Alcohol and drug interactions.

Alcohol use is a serious health problem in the Czech Republic with significant impact on morbidity, mortality and pharmacotherapy. Approximately 15-18% of the population drink risky, and the combination of alcohol and drugs is common and often overlooked. Ethanol is metabolised predominantly in the liver by the enzymes ADH and CYP2E1. Chronic consumption induces CYP2E1, which might increase the production of toxic metabolites of some drugs. Ethanol acts on the CNS mainly through GABA, NMDA and adenosine receptors and affects ion channels, contributing to its CNS-depressant effects. Alcohol-dru

Drug Interactions with Antihypertensives.

PURPOSE OF REVIEW: Hypertension is remarkably prevalent, affecting an estimated 1.13 billion people worldwide. It often requires the use of multi-drug regimens and is commonly associated with a myriad of other comorbidities which increase medication use. The pervasive use of antihypertensive medications combined with the presence of polypharmacy in many hypertensive patients results in significant risk of drug interactions. This review will summarize the relevant literature to assist clinicians in mitigating drug interaction risks when prescribing antihypertensives. RECENT FINDINGS: Pharmacok

Drug interactions with statins.

The 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase inhibitors (statins) are generally well tolerated as monotherapy. Statins are associated with two important adverse effects, asymptomatic elevation in liver enzymes and myopathy. Myopathy is most likely to occur when statins are administered with other drugs. Statins are substrates of multiple drug transporters (including OAT- -P1B1, BCRP and MDR1) and several cytochrome P450 (CYP) enzymes (including CYP3A4, CYP2C8, CYP2C19, and CYP2C9). Possible adverse effects of statins can occur due to interactions in concomitant use of drugs th

Grapefruit and drug interactions.

Since the late 1980s, grapefruit juice has been known to affect the metabolism of certain drugs. Several serious adverse effects involving drug interactions with grapefruit juice have been published in detail. The components of grapefruit juice vary considerably depending on the variety, maturity and origin of the fruit, local climatic conditions, and the manufacturing process. No single component accounts for all observed interactions. Other grapefruit products are also occasionally implicated, including preserves, lyophylised grapefruit juice, powdered whole grapefruit, grapefruit seed extra

Drug interactions with fluoroquinolones.

The fluoroquinolones are a new class of antimicrobial agents that are now widely prescribed for a number of bacterial infections. Because of their complex pharmacokinetics, there is a potential for several types of drug interactions. Currently, only two drug interactions have been well studied. These involve a decrease in absorption when fluoroquinolones are given in combination with multivalent metal cations and an inhibition in the metabolism of methylxanthines by fluoroquinolones such as ciprofloxacin, enoxacin, and norfloxacin. These drug interactions can be easily avoided. Significant dec

Adverse virus-drug interactions.

Over the last 3 decades, epidemiologists and clinicians have identified a few clinical entities that appear to result when a viral infection and a chemical exposure overlap and interact. Ampicillin rash during infectious mononucleosis, Reye's syndrome following salicylate ingestion and certain viral infections, and the association of AIDS-related Kaposi's sarcoma with abuse of nitrite inhalants and infection due to human immunodeficiency virus are examples of such phenomena. Preclinical research provides additional evidence that viruses and chemicals may interact and produce illnesses in anima

[Drug interactions in dentistry].

The authors describe the various interactions which may exist between the medical treatments followed by patients and various groups of substances used in odonto-stomatology: non-steroid anti-inflammatory drugs, antibiotics, antifungicides, vasoconstrictors and local anaesthetics. They clarify the clinical risk relating to each of these medicament interactions.

Oral contraceptive drug interactions.

Approximately 50 million women use oral contraceptives (OC). Studies and case reports demonstrate that OC failure may be caused by rifampin, anticonvulsant drugs, and possibly some antibiotics. Contraceptive steroids may interfere with the metabolism of the benzodiazepines, theophylline, and the glucocorticoids. Future investigation will document the clinical significance of other OC interactions as well as give rise to new interactions.

Drug interactions in anesthesia.

