Journal of the American Society of Echocardiography : official publication of the American Society of Echocardiography1996eng
Transesophageal echocardiography is an accurate tool for the immediate diagnosis of acute aortic dissection. In addition to establishing the diagnosis of dissection, transesophageal echocardiography provides determination of its extent and detection of complications. The purpose of this study was to delineate the full spectrum of abnormalities present in acute dissection of the ascending aorta as assessed by transesophageal echocardiography. Forty consecutive patients with acute ascending aortic dissection were evaluated. Specific attention was paid to complications of aortic valve insufficien

In a 74-year-old patient who had undergone coronary artery bypass grafting 8 years earlier, angiography was performed for recent onset of angina pectoris. Surprisingly, angiography revealed chronic dissection of the ascending aortic vessel with occlusion of a saphenous vein graft and partial thrombosis of the false lumen. Additionally, there was significant progression of coronary artery disease with new stenoses in both the ostium and body of the saphenous vein graft to the right coronary artery and a high grade left main coronary artery stenosis. Because any surgical intervention was refused

Dissection of the ascending aorta is a rare complication of aortocoronary bypass surgery. A 63-year-old man who had received a double graft was found to have dissection of the ascending aorta, which was shown angiographically four months after operation. Since the patient was asymptomatic, operation was postponed. He has been followed up regularly and two years later his condition is still satisfactory.

OBJECTIVE: To evaluate the dynamics of intraventricular and intraaortic blood flows in ascending aorta dissection according to the data of transesophageal echocardiography (TEE) in the intraoperative period.
MATERIAL AND METHODS: During the study, 36 patients with dissecting aneurysms of the ascending aorta with mean age 52.4±7.5 years were examined. All patients underwent TEE in 2, 3 and 4-chamber positions before and after the surgery. The data were stored and analyzed on the EchoPAC (GE) workstation. Left ventricular ejection fraction (LVEF), end-diastolic (EDV) and end-systolic (ESV) volu

We present a 78-year-old man who developed Stanford type A aortic dissection seven years after aortic valve replacement. At the previous operation, the diameter of the ascending aorta was 40 mm. Three years later, computed tomography revealed the diameter to have expanded to 50 mm. Four years later, the patient developed aortic dissection with entry at the ascending aorta, and a graft replacement was performed.

A 55-year-old man developed acute chest pain and dyspnea. Computed tomography demonstrated a rupture of a giant aneurysm of the ascending aorta. The lesion was 14 cm in diameter--the largest ever reported-and resulted from chronic aortic dissection. The patient did not have aortic insufficiency or aortic dissection around the coronary ostium. Graft replacement of the ascending aorta was performed successfully under deep hypothermia with right hemisphere perfusion.

Aortic dissection is a medical emergency carrying high morbidity and mortality. Prompt diagnosis is sometimes difficult because of its varying presentations, but it is critical to the achievement of good clinical outcomes. This report describes 2 cases of painless aortic dissection that presented with aortic valve regurgitation. In both, the dissection was limited to the ascending aorta just distal to the aortic valve. These dissections were diagnosed by transthoracic and transesophageal echocardiography.

Acute aortic dissection is a life-threatening cardiovascular emergency and prompt diagnosis is crucial to improve the patient outcomes. Bedside transthoracic echocardiography performed in the emergency department plays a pivotal role in the initial workup of a suspected aortic disease. In this case report, we present a clinical case in which emergency echocardiographic evaluation initially suggested an erroneous diagnosis of aortic dissection, ruled out by chest computed tomography angiography and therefore highlighting a potential pitfall in echocardiography.

BACKGROUND: The histologic reaction in the adventitia to aortic dissections may be relevant to dating the onset of symptoms and to providing insight into pathogenesis.
DESIGN: We prospectively studied 43 surgically excised acute ascending aortic dissections before false-lumen rupture, with emphasis on inflammatory reaction in the false-lumen wall in relation to duration of symptoms.
RESULTS: A total of 31 men and 13 women were included in the study. Duration of symptoms was <12 hours (n=9), 12 to 24 hours (n=12), 1 to 2 days (n=8), 2 to 7 days (n=11), and >1 week (n=3). Medial inflammation w

Life-threatening acute aortic syndrome includes aortic dissection, intramural hematoma (IMH) and penetrating atherosclerotic ulcer (PAU). PAU and IMH carry a greater risk for aortic rupture. They most commonly occur in the descending thoracic aorta and are rare in the ascending aorta. Early diagnosis and intervention is crucial. We present the case of a 72-year-old female with a PAU of the ascending aorta complicated by hemopericardium with tamponade as well as IMH, successfully treated after rapid diagnosis and intervention, followed by a brief literature review.

