Tourette syndrome. The pediatric perspective.

I report the clinical details of Tourette syndrome in 15 children. The condition typically starts at age 6 years with eyeblinking, and the child soon develops other tics and abnormal vocalizations. Coprolalia and echolalia occure but are infrequent. The average delay in correct diagnosis in this series was four years. Treatment with haloperidol produces a good or excellent response in three quarters of the patients. Many of the children have a history of encephalopathic events, "soft signs" on neurologic examination, and problems in school. Personal and social adjustmen are generally good, how

Latah: an Indonesian startle syndrome.

The nature of culture-specific startles syndromes such as "Latah" in Indonesia and Malaysia is ill understood. Hypotheses concerning their origin include sociocultural behavior, psychiatric disorders, and neurological syndromes. The various disorders show striking similarities despite occurring in diverse cultural settings and genetically distant populations. They are characterized clinically by exaggerated startle responses and involuntary vocalizations, echolalia, and echopraxia. Quantifying startle reflexes may help define Latah within the 3 groups of startle syndromes: (1) hyperekplexia, (

Jumping Frenchmen of Maine.

The "Jumping Frenchmen of Maine" were described by George Beard in 1878. They had an excessive startle response, sometimes with echolalia, echopraxia, or forced obedience. In 1885, Gilles de la Tourette concluded that "jumping" was similar to the syndrome that now bears his name. Direct observations of jumpers have been scarce. We studied eight jumpers from the Because region of Quebec. In our opinion, this phenomenon is not a neurologic disease, but can be explained in psychological terms as operant conditioned behavior. Our cases were related to specific conditions in lumber camps in the 19t

A perplexing document in the early history of Gilles de la Tourette Syndrome: Melotti's rendition of a "Lecture of Charcot" (including a complete translation from the Italian with commentary).

In 1885, Dr. Guilio Melotti published an Italian translation of a lecture on "Convulsive Tics with Coprolalia and Echolalia" given by Jean-Martin Charcot. Although this lecture often has been cited as an authoritative statement of Charcot's view, until now it has not been translated into English. The lecture presents a number of statements that appear nowhere else in Charcot's published corpus, including some that seem to contradict Charcot's other pronouncements on maladie des tics. Although the Melotti-Charcot lecture may portray Charcot's position accurately in many passages, the article mo

Tourette syndrome.

Tourette syndrome (Gilles de la Tourette disease) is a disorder of involuntary muscular tics, vocalizations and compulsive behavior. The tics and muscle movements vary in form and course; the complex repetitive patterns are eventually replaced by other patterns. The vocalization may be in the form of sounds, words or profanities and sometimes echolalia, echopraxia and palilalia. The onset may be from age two to 15 but is usually between ages eight and 12. Recent studies suggest that there is a hypersensitivity of dopamine receptors. Most patients respond well to haloperidol, but other drugs th

Progressive Supranuclear Palsy with Predominant Speech/Language Disorder.

We herein report a rare case of progressive supranuclear palsy (PSP) that initially presented as progressive fluent aphasia and echolalia. A 75-year-old woman exhibited significant language impairments for 2 years before developing motor symptoms, including vertical gaze palsy and frequent falls. Neuroimaging revealed asymmetric frontal and midbrain atrophy, with reduced perfusion in the left frontal and parietal lobes. This case highlights the importance of recognizing atypical PSP presentations, particularly language-dominant variants that may mimic other neurodegenerative disorders. Early i

[From tics to the Gilles de la Tourette syndrome].

We report a case whose progressive course and forms of manifestation of generalized tic movements indicate relatively rare Gilles de la Tourette disease. It is characterized by the triad--non-coordinated mobility-dyskinesia, echolalia corporalia, epilepsy and according to the current data from the literature, compulsive behavior, phobias and sometimes episodes of insanity in the advanced stage. The clinical case we report as well as cases from the world literature is rather dubious, asks for prolonged follow-up and has polymorphic symptomatology and necessitates a multidisciplinary therapeutic

[Current theories on the etiology and treatment of Gilles de la Tourette's disease].

Gilles de la Tourette syndrome is a condition marked by: (1) onset usually in childhood and adolescence, i.e. between 2 and 15 years of life; (2) violent facial tics and echolalia; (3) increased excitability and apathy; (4) progressive increase in symptoms intensity; (5) chronic course. This syndrome is threefold more frequent in men than in women. None hypothesis concerning its etiopathogenesis (genetic, organic, organic-functional, psychomotor, and mixed) does explain its origin. Many cases respond with some degree of relief to neuroleptics, carbamazepine, clonidine, and glucocorticosteroids

Gilles de la Tourette syndrome. Interactions with other neuropsychiatric disorders.