The threat of adverse drug interactions increases daily as more and more drugs become available. All physicians should be familiar with the problems of drug interactions and their management. Careful preoperative evaluation and screening combined with limited prescribing can help to limit the magnitude of this problem.

Drug interactions in psychopharmacology.

The mechanisms underlying psychotropic drug interactions include alterations in gastrointestinal absorption, protein-binding, specific and nonspecific metabolic degradation, receptor interactions, and renal clearance. The risk of interactions can be minimized by detailed drug history-taking, use of rapid reference sources, patient education, and physician behaviors that include serious attention to somatic complaints, identification of "at risk" situations, and rational prescribing habits.

Investigating drug--ethanol interactions.

Methodology developed in our laboratories for testing the interactive effects of ethanol and drugs on human psychomotor performance is discussed. An attempt has been made to relate the findings of our studies to the results of real-life impairment, particularly in traffic crashes. Proposals for more comprehensive testing of drug--ethanol interactions have been put forward which may increase the predictive value of such tests.

Drug interactions with amiodarone.

There are a number of important drug interactions with amiodarone. This agent appears to have a marked effect on the kinetics of some commonly used cardiovascular drugs, such as warfarin, digoxin, quinidine, and procainamide, and has dynamic interactions with others, such as the beta blockers and some calcium antagonists. Bleeding has been reported, apparently caused by a potentiation of the anticoagulant effect of warfarin by amiodarone. Torsades de pointes has been observed when quinidine, propafenone, or mexiletine is given together with amiodarone. Furthermore, amiodarone may interact with

Lithium and drug interactions.

The mutual enhancement of the side effects of muscular tremor and rigidity caused by lithium and neuroleptic drugs should be recognised. However, at present the 'case' for a neuroleptic-lithium interaction producing severe organic brain syndrome is arguably weak; in the absence of standardised sampling for monitoring lithium serum concentrations such reports may simply reflect 'pure' lithium intoxication rather than a drug interaction. Certainly it seems unwarranted in this connection to ascribe specific properties to haloperidol. A potentially high risk of interaction should be recognised bet

Drug-pyridoxal phosphate interactions.

In this review it has been pointed out that vitamin B6 and its vitamers can be involved in many interactions with a number of drugs, as well as with the actions of various endocrines and neurotransmitters. Nutritional deficiencies, especially of vitamins and proteins, can affect the manner in which drugs undergo biotransformation, and thereby may also modify the therapeutic efficacy of certain drugs. The differences between nutritional vitamin B6 deficiency and the hereditary disorder producing pyridoxine dependency are discussed. In addition to a pyridoxine deficiency being able to adversely

Urate excretion: drug interactions.

A derivative of probenecid, 2-nitroprobenecid, was studied in chimpanzees and Cebus monkeys. The uricosuria induced by the drug could be diminished by the infusion of p-aminohippurate (chimpanzee) or hippurate (monkey). Both hippurates inhibited the secretion of the drug and it is likely that the diminished response was the result of decreased access of 2-nitroprobenecid to its site of action. In contrast, pyrazinoate diminished the response to 2-nitroprobenecid without disturbing its renal disposition (both species). This action of pyrazinoate is attributed to its ability to inhibit the secre

Drug interactions with alcohol.

Ethanol and drugs can affect each other's absorption, distribution, metabolism, and excretion. When ingested together, ethanol can increase drug absorption by enhancing the gastric solubility of drugs and by increasing gastrointestinal blood flow. However, high concentrations of ethanol induce gastric irritation causing a pyloric spasm which in turn may delay drug absorption and/or reduce bioavailability. The 'quality' of the alcoholic beverage, independent of its ethanol content, can contribute to altered absorption of a drug. Ethanol is not bound to plasma proteins extensively enough to modi

[Alcohol-psychotropic drug interactions].

The problem of ethanol interactions with psychotropic drugs involves various factors whose complex action is examined in this article. Great care should be taken with regard to the alcohol consumption among patients treated with sedatives; the depressive action of ethanol on the central nervous system could be potentiated especially at the beginning of the treatment, as it seems that a certain habituation to the interactions may exist. However, one should be aware that the personality of the patient is perhaps more important to take into account that the consumption of drugs, at least as regar
