Viral etiology for inner ear diseases: proven, unproven, unlikely.

This is a revision article that deals with the broad field of inner ear disease caused by viral infections. Some of these entities have been proven to have a viral etiology. Others have strong evidence in favor of a viral causation but still cannot be considered as a viral disease. Finally, other entities are suggestive of a viral etiology but when the whole body of evidence is considered one concludes that a viral etiology is indeed unlikely. We review the literature and add our own experience in this subject. Clearly, the most important evidence about this subject came from the study of temp

What inner ear diseases cause benign paroxysmal positional vertigo?

Benign paroxysmal positional vertigo (BPPV) originating from the posterior semicircular canal (pSCC) is a common vestibular disorder that is easy to diagnose and usually easy to treat. The majority of patients with BPPV have no known inner ear disease; they have "primary" or "idiopathic" BPPV. However, a minority does have objective evidence of an inner ear disease on the same side as the BPPV and this group has "secondary" or "symptomatic" BPPV. Previous publications differ on the prevalence of secondary BPPV and about the types of inner ear diseases capable of causing it. In order to determi

[Differences in temporal bone development in various ear diseases].

To investigate the differences in temporal bone development in various ear diseases--bilateral Meniere's disease (n = 13), unilateral Meniere's disease (n = 41), unilateral chronic otitis media (COM) (n = 25), temporal bone fracture (n = 9) and otosclerosis (n = 12)--the following 4 distances on a temporal bone CT slice encompassing the lateral semicircular canal (LSC) were measured by using the NIH Image program. These distances included the minimal distance between the posterior semicircular canal (PSC) and the posterior petrous surface (PPS) (P-P distance), the minimal distance between the

[Viral serology in ear diseases of presumably neuritic origin].

A viral etiology is often advanced to explain the origin of Bell's palsy, sudden deafness and certain vertigos. Virology findings were compared in 72 patients with either chicken pox, herpes, zona or other "viral" infections, where the viral origin was not absolutely clear, and a population of healthy volunteers. Results confirmed the indefinite viral nature of these affections. However, recent studies have shown that definite viral disorders are not always associated with an increase in antibody levels, and the intervention of a virus should not be totally discounted in some of these affectio

[Possible viral etiology of inner ear diseases (author's transl)].

After a short review of the possible viral etiology of inner ear diseases viral titers of 20 patients with inner ear problems are given. In the history of nearly all patients a viral infection was found. Inspite of some positive results in the titrations no significance was seen. Therefore we refute a relation between viral infection and inner ear troubles; for a practical point of view virological diagnostics is not indicated.

Results of mastoid operations in various chronic ear diseases.

A study of mastoid operations undertaken for various chronic middle ear entities, using closed and open techniques, revealed the following: 1. Marsupialized mastoid balls were found less disappointing than often considered; small cavities were dry in 90 percent and large ones in 70 percent of cases. 2. Residual disease (epidermoid cysts) appeared in 27 percent of the CAT-operated ears and in 13 percent when the open technique was employed. 3. Residual disease appeared twice as often when the primary disease involved the tympanic cavity and in young patients than when the attic and mastoid were

[Diseases of the inner ear due to excessive noise].

Noise causes damage to the auditory system either to excessive kinetic energy, which results in acoustic trauma once the mechanical resistance capacity of internal ear tissues has been exceeded; or else, a long term exposure to noise of non-traumatic intensity can exhaust the sensory cell metabolism to such an extend that numeric atrophy of the auditory neuroepithelium will develop as a result of hyperactivity. Progressive necrosis will develop if Corti's organ barrier against endolymph is broken. The pathomorphological types of auditory organ damage are in agreement with the characteristic fu

[The role of the inner ear biological barrier in targeted drug delivery for inner ear diseases].

The endolymphatic barrier, composed of tight junctions and ion transporters, regulates the ionic gradient between endolymph and perilymph and plays a pivotal role in maintaining inner ear function. Disruption of this biological barrier can result in ionic imbalance, causing both auditory and vestibular dysfunction. Medication needs to enter the perilymph instead of the endolymph by passing through the perilymphatic barrier to reach the hair cells. Targeted drug delivery has emerged as a promising therapeutic strategy for inner ear diseases, including intratympanic injection, intracochlear infu

Surgery of the 'only hearing ear' with chronic ear disease.

The management of chronic ear disease affecting the only hearing ear is a controversial subject. The relative scarcity of literature on the subject prompted us to prepare a questionnaire which was sent to European and American otologists and to review 19 cases operated at the ENT Clinic of the University of Parma, Italy, and 16 cases operated at The Baptist Memorial Hospital, Memphis, U.S.A. Surgery of cholesteatoma involving the only hearing ear is advised by all the interviewed otologists without exception, even in the presence of a labyrinthine fistula. The cases from the University of Parm

Middle ear ototoxic treatment for inner ear disease.