Gilles de la Tourette syndrome is a neuropsychiatric disease with a childhood onset below age 16, characterized by chronic involuntary movements, obsessions, compulsions, utterances, echolalia, coprolalia and aggressive behavior symptoms. Two unique case histories are described in this report, one with an intercurrent history of primary anorexia nervosa and the other with rheumatic encephalitis. They were successfully treated; the former with chlorimipramine, and the latter with combination of L-tryptophan, nicotinic acid, and pyridoxine HCl. These two cases illustrate the possibility that one

The psychopathology of echophenomena.

Repetition of other person's activities is, to some extent, part of normal human behaviour. However, in some circumstances it becomes pathological and symptomatic of an underlying disease process. The pathogenesis of this type of behaviour is very diverse. The circumstances in which echoing can be expected to occur are reviewed.

Phencyclidine poisoning.

Phencyclidine hydrochloride abuse has become increasingly common and should be considered in patients with unexplained acute psychosis, dystonic reactions, status epilepticus, or coma. Two phencyclidine-intoxicated patients had bizarre combinations or disorientation, hallucination, agitation, and dyskinetic motor activity. Supportive care and reduction of sensory stimulation are the basis for management of the symptoms.

Catatonia in a psychiatric setup having diverse etiology: A case series.

Catatonia is a neuropsychiatric syndrome characterized by stupor, mutism, negativism, posturing, mannerisms, stereotypies, waxy flexibility, catalepsy, grimacing, echolalia, and echopraxia. This syndrome has a plethora of etiologies that arise from neurological and medical conditions, psychiatric illnesses, and withdrawal of psychoactive substances. Adding to this, it has numerous differentials, including an acute confusional state, which often adds to the diagnostic dilemma. The incidence of catatonia is between 5 and 20%. A comprehensive workup of the patient is required to rule out organic

Rethinking language in autism.

In this article, we invite a rethinking of traditional perspectives of language in autism. We advocate a theoretical reappraisal that offers a corrective to the dominant and largely tacitly held view that language, in its essence, is a referential system and a reflection of the individual's cognition. Drawing on scholarship in Conversation Analysis and linguistic anthropology, we present a multidimensional view of language, showing how it also functions as interactional accomplishment, social action, and mode of experience. From such a multidimensional perspective, we revisit data presented by

Acute Catatonia Following a Cerebellar Stroke: A Case Report.

Catatonia can present with a wide spectrum of psychomotor symptoms and should be considered in the differential diagnosis of hospitalized patients with speech and motor difficulties. Catatonia is defined as the presence of three or more of the following: catalepsy, waxy flexibility, stupor, agitation, mutism, negativism, posturing, mannerisms, stereotypies, grimacing, echolalia, and echopraxia. In this case, a 72-year-old black woman was admitted with difficulty with speech and ambulation and found to have a cerebellar stroke on a brain MRI. However, her symptoms of variable rigidity, mutism,

Otitis and autism spectrum disorders.

The case of a 5-year-old child diagnosed as having pervasive developmental disorder (PDD), autistic type, from age 1 is reported. After surgery of vegetation in middle ear for repetitive otitis, the child presented an improvement in autistic behaviours, previously expressed as impaired social interactions, qualitative abnormalities in communication, a marked delay in language development, echolalia, stereotypies and self-aggressive behaviours. The aim of this paper is to bring attention to occurrences of misdiagnosis of PDD, which can occur when an adequate screening of the autistic syndrome i

Sign language tics in a prelingually deaf man.

Gilles de la Tourette syndrome is characterized by vocal and motor tics starting in childhood. Vocal tics may be either noises or words, and the vocal language tics may consist of obscenities (coprolalia) and repetitions of speech that has been heard (echolalia). We describe a prelingually deaf man who has the full array of tics seen in Gilles de la Tourette syndrome, but in whom vocal language tics are replaced by equivalent sign language tics. This is, to our knowledge, the first report of sign language tics in a person with prelingual deafness. The implications of this phenomenon for the se

Zolpidem in Elderly Patients With Acute Treatment Resistant Catatonia: A Case Report.

Catatonia is defined by the Diagnostic and Statistical Manual of Mental Disorders-fifth Edition (DSM-5) as the presence of 3 or more of the following symptoms: catalepsy, waxy flexibility, stupor, agitation, mutism, negativism, posturing, mannerisms, stereotypies, grimacing, echolalia, and echopraxia. Screening instruments, such as the Bush Francis Catatonia Rating Scale (BFCRS) or the Northoff Catatonia Scale, are utilized to screen and track the severity and course of symptoms. The primary pharmacologic treatment for catatonia is the lorazepam challenge test. Second-line pharmacologic therap

Autism, mental retardation, and chromosomal abnormalities.

There are reports of sex chromosomal abnormalities including XXY, XYY, and fragile X karyotypes in autistic individuals, but structural autosomal defects have rarely been reported. This paper presents four patients with autism, mental retardation, minor dysmorphic features, and structural autosomal defects. These patients shared autistic features including fascination with inanimate objects, catastrophic reactions to changes in their environment or their daily routine, echolalia, and poor relatedness; IQ scores indicate mild to severe retardation. Their autosomal abnormalities included inversi
