GASTRO-DUODENAL CROHN'S DISEASE.

Gastro-duodenal Crohn's disease is rare. Thirty-one previously reported cases are briefly reviewed; histological confirmation of the diagnosis was not always possible. Details are given of a patient with pyloro-duodenal involvement accompanied by terminal ileitis and appendicitis where surgical specimens were available for study. The differential diagnosis is considered from the clinical and pathological aspects.

[Characteristics of the course of gastric and duodenal ulcer disease concurrent with duodenal insufficiency].

AIM: To comprehensively study the course of gastric ulcer disease (GUD) and duodenal ulcer disease (DUD) concurrent with chronic duodenal insufficiency (CDI). MATERIAL AND METHODS: Ulcer disease (UD) was verified on the basis of the results of clinical and fibrogastroduodenoscopic examinations. The data of contrast duodenography and cavitary manometry were used to identify CDI. Gastroduodenal motor activity was investigated using the peripheral electrogastrograph EGG-4M. The results of pH measurements were employed to assess the state of gastric acid secretion and duodenal pH values. RESULTS

[Surgical treatment of gastric and duodenal ulcer disease with chronic duodenal obstruction].

One hundred and twenty-nine surgeries were performed from 1990 to 2001 for gastric (50) or duodenal (79) ulcers with chronic duodenal obstruction. Resection of 1/2 of the stomach by Bilrot-I was performed in 118 patients, pylorus-saving resection with creation of areflux valve in duodenal bulb--in 5, selective proximal vagotomy with duodenoplasty--in 6 patients. In early postoperative period complications were seen in 10 (10.9%) patients, postoperative lethality was 0.8% (one patient died). There was no insufficiency of anastomotic sutures. Postoperative stay was 11.3 +/- 1.5 days. In remote p

Duodenal Chemosensing of Short-Chain Fatty Acids: Implications for GI Diseases.

PURPOSE OF REVIEW: Short-chain fatty acids (SCFAs), the main bacterial fermentation products in the hindgut of hindgut fermenters, are also present in the foregut lumen. We discuss the impact of SCFAs in the duodenal defense mechanisms and in the gastrointestinal (GI) pathogenesis. RECENT FINDINGS: Luminal SCFAs augment the duodenal mucosal defenses via release of serotonin (5-HT) and glucagon-like peptide-2 (GLP-2) from enteroendocrine cells. Released GLP-2 protects the small intestinal mucosa from nonsteroidal anti-inflammatory drug-induced enteropathy. SCFAs are also rapidly absorbed via S

[Oxygen metabolism disturbances in the pathogenesis of gastric and duodenal diseases].

Oxygen metabolism disturbances are the pathogenetic factors of many diseases, including digestive tract diseases. Last time much attention was given to importance of active oxygen form, oxygen free radicals. Free radicals are one of the tissue aggressive factors and they are the pathogenic factor of gastritis and duodenitis, stress ulcer, gastric and duodenal ulcer disease and gastric cancer. In this paper the review of investigations of oxygen metabolism in gastric and duodenal diseases was presented.

[Immunoglobulins in the gastric juice in some stomach and duodenal diseases].

IgA, IgM and IgC content in blood serum and gastric juice were determined in 89 patients with different gastric and duodenal diseases (57 with duodenal ulcers, 13 with gastric ulcers 8 with chronic atrophic gastritis, 9 with superficial chronic gastritis and gastroduodenitis and two with gastric cancer). Immunoglobulin was established in gastric juice (over 0,3 mg/100 ml) in 49 per cent of all the patients examined with gastric and duodenal diseases. No correlation was found between serum immunoglobulin level and their presence in the gastric juice. No correlation exists between the volume of …

Duodenal, prepyloric, and combined duodenal/prepyloric ulcer disease: three distinct entities of juxtapyloric ulcer disease?

One hundred and seven patients with long-standing and severe chronic juxtapyloric ulcer disease were classified in accordance with the location of the present ulcer and previous ulcers into 1) pure duodenal (DU), 2) pure prepyloric (PU), and 3) combined duodenal/prepyloric (DU/PU) or prepyloric/duodenal (PU/DU) ulcer disease. In a prospective follow-up study over a 3-year period after parietal cell vagotomy (n = 39) or during continuous treatment with cimetidine (n = 62) patients with DU had recurrent ulcers located exclusively to the duodenal bulb and patients with PU, exclusively to the prep

[Helicobacter pylori--an etiopathogenic factor of stomach and duodenal diseases among children].

One of the newest conceptions on the causes of gastritis, duodenitis, peptic and duodenal ulcers states that an infectious factor--Helicobacter pylori is responsible. The purpose of the this study was to estimate the frequency of Helicobacter pylori among children and the correlation between its presence and gastritis, duodenitis, peptic and duodenal ulcers and biliary reflux. Helicobacter pylori infection seems to be a serious etiopathogenic factor of stomach and duodenal diseases among children. Inflammatory changes of the gastric mucosa, as well as peptic and duodenal ulcers more frequent i…

[The morphofunctional characteristics of the duodenal mucosa in patients with a duodenal ulcer at different phases of the disease].

Endoscopic, morphological, histochemical and biophysical properties of the duodenal mucosa as well as secretory activity of the stomach and pancreas were studied and compared in 398 patients with duodenal ulcer in different phases (active ulcer, healing ulcer, red and white scars). Sixteen patients were observed over time. Analogous studies were conducted in 88 patients with chronic diseases of the biliary and duodenopancreatic zone (with non-ulcerous duodenitis) and in 32 normal persons. The duodenal mucosa was established to undergo consistent morphofunctional reorganization related to the d

Chronic nonspecific duodenitis. A multiple biopsy study of the duodenal bulb in health and disease.

A clinical, endoscopic and histopathologic study on chronic nonspecific duodenitis was performed in 174 subjects, 124 patients (control patients, patients with gastric or duodenal ulcer and nonulcer dyspepsia) and 50 healthy volunteers without clinical history of gastro-intestinal disease. Chronic nonspecific duodenitis was found in 6 healthy volunteers (12%), 5 control patients (7%), one gastric ulcer patient (7%), all duodenal ulcer patients and in 15 patients with nonulcer dyspepsia (83%). Chronic non-specific duodenitis is a focal process affecting the duodenal bulb. It can be missed when

Surgical management of duodenal crohn's disease.

BACKGROUND: Operative options for duodenal Crohn's disease include bypass, stricturoplasty, or resection. What factors are associated with operation selection and whether differences exist in outcomes is unknown. METHODS: Patients with duodenal Crohn's disease requiring operative intervention across a multi-state health system were identified. Patient and operative characteristics, short-term surgical outcomes, and the need for future endoscopic or surgical management of duodenal Crohn's disease were analyzed. RESULTS: 40 patients underwent bypass (n = 26), stricturoplasty (n = 8), or resect