Retrograde dissection of the aorta is extremely rare during percutaneous coronary intervention (PCI), but is a recognized and potentially life-threatening complication. We describe a case in which retrograde dissection of the aorta, necessitating urgent surgical repair, occurred during an attempt to open a chronically occluded right coronary artery. Initially localized, the dissection extended during an attempt to seal the right coronary ostium. Our experience suggests that if localized aortic retrograde dissection occurs, the management will depend on the stability of the distal coronary vess

The Japanese journal of thoracic and cardiovascular surgery : official publication of the Japanese Association for Thoracic Surgery = Nihon Kyobu Geka Gakkai zasshi1999eng
Aortic dissection is a rare but devastating complication of cardiac surgery. Adequate and early diagnosis of intraoperative aortic dissection and quick therapeutic decision making are the keys for saving patients in such cases. We describe the case of a 68-year-old man referred for CABG and mitral valve replacement with severe calcification of the ascending aorta. Intra-operative transesophageal echocardiography was useful for diagnosis of intra-operative aortic dissection and malperfusion of the true lumen. Immediate switching of the arterial perfusion site established flow in the true lumen

PURPOSE: In 1977, the use of gelatine-resorcine-formaline (GRF) biological glue during surgery of acute type A aortic dissection was proposed. The present study retrospectively analyses the late results obtained with this adjunct in an experience extending over a 20-year period.
PATIENTS AND METHODS: From January 1977 to July 1997, 193 patients (139 males and 54 females) aged from 15 to 79 years (mean age: 53 +/- 14 years) underwent an emergency operation for type A aortic dissection in our institution. All patients suffering from acute type A dissection and 162 (84%) were operated on within

A 63-year-old man with a history of hypertension and coronary artery bypass grafting (1 year ago) was admitted with acute onset severe chest pain suggesting bypass dysfunction. Biplane cineangiography revealed acute aortic dissection Stanford Type A without involvement of the aortic valve, the coronary arteries or the proximal anastomoses of the two venous bypass grafts, one of which was occluded. Urgent repair of the aorta by a prosthesis and reinsertion of the patent venous graft in the innominate artery by interposition of saphenous vein was performed without complications. Sixteen months l

Since 2000 Dorros et al. reported the first case of type A aortic dissection (TAAD) treated with an endovascular repair, surgeons explored a novel treatment option for TAAD gradually. The application of endovascular repair for TAAD highlights some points below which should pay attention to because of the special anatomic location and morphological characteristics of ascending aorta: (1) customized treatment strategy based on the characteristics of patients; (2) pre-operation evaluation; (3) selection of the stent and delivery system; (4) selection of the access sites; (5) preservation of the c

Bicuspid aortic valve (BAV) is a congenital anomaly associated with structural weakness of the aortic wall. Sudden onset of symptoms in patients with BAV, such as sudden severe back pain, and pulse inequality between the extremities or tension disparity should alert clinicians to acute aortic syndromes, as they require prompt diagnosis and management. Retrograde aortic dissection, which is a rare form of acute aortic syndrome, is an uncommon life-threatening entity and may produce atypical computed tomography (CT) or magnetic resonance imaging findings, leading to difficulty in diagnosis. Patients presenting with pseudoaneurysm of the aortic root close to the sternum, severe aortic regurgitation, and chronic dissection of the descending aorta represent a formidable surgical challenge. Closed chest circulatory arrest in this setting carries a high risk of myocardial or brain damage. We propose a modification of the port-access technique that allows control and access of the pseudoaneurysm, avoiding the need for closed chest circulatory arrest. Our strategy is based on transaxillary endoclamping of the ascending aorta and combines the advantages of antegrade aortic flow with the

Between April 1974 and November 1992, 181 patients were operated for aneurysm (106) or dissection (75) of the ascending aorta. Eighty patients had replacement with a valvular conduit with reimplantation of the coronary arteries (Bentall procedure), 48 had aortic valve replacement with replacement of the supra-coronary ascending aorta and 53 underwent isolated replacement of the ascending aorta. Twenty-nine patients (16%) died in the postoperative period, mainly of myocardial or neurological complications. Univariate statistical analysis completed by logistic regression analysis revealed the fo
