[Molecular basis of the primary ciliary dyskinesias].

INTRODUCTION: The primary ciliary dyskinesias (PCD) are rare diseases characterised by infection of the airways due to impaired muco-ciliary clearance. Half the patients have situs inversus making up Kartagener's syndrome. STATE OF THE ART: Primary cilia play a role in development. In the adult ciliated cells occur mainly in the airways and the genital tract. The axoneme, the internal structure of the cilia, is made up of a central pair of microtubules surrounded by peripheral doublets carrying the inner and outer dynein arms. These multiprotein complexes are composed of chains of dynein whos

Paroxysmal dyskinesias in patients with HIV infection.

OBJECTIVE: To determine the clinical features of paroxysmal dyskinesias among HIV type 1 (HIV-1)-seropositive patients. BACKGROUND: Movement disorders have been associated with HIV infection, although the full spectrum of these disorders remains uncertain. METHODS: Six adult HIV-1-seropositive patients presenting with paroxysmal dyskinesias were identified. Each patient underwent metabolic, CSF, EEG, and neuroimaging studies. RESULTS: Mean age at onset was 34.5 years and five of six patients were AIDS defined. Dyskinesias were focal, multifocal, or hemidystonic in four patients and generali

Parkinsonian monkeys with prior levodopa-induced dyskinesias followed by fetal dopamine precursor grafts do not display graft-induced dyskinesias.

Clinical trials testing the hypothesis that fetal dopamine grafts would provide antiparkinsonian benefit in patients who had already developed side effects from their long-term use of L-dopa revealed, in some cases, the presence of dyskinesias even in the absence of L-dopa. The form, intensity, and frequency of these dyskinesias were quite variable, but their manifestation slowed the clinical development of cell replacement therapies. Rodent models of graft-induced dyskinesias (GIDs) have been proposed, but their accuracy in modeling GIDs has been questioned because they usually require amphet

[Dyskinesias as neurologic complications of hematologic disorders].

Dyskinesias as the neurologic complications of hematologic disorders were described in detail on abetalipoproteinemia and ataxia telangiectasia as well as in summary on other hematologic diseases. Dyskinesias as the iatrogenic neurologic manifestations in hematologic disorders were also briefly described. Although the pathogenetic mechanisms of dyskinesias observed in hematologic disorders were quite heterogeneous, they could provide us with good suggestions and clues to clarify the true mechanisms of their own as well as other dyskinesias.

Subthalamic DBS-induced diphasic dyskinesias: An overlooked complication?

INTRODUCTION: Deep brain stimulation (DBS) has become the standard of care in patients with Parkinson's Disease (PD) experiencing motor fluctuations. The existing literature suggests that deep-brain stimulation (DBS) of the subthalamic nucleus (STN) alleviates levodopa-induced dyskinesias (LID), particularly those associated with high plasma levels of levodopa (peak dose), through medication reduction. CASE PRESENTATION: We report a case of early-onset PD with motor fluctuations and levodopa-induced peak-dose dyskinesias, who developed an interesting phenomenon of diphasic dyskinesias during

[Paroxysmal dyskinesias - disorder categories, their causes and treatment].

Paroxysmal dyskinesias refer to category of abnormal involuntary movements, such as chorea, dystonia, athetosis, ballism or their various configurations. Depending on the type of seizure, sudden movement, stress, emotions, coffee or alcohol may be the trigger factors. Acute seizures are characterized by short duration and are self-limitated. Patients present correct portray of movements between seizures. Intact consciousness during seizure is the invariable characteristic of all paroxysmal dyskinesias. The intent of this work is to systematize knowledge about paroxysmal dyskinesias. This resea

Therapies for dopaminergic-induced dyskinesias in Parkinson disease.

Existing and emerging strategies for managing L-dopa-induced dyskinesias (LIDs) in patients with Parkinson disease have involved either delaying the introduction of L-dopa therapy, treatment with an antidyskinetic agent, using a therapy or delivery system that can provide continuous dopaminergic stimulation, or using novel agents that target receptors implicated in the mechanisms underlying LIDs. Treatment with dopamine agonists such as pramipexole or ropinirole allows levodopa to be delayed, but once levodopa is added to the drug regimen the usual course of onset of dyskinesias is observed. Other pharmacological treatments for motor complications and dyskinesias.

Controlling motor complications becomes increasingly difficult with disease progression. The "wearing-off" phenomenon is the most-common motor fluctuation. Wearing-off can be treated by dietary manipulation, shortening the dosing interval, substituting sustained-release levodopa, adding amantadine, or monoamine oxidase type B inhibitors, and other options, including catechol-O-methyltransferase inhibitors and the approved dopamine agonists addressed in another chapter. The rotigotine constant-delivery system is being developed to treat wearing-off symptoms. Istradefylline (KW-6002), an adenosi

[Treatment of tardive dyskinesias with 7-methoxytacrine. I].

The 7-metoxytacrine (7-MEOTA) is an original Czech cholinergic agent synthetized in the labs of the Military Medical Academy in Hradec Králové. The treatment of tardive dyskinesias is one of the possible indications of the use of 7-MEOTA in clinical practice. The authors have summed up their experience from the first phase of clinical tests with 7-MEOTA in psychiatrical patients suffering from tardive dyskinesias in long-termed administration of cholinergic agents. The clinical efficiency and tolerance of 7-MEOTA have been evaluated after a single administration of 100 mg of 7-MEOTA per os in

[Paroxysmal dyskinesias in pediatrics: diagnostic route and treatment].

Paroxysmal movement disorders comprise a heterogeneous group of entities characterized by recurrent, self-limited episodes of abnormal movements. They are mainly divided into paroxysmal dyskinesias (PxD) and episodic ataxias, and typically begin in childhood or adolescence, with a tendency to improve in adulthood. The most widely used clinical classification of PxD is based on triggering factors and includes paroxysmal kinesigenic dyskinesia (PKD), paroxysmal non-kinesigenic dyskinesia (PNKD), and exercise-induced dyskinesia (PED). There is marked genotype-phenotype overlap, which limits the u

Paroxysmal kinesigenic dyskinesias and pseudohypo-parathyroidism type Ib.

Pseudohypoparathyroidism is a rare cause of paroxysmal dyskinesias. We describe an otherwise well 10-year-old girl who was diagnosed with pseudohypoparathyroidism type Ib after presenting with involuntary movements of the hands and feet that occurred while running or walking. Magnetic resonance imaging of the brain indicated T(1) hyperintensities of the bilateral basal ganglia. A computed tomography scan of the head revealed diffuse cerebral calcifications, most prominent in the basal ganglia. Treatment with calcitriol and calcium carbonate led to a complete resolution of her signs. We recomme

Investigating levodopa-induced dyskinesias in the parkinsonian primate.

Research into the cause of dyskinesias arising from levodopa treatment has been vexingly limited, partly due to the lack of an inexpensive and widely available animal model. Rodents do not develop levodopa-induced dyskinesias in a clinically recognizable form. However, nonhuman primates with 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP)-induced parkinsonism readily develop levodopa-induced dyskinesias that are virtually indistinguishable from those seen in patients with Parkinson's disease. We have developed and validated a five-point Global Primate Dyskinesia Rating Scale to accurately
