Small Molecule-Drug Conjugates: An Emerging Drug Design Strategy for Targeted Therapeutics.

Traditional chemotherapeutic agents face challenges due to nonselective cytotoxicity. Targeted therapy, as an essential component of precision medicine, has achieved notable breakthroughs in recent years. The successful clinical translation of antibody-drug conjugates (ADCs) initiated the era of targeted conjugate drugs. Small molecule-drug conjugates (SMDCs), which utilize small molecules as targeting ligands, have overcome numerous limitations associated with ADCs. These agents demonstrate favorable pharmacokinetic properties, enhanced cellular penetration capabilities, and nonimmunogenicity

Conquering viral drug resistance: Structural and mechanistic paradigms for antiresistance drug design.

Viral drug resistance remains a critical challenge in antiviral therapy. This perspective highlights five studies on severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), human immunodeficiency virus type 1 (HIV-1), monkeypox virus (MPXV), influenza A virus (IAV), and Hepatitis B virus (HBV), revealing novel resistance mechanisms and innovative strategies. For SARS-CoV-2, GC376's flexible benzyl group overcomes nirmatrelvir resistance. HIV-1's non-nucleoside reverse transcriptase inhibitors (NNRTIs) 5i3 adapts to resistant mutants via a quinazoline scaffold, while MPXV's tecovirimat ac

Design Features of Drug-Drug Interaction Trials Between Antivirals and Oral Contraceptives.

The aim of this work was to explore the major design features of drug-drug interaction trials between antiviral medications (AVs) and oral contraceptives (OCs). Information on these trials (n = 27) was collected from approved drug labels and clinical pharmacology reviews conducted by the U.S. Food and Drug Administration. The primary objective of all trials was to evaluate changes in OC exposure following the coadministration of AVs. In addition, an evaluation of potential pharmacodynamic interaction was performed in 10 of these trials. Twenty-two trials were open label with a fixed-sequence d

In vitro design of intrathecal drug administration therapies.

Due to the scarcity of reliable in vivo data, the pharmacokinetics of intrathecally (IT) administered drugs remain inadequately quantified. Designing new therapies is further hindered by variability in experimental methods, inter-individual and inter-species differences, and poor reproducibility across animal and human studies. To address these limitations, we developed an anatomically accurate, subject-specific replica of the cerebrospinal fluid (CSF)-filled spaces of the human central nervous system (CNS) using a multistep mold/casting process. The 3D-printed, transparent, deformable CNS pha

[Design and Functionalization of Molecules Towards Drug Discovery].

We performed the design, synthesis and evaluation of ligands of nuclear receptors [vitamin D receptor (VDR), retinoid receptor and a VDR mutant], stapled short helical peptides as VDR-coactivator interaction inhibitors, and estrogen receptor degradation inducers based on a protein knockdown strategy and selective estrogen receptor down-regulators. We also demonstrated the design and functionalization of helical peptides, and performed conformational analysis of peptides, as well as an in silico study on the prediction of bioactivity for the regulation of new designer drugs.

Achieving femtomolar affinities in structure-based drug design.

Developing small-molecule compounds as effective and safe pharmaceuticals relies on their ability to bind disease-associated proteins with high affinity and selectivity. However, achieving ultra-high affinity in the femtomolar range is a major challenge in medicinal chemistry due to the inherent constraints of protein-ligand interactions. Here, we introduce a novel class of di-meta-substituted fluorinated benzenesulfonamide compounds that achieve an extraordinary dissociation constant of 44 fM for carbonic anhydrase IX (CAIX). CAIX, a transmembrane enzyme implicated in hypoxic tumor progressio

Oxazaphosphorine cytostatics: from serendipity to rational drug design.

On the basis of the discovery that the proapoptotic aldehyde 3-hydroxypropanal is a cyclophosphamide metabolite, a novel mechanism of action of oxazaphosphorine cytostatics is presented and confirmed by animal experiments. Furthermore, it is shown that new oxazaphosphorine cytostatics, which are on orders of magnitude more effective than already existing, can be developed on the basis of the new model for the mechanism of action.

Drug-eluting balloon: design, technology and clinical aspects.

A drug-eluting balloon is a non-stent technology in which the effective homogenous delivery of anti-proliferative drugs is processed by the vessel wall through an inflated balloon. This is done to restore luminal vascularity in order to treat atherosclerosis, in-stent restenosis and reduce the risk of late thrombosis without implanting a permanent foreign object. The balloon technology relies on the concept of targeted drug delivery, which helps in the rapid healing of the vessel wall and prevents the proliferation of smooth muscle cells. Several drug eluting devices in the form of coated ball

Structure-based drug design for dopamine D3 receptor.

D2-like receptors are members of dopamine receptors, including dopamine D2 receptor (D2R), dopamine D3 receptor (D3R), and dopamine D4 receptor (D4R), which modulate behavior, cognition, and emotion. D2-like receptors are critical targets for drug development. Particularly, D3R has been identified as a therapeutic target for antipsychotic and anti-parkinsonian drugs. Recently, the crystal structure of D3R was reported. Here we summarize the available active compounds for D3R and the structure-activity relationships (SAR) studies of them. This provides lead templates for further chemical modifi

Fragment-Based Drug Design of Selective HDAC6 Inhibitors.

Medicinal chemistry society has enough arguments to justify the usage of fragment-based drug design (FBDD) methodologies for the identification of lead compounds. Since the FDA approval of three kinase inhibitors - vemurafenib, venetoclax, and erdafitinib, FBDD has become a challenging alternative to high-throughput screening methods in drug discovery. The following protocol presents in silico drug design of selective histone deacetylase 6 (HDAC6) inhibitors through a fragment-based approach. To date, structural motifs that are important for HDAC inhibitory activity and selectivity are describ
