Analysis of the early warning score to detect critical or high-risk patients in the prehospital setting.

The early warning score can help to prevent, recognize and act at the first signs of clinical and physiological deterioration. The objective of this study is to evaluate different scales for use in the prehospital setting and to select the most relevant one by applicability and capacity to predict mortality in the first 48 h. A prospective longitudinal observational study was conducted in patients over 18 years of age who were treated by the advanced life support unit and transferred to the emergency department between April and July 2018. We analyzed demographic variables as well as the physi

Performance of the Obstetric Early Warning Score in critically ill patients for the prediction of maternal death.

BACKGROUND: Every day, about 830 women die worldwide from preventable causes related to pregnancy and childbirth. Obstetric early warning scores have been proposed as a potential tool to reduce maternal morbidity and mortality, based on the identification of predetermined abnormal values in the vital signs or laboratory parameters, to generate a rapid and effective medical response. Several early warning scores have been developed for obstetrical patients, but the majority are the result of a clinical consensus rather than statistical analyses of clinical outcome measures (ie, maternal deaths)

Significant deficiencies in the overnight use of a Standardised Early Warning Scoring system in a teaching hospital.

National Institute for Health and Clinical Excellence guidelines recommend the use of 'Track and Trigger' systems to identify early clinical deterioration. The Standardised Early Warning Score (SEWS) is used in the Royal Infirmary of Edinburgh. Previous work, suggested that the frequency and accuracy of SEWS documentation varied throughout the hospital. A prospective study was performed over a 14-night period looking at SEWS documentation in patients causing clinical concern requiring medical review, or triggering a SEWS of 4 (the 'trigger' score). SEWS charts were examined the following morni

Long-term effect of introducing an early warning score on respiratory rate charting on general wards.

The respiratory rate is an early indicator of disease, yet many clinicians underestimate its importance and hospitals report a poor level of respiratory rate recording. We studied the short- and long-term effects of introducing a new patient vital signs chart and the modified early warning score (MEWS), which incorporates respiratory rate on the prevalence of respiratory rate recording in six general wards of our hospital. Prior to the commencement of the study, the average percentage of occupied beds where at least one respiratory rate recording had been made in a single 24-h period was 29.5+

Enhancing Nurses' Capability to Promptly Escalate Care Using the Early Warning Score: A Quality Improvement Initiative.

BACKGROUND: Clinical deterioration in general ward patients is largely preventable with early recognition of abnormal vital signs and prompt escalation of care. LOCAL PROBLEM: Limited knowledge of the early warning score (EWS) and an inability to recognize and respond to clinical deterioration among general ward nurses led to delays in escalation of care. METHODS: This quality improvement project used iterative Plan-Do-Study-Act cycles. Outcomes included knowledge, EWS protocol compliance, and care escalation for patients with an EWS score ≥5 to prevent further deterioration. INTERVENTIONS:

Evaluation and improvement of the National Early Warning Score (NEWS2) for COVID-19: a multi-hospital study.

BACKGROUND: The National Early Warning Score (NEWS2) is currently recommended in the UK for the risk stratification of COVID-19 patients, but little is known about its ability to detect severe cases. We aimed to evaluate NEWS2 for the prediction of severe COVID-19 outcome and identify and validate a set of blood and physiological parameters routinely collected at hospital admission to improve upon the use of NEWS2 alone for medium-term risk stratification. METHODS: Training cohorts comprised 1276 patients admitted to King's College Hospital National Health Service (NHS) Foundation Trust with

Development and validation of a machine learning based early warning scoring system for high altitude polycythemia.

BACKGROUND: High-altitude polycythemia (HAPC) lacks a lifestyle-focused risk-stratification tool among lifelong high-altitude residents. Here we aimed to develop and validate a novel machine-learning predictive scoring system for HAPC using readily modifiable lifestyle variables in this population. METHODS: In a high altitude cohort (≥4,500 m, n = 1,089), 82 candidate variables were reduced to seven lifestyle predictors via LASSO, Logistic regression, XGBoost and random forest models were trained and compared (10 fold cross validation). RESULTS: Logistic regression achieved the best balance

Clinical Study of a New Modified Early Warning Scoring System for Rapidly Evaluating Shock in Adults.

BACKGROUND: Shock is one of the most common severe syndromes requiring emergency treatment. Acute myocardial infarction guidelines, the surviving sepsis campaign, and low blood volume resuscitation guidelines indicate the prioritization of early identification of shock. APACHE II (Acute Physiology and Chronic Health Evaluation II), SOFA (Sequential Organ Failure Assessment), and MEWS (Modified Early Warning System) scores are used to predict mortality in ICU (intensive care unit) patients. However, similar to APACHE II, SOFA cannot be used for rapid assessment. Hence the need for a new scoring

Development of a preoperative Early Warning Scoring System to identify highly suspect COVID-19 patients.

BACKGROUND AND AIMS: The coronavirus disease 2019 (COVID-19) is spreading at an unprecedented speed. Lack of resources to test every patient scheduled for surgery and false negative test results contribute to considerable stress to anesthesiologists, along with health risks to both caregivers and other patients. The study aimed to develop an early warning screening tool to rapidly detect 'highly suspect' among the patients scheduled for surgery. METHODS: Review of literature was conducted using terms 'coronavirus' OR 'nCoV 2019' OR 'SARS-CoV-2' OR 'COVID-19' AND 'clinical characteristics' in
