBT) skills group for BED implemented in an outpatient community clinic. Although based on Safer, Telch, and Chen's (2009) manual for BED and bulimia nervosa (BN), notable adaptations included shortening the group's duration from 20 to 12 weeks, adding an interpersonal effectiveness module and DBT-informed "Holiday Plan" worksheets, and providing inclement weather alternatives. Despite the added challe

Inpatient dialectical behavior therapy (DBT) is an effective treatment for borderline personality disorder (BPD), but often treatment is ended prematurely and predictors of dropout are poorly understood. We, therefore, studied predictors of dropout among 60 women with BPD during inpatient DBT. Non-completers had higher experiential avoidance and trait anxiety at baseline, but fewer life-time suicide attempts than completers. There was a trend for more anger-hostility and perceived stigma among non-completers. Experiential avoidance and anxiety may be associated with dropout in inpatient DBT. L

BACKGROUND: Trauma, acculturation stressors, and discrimination are among many factors that place Latinx children and adolescents living in the United States at high risk for mental health distress and suicidal behavior. Despite these well-documented risk factors, mental health providers struggle to continuously engage Latinx youth and families in mental health care, including evidence-based treatments for addressing suicidal behaviors such as Dialectical Behavior Therapy for Adolescents (DBT-A). One barrier to engagement is access to such effective treatments.

OBJECTIVE: The authors compared the efficacy of 6 months of dialectical behavior therapy (DBT) to 6 months of selective serotonin reuptake inhibitors with clinical management (SSRI/M) in reducing suicide-related events (SREs), suicide attempts, nonsuicidal self-injury (NSSI), and related clinical outcomes in individuals diagnosed with borderline personality disorder.
METHODS: The study sample comprised 84 individuals with borderline personality disorder; more than one suicide attempt, SRE, or NSSI episode in the past 6 months; and another of these behaviors in the past year. Two-thirds had co

BACKGROUND: Suicide is a leading cause of death and significant concerns for adolescents and young adults. Dialectical Behavior Therapy for Adolescents (DBT-A) is an evidence-based intervention that effectively reduces life-threatening behaviors. Limited research examines the impact of adding individualized skills coaching sessions (ISCS) to the DBT-A model, a novel intervention referred to in this paper as "Enhanced DBT-A." This study evaluates the effectiveness of Enhanced DBT-A in reducing suicidality over 1 year of treatment in an outpatient and intensive outpatient setting.

OBJECTIVE: Borderline Personality Disorder (BPD) is characterized by extreme emotional dysregulation. Dialectical Behavioral Therapy (DBT) is an empirically supported and effective treatment for BPD in adolescents and adults. However, the extent to which exposure to childhood abuse (i.e. physical and sexual abuse) impacts treatment outcomes is not well understood. This open trial investigated the effectiveness of an intensive residential DBT program in reducing emotion dysregulation in adolescents with BPD symptoms, as well as the impact that severity and type of childhood abuse had on treatme

We present the case of an adult male ("Jake") with chronic Anorexia Nervosa, buttressed by dysfunctional levels of perfectionism and aggravated by long-standing mood and anxiety disturbance, successfully treated with Radically Open Dialectical Behavior Therapy (RO DBT). RO DBT is an evidence-based, transdiagnostic psychotherapy designed to address perfectionistic overcontrolled coping through teaching flexibility, openness, and healthy self-doubt. We illustrate this treatment approach by means of describing its application to this case, including discussing core RO DBT treatment strategies and
