Hemodynamic Challenges of Lower Extremity Lymphovenous Anastomosis: A Critical Reappraisal.

Lymphovenous anastomosis (LVA) has become an established microsurgical treatment for lymphedema, yet the hemodynamic basis for its efficacy in the lower extremity has not been rigorously examined. Most assessments of anastomotic function are performed in the supine position, where lymphatic pressure exceeds venous pressure, creating a favorable gradient for drainage. However, adults spend 16-18 h daily in upright postures, during which ankle-level venous pressure rises to 80-100 mmHg while lymphatic pressure increases only modestly. This pressure reversal raises questions about whether lower e

'In-the-bag' hyphaema--a rare complication of posterior chamber lens implantation.

A 72-year-old woman developed a hyphaema on the first postoperative day after a combined trabeculectomy and extracapsular cataract extraction with posterior chamber intraocular lens implantation. On the second day the blood had redistributed to accumulate 'in the bag' posterior to the intraocular lens and had formed a fluid level. Postural drainage helped to clear the hyphaema from both locations, with no short-term effect on visual function or the filtering bleb.

Effect of Different Surgical Positions on the Changes in Cerebral Venous Drainage in Patients Undergoing Neurosurgery: A Prospective Observational Study.

BACKGROUND: Some patient positions employed to facilitate neurosurgery can compromise cerebral venous drainage and lead to increased intracranial pressure, intraoperative bleeding, and brain bulge. This study evaluated the impact of different surgical positions on internal jugular vein (IJV) flow as a measure of cerebral venous drainage and on optic nerve sheath diameter (ONSD) as an indirect assessment of intracranial pressure. METHODS: Ninety patients scheduled for elective primary brain tumor resection surgery were included in 3 groups of 30 each according to the surgical position: supine,

[Postural technique in prone position: hemodynamic and respiratory parameters and complications].

Therapeutic strategies used in the treatment of adult respiratory distress syndrome (ARDS) recommend placing the patient in prone position as an effective method for optimizing ventilation-perfusion parameters. We evaluated the therapeutic effect of postural treatment in prone position with the following goals: Comparison of hemodynamic and respiratory parameters before and after placing the patient in prone position, while in prone position, and before and after postural treatment. Complications associated with turning and time in prone position. A prospective study was made of 30 turns in 15

[Esophago-thoracic fistula after pneumonectomy and long-term main bronchus stump fistula].

A woman patient admitted for treatment, who had reached the age of 65 years, had a previous history of an open pulmonary tuberculosis in 1946 that had been treated at that time by means of collapsotherapy and phrenico-exeresis. Pleuropneumonectomy was performed in 1976 because of a residual cavity of a thoracic empyema. Since 1978 the patient suffered from a fistula of the bronchus treated by postural therapy without achieving a cure. In 1989 a fistula formed between oesophagus and pneumonectomy cavity. Clinically this was associated with an increasing reduction of performance and a suddenly i

Differential lung physiotherapy using a double lumen tube in flail chest and refractory lung atelectasis.

The management of refractory lung atelectasis in a patient with flail chest can be difficult. Treatment of lung atelectasis is complicated by practical problems with the application of chest physiotherapy, postural drainage and incentive spirometry. Although double lumen endotracheal tubes have been used in such cases for the purpose of differential lung ventilation, the use of differential lung physiotherapy has not been reported before. This report describes the successful application of this technique in a patient with flail chest and refractory lung atelectasis.

Assessment of drainage in PUJ dilatation: pelvic excretion efficiency as an index of renal function.

Renal function is important when assessing the response of a dilated renal pelvis to a diuretic stimulus. Yet there is little in the literature to suggest how this should be undertaken. Our aim was to develop a model which we have called pelvic excretion efficiency (PEE). The PEE, which may be used to assess drainage, is a mathematical model of the ratio of the total kidney excretion to the total amount of isotope extracted from the blood by the kidney. Thirty-three children with a prenatal diagnosis of unilateral renal pelvic dilatation (PUJ) were treated conservatively after birth. As a grou

Comparison of the active cycle breathing technique with other self-administered breathing techniques for the management of mucociliary production in cystic fibrosis: A systematic review.

BACKGROUND: Cystic fibrosis (CF) is characterized by excessive airway mucus, recurrent infections, impaired ventilation, and progressive decline in lung function. Airway clearance techniques are central to management. Among them is the active cycle of breathing technique (ACBT), which incorporates breathing control, thoracic expansion exercises, and the forced expiratory technique (FET). Comparative evaluation with other self-administered exercises is needed to identify the most effective approach for clinical use. OBJECTIVES: To evaluate the effects of ACBT compared with other self-administe

Aspiration of a tracheostomy plug.

A plastic truncated conical tracheostomy plug passed down the outer cannula of a Jackson tracheostomy tube and into the lungs of a respirator-dependent man during the monthly change of the inner cannula. Despite repeated flexible and rigid bronchoscopic visualization, attempts at dislodgement were unsuccessful. An exploratory thoracotomy was also unsuccessful in recovering the plug. Seventeen days postaspiration, with percussion and postural drainage, the patient expectorated the plug, even in the absence of the cough reflex. Such plugs have now been modified by passing a ring through the hole

Invasive pulmonary aspergillosis after near-drowning.

After near-drowning following a car accident, a 27-year-old man developed severe ARDS. Six days later Aspergillus fumigatus was isolated in his sputum, and invasive pulmonary aspergillosis developed thereafter. Aspergillus titre increased, and chest tomograms revealed cavities in both lungs. The treatment consisted essentially of Amphotericin B and 5 fluorocytosine, intravenously and by inhalation, intensive postural drainage and mechanical ventilation with PEEP. After 41 days he was cured and discharged from the ICU. Six months later he returned to his job and clinical examination was normal.

Forced expiratory technique, directed cough, and autogenic drainage.

In health, secretions produced in the respiratory tract are cleared by mucociliary transport, cephalad airflow bias, and cough. In disease, increased secretion viscosity and volume, dyskinesia of the cilia, and ineffective cough combine to reduce secretion clearance, leading to increased risk of infection. In obstructive lung disease these conditions are further complicated by early collapse of airways, due to airway compression, which traps both gas and secretions. Techniques have been developed to optimize expiratory flow and promote airway clearance. Directed cough, forced expiratory techni

New vascular system in reptiles: anatomy and postural hemodynamics of the vertebral venous plexus in snakes.

Using corrosion casting, we demonstrate and describe a new vascular system--the vertebral venous plexus--in eight snake species representing three families. The plexus consists of a network of spinal veins coursing within and around the vertebral column and was previously documented only in mammals. The spinal veins of snakes originate anteriorly from the posterior cerebral veins and form a lozenge-shaped plexus that extends to the tip of the tail. Numerous anastomoses connect the plexus with the caval and portal veins along the length of the vertebral column. We also reveal a posture-induced

The effect of middle ear effusion on the inner ear condition in children.

BACKGROUND: Otitis media with effusion (OME) is the most common cause of hearing impairment among children in developed nations. Middle ear (ME) fluid accumulation leads to progressive hearing impairment, usually of the conductive type. In some cases, mixed hearing loss associated with OME has been noted. It was reported that effusion in the ME has a negative impact on the vestibular system of the inner ear. OBJECTIVES: The aim of this random-sample cohort study was to evaluate postural stability and the influence of ME drainage on vestibulospinal reflexes in children with OME, and to determi
