Anterior tooth replacement has always posed an esthetic challenge for dentists. A multidisciplinary approach makes it possible to provide patients with esthetics and function. Treatment planning should involve other specialists when clinicians are faced with the additional esthetic concerns of the patient (such as anterior spatial anomalies and esthetically unpleasant existing restorations). Metal-ceramic fixed partial dentures have been used to allow the replacement of missing teeth for many years, providing both strength and esthetics. Several other options also are available, including impl

Midline diastemas are one of the most prevalent dental malocclusions. In young adults this may create aesthetics problems, especially in individuals marked by a gap between central incisors higher than 4 mm. This case report demonstrates the treatment of a patient with Angle's Class I malocclusion and midline diastema with the M-spring appliance resulting in complete closure within four months. The case included a frenectomy for correction of the abnormal labial frenal attachment followed by an M-spring orthodontic appliance. This "M" shaped device, with specific activation points, provides co

Determining the appropriate technique for diastema closure is challenging, and the decision must be evidence based. The objective of these case reports is to describe different approaches to diastema closure using direct and indirect techniques, focusing on the characteristics of the patient and clinical requirements to guide treatment. In the first case, a 16-year-old patient had multiple diastemas in the maxillary anterior dentition. The clinical evaluation revealed microdontia of the lateral incisors and malpositioned teeth. The treatment included tooth whitening and placement of composite

The fracture of restorations used for diastema closure is a clinical concern. The objective of this study was to perform a finite element-based comparative analysis of functional stress patterns in composite resin veneer restorations used for diastema closure to determine the influence of factors such as the preparation design, proximal extension, loading level, and vector of stress (loading angle). Three-dimensional finite element stress analysis was performed on a maxillary central incisor model to examine the stress distribution resulting from a nanofilled composite veneer restoration with

Maxillary midline diastema is a common aesthetic concern of dental patients. Various treatment modalities have been employed to close diastemas. This case report describes an unusual orthodontic treatment approach for a 25-year-old African American female patient with a large maxillary midline diastema of 5mm, bialveolar dental protrusion and unilateral Class III malocclusion. The treatment included one mandibular incisor extraction, followed by retraction of the incisors. At the end of the 16-month active treatment period, favourable aesthetic and occlusal outcomes were attained. Closure of m

Various oral complications such as gingival recession, restricted lip movement and tooth malalignment are the result of an abnormal frenum. Management of these types of frenum is either frenectomy or frenotomy. Methods for performing frenectomies include the conventional scalpel technique, Z-plasty, Miller's technique, V-Y plasty, lasers, and electrocautery. This case report details the successful management of an abnormal frenum attachment using electrocautery to ease and reduce discomfort to the 19-year-old female patient, causing aesthetic concerns. For its precision, minimal bleeding and p

BACKGROUND: Pediatric otolaryngologists have seen an increased focus on upper lip frenum as a possible culprit for feeding difficulties and the development of maxillary midline diastema (MMD). This increase may be encouraged by parents' exposure to medical advice over the internet about breastfeeding and potential long-term aesthetic concerns for their children. Subsequently, there has been increased pressure on pediatric otolaryngologists to perform superior labial frenectomies. There has been a reported 10-fold increase in frenectomies since the year 2000. However, there is no consensus with

Multiple treatment options are available for patients who have impacted canines in addition to congenitally missing lateral incisors. This article describes the treatment of a 13-year-old postpubertal girl with bilaterally impacted permanent maxillary canines, missing lateral incisors, retained deciduous canines, and a midline diastema. The orthodontic treatment plan included extraction of the deciduous canines. A lingual and labial approach (1-couple force system) was used to move the permanent canines into the arch. Through a collaborative team effort, including an orthodontist and a periodo

This paper presents an orthodontic case of a large (14.5 mm) maxillary midline diastema that was related to the presence, and subsequent removal of 2 median maxillary supernumerary teeth and resulting bone loss. A combined orthodontic and maxillofacial approach involving bone grafting and fixed appliances was used to close the space. This episode of care was organized as interceptive treatment during development of the permanent dentition.

STATEMENT OF PROBLEM: Managing excess anterior spacing is a common problem in dental practice. During orthodontic or restorative treatment planning, clinicians must decide how to distribute this space. However, few reports are available in the dental literature about the attractiveness and esthetic impact of anterior spaces other than midline diastemas.
PURPOSE: The purpose of this study was to compare the esthetic perception of 4 different anterior diastemas, including simian diastemas (symmetrical diastemas between lateral incisor and canine), diastemas suggested by Frush and Fisher (asymme

RATIONALE: Midline diastema is an unpleasant condition that affects a patient's smile and causes significant aesthetic concerns. Treating diastema and achieving the desired aesthetics, function, and harmony can be challenging without invasive treatment. This case report demonstrates a minimally invasive approach to closing a lower midline diastema.
PATIENT CONCERNS: A 38-year-old female patient presented to the dental clinic at King Khalid University with the primary complaint of a "large space in the lower front tooth region." Her medical history indicated no significant health concerns.

The aim of this study was to determine the prevalence of midline diastema, tongue tie and frenum attachments amongst school children in Nairobi. A total of 1802 children aged between 4 and 16 years were selected randomly using multistage sampling technique. To avoid oversampling in either sex, a proportionate sampling procedure was used. Thereafter, a thorough intra-oral examination was carried out using a mouth mirror under artificial or natural light with the children lying on a supine position. Presence or absence of midline interdental spaces unusually bigger than other interdental spaces

We describe a family in which eight persons (three males, five females) in three generations had a syndrome of bilateral and strikingly symmetrical triphalangeal thumbs and radial hypoplasia. The affected males also had a first-degree hypospadias and all affected family members had an anterior maxillary diastema. The syndrome was inherited in an autosomal dominant fashion.

The malocclusion known as maxillary midline diastema frequently occurs. For the management of diastema, a variety of therapy approaches are recommended. This article gives a case report of a female patient, age 24, who underwent treatment for a 4-mm maxillary midline diastema following the extraction of an additional tooth between her upper central incisors. The orthodontic treatment index was minimal (grade 2). The maxillary central incisors were fitted with bonded McLaughlin-Bennett-Trevisi (MBT) 0.018" brackets. A rectangular archwire was used after the circular archwire to ligate and activ

INTRODUCTION: This study aimed to investigate the esthetic impact of mandibular crowding and maxillary midline diastema in children in the mixed dentition.
METHODS: The sample for this cross-sectional study comprised 785 children, aged 8-10 years, in the late mixed dentition. Mandibular crowding and maxillary midline diastema were evaluated clinically with the Dental Aesthetic Index. Mandibular crowding and maxillary midline diastema were evaluated to determine tooth size-arch length discrepancies. The sample was stratified as group 1, children without maxillary midline diastema or mandibular

The use of direct composite resin for diastema closure has technique advantages, including that the restorative procedure can be carried out in one appointment at a reasonable cost and without the removal of sound tooth structure. The use of a rubber dam for closing diastemas with composite resin is of paramount importance as it prevents moisture contamination and ensures increased gingival retraction compared to other techniques. This provides better access to the cervical area of the tooth, facilitating proper placement of resin to recreate the natural anatomical contours and contact point.