Twenty-nine patients seriously disabled by Ménière's disease due to frequent attacks were treated with Gentamicin administered in the middle ear once daily until first sign of an inner ear disturbance, usually a spontaneous nystagmus and a sensation of unsteadiness. All patients except one were relieved from their vertiginous attacks and returned to normal activities. Tinnitus was usually diminished or absent, as was the feeling of pressure in the ear. The hearing was slightly improved in 5 patients, worse in 9 patients and two treated ears became deaf. The indication for an intratympanal trea

Involvement of the external ear by inflammatory skin diseases.

Chronic inflammatory skin disorders comprise frequent and rare dermatoses. The etiology of these diseases is broad and ranges from genetic factors to environmental and lifestyle issues. In this review, we focus on afflictions of the auricle by the eczema group, psoriasis, and pityriasis rubra pilaris, autoimmune disorders like relapsing polychondritis and cutaneous lupus erythematosus, and juvenile spring dermatosis. The anatomy of this particular region can alter the clinical presentation, course, and treatment options for these skin disorders. Atopic dermatitis, seborrheic dermatitis, and ps

[Accident-related inner ear diseases - clinical aspects and conditions].

Trauma-related inner ear disorders following head impacts can have various causes. The use of the more recent classification of temporal bone fractures into "otic capsule sparing" (OCS) and "otic capsule violating" (OCV) is useful in the context of medical assessment. The available imaging is often insufficient, and the consequences of otic capsule fractures are underestimated in emergency management. Therefore, indirect signs (e. g., hemotympanum, cerebrospinal fluid otorrhea, facial palsy) often must be evaluated. Even without radiological evidence of a fracture, temporary inner ear damage c

Precision medicine: a new era for inner ear diseases.

The inner ear is the organ responsible for hearing and balance. Inner ear dysfunction can be the result of infection, trauma, ototoxic drugs, genetic mutation or predisposition. Often, like for Ménière disease, the cause is unknown. Due to the complex access to the inner ear as a fluid-filled cavity within the temporal bone of the skull, effective diagnosis of inner ear pathologies and targeted drug delivery pose significant challenges. Samples of inner ear fluids can only be collected during surgery because the available procedures damage the tiny and fragile structures of the inner ear. Conc

Prevalence of ear diseases in the children of Delhi.

OBJECTIVE: This study aimed to assess the prevalence and profile of ear diseases in children from Delhi, India. METHODS: A population-based cross-sectional door-to-door survey was carried out in two districts of Delhi, and involved children of all demographic sections of the region. A total of 4626 children aged between 18 days and 15 years underwent examinations including otoscopy, impedance audiometry and hearing screening. RESULTS: In all, 14.8 per cent of the study sample was diagnosed with one or more ear morbidities, the most common being cerumen impaction (7.5 per cent) and chronic su

[Stem-cell-based approaches for treating inner ear diseases].

The capacity of stem cells to regenerate lost tissue cells has gained recognition among physicians. Despite the successful use of blood stem cells for treating blood cancers, other stem cell types have not yet been widely introduced into clinical practice. Therapy options involving stem cells for inner ear diseases consequently have not been implemented. Nonetheless, several reports have recently been published describing the generation of morphologically and immunologically distinctive inner ear cell types-such as hair cells, supporting cells, and spiral ganglion neurons-from stem cells. [Correlation between inner ear disorders and temporomandibular joint diseases].

Acute and chronic inner ear diseases involve many etiological factors, some as yet unknown. ENT-specific, orthopedic, hemorrheological, immunological and neurological disorders can affect the cochleovestibular system and induce hearing loss, vertigo and/ortinnitus. We performed a prospective study to analyze factors of the dentognathological system and of the temporomandibular joint that can influence acute and chronic inner ear dysfunctions. A total of 138 patients (49.3% female, 50.9% male) receiving clinical treatment for inner ear dysfunctions (12.3% chronic sensorineural hearing loss, 15.

Middle ear diseases in cleft palate patients in Singapore.

A preliminary prospective study of the correlation between middle ear diseases and cleft palate was started in February 1987. Eighty three patients from the Plastic Surgical Department of the Singapore General Hospital have been screened. Three quarters of the ears screened were normal. Middle ear effusion was the commonest disease especially in younger children found in eighteen of thirty four abnormal ears; all these patients benefited from myringotomy and insertion of a ventilating tube. Other types of middle ear disease included adhesive otitis media, perforation of the eardrum, attic retr

[Therapeutic considerations in vascular diseases of the inner ear].

I propose an analogy between vascular diseases of the inner ear and those of other organs like brain, heart or peripheral vessel diseases in which functional and organic alteration can be objectively demonstrated. Symptomatic disturbances of inner ear circulation are monoform in its clinical appearance and the term of 'otangina' is proposed for the functional result. Three types of vascular distribution are discussed for the different regulatory entities, (I) the proximal cerebral vascular type, (II) the predistal innervated regulatory type and the (III) capillary regulatory type.
