[Supra and intercondylar elbow fractures in children].

Supra- and intercondylar elbow fractures are rare in children. The treatment of these fractures is controversial. The purpose of this report is to discuss diagnosis and treatment of this unusual injury. Our study looked at nine patients between six and 15 years old (average age: 11.5). In four patients, the fracture was T-condylar and in five patients, the fracture was epiphyseal-diaphyseal. Communition was noted in five cases. All fractures are secondary to high-energy trauma. All patients were treated by open reduction and internal fixation. Three patients had associated nervous lesions. Pat

Congruent elbow plate fixation of olecranon fractures.

OBJECTIVES: We hypothesize that clinical results and patient outcomes following treatment of olecranon fractures with a congruent elbow plating system will be comparable to other available plating systems. Our results will be compared to previously published reports. DESIGN: Retrospective study. SETTING: Level 1 academic referral center. PATIENTS/PARTICIPANTS: The trauma registry was reviewed to identify all olecranon fractures treated with open reduction and internal fixation between January 2001 and December 2004 using the Mayo Congruent Elbow Plate system. Thirty-two patients were identi

Total Elbow Arthroplasty for Distal Humerus Fractures.

Total elbow arthroplasty is a good treatment alternative for selected patients with distal humerus fractures. Its attractiveness is related to several factors, including the possibility of performing the procedure; leaving the extensor mechanism intact; faster, easier rehabilitation compared with internal fixation; and overall good outcomes reported in terms of both pain relief and function. Implant failure leading to revision surgery does happen, and patients must comply with certain limitations to extend the longevity of their implant. Development of high-performance implants may allow expan

Total elbow arthroplasty for distal humerus fractures.

Total elbow arthroplasty has become increasingly popular for the treatment of distal humerus fractures in elderly patients with poor bone quality, comminution, and/or pre-existent elbow abnormalities. The procedure is performed without violating the extensor mechanism; the fractured fragments are exposed and resected on both sides of the triceps, and the components can be implanted through the same exposure. Early outcomes are satisfactory in most elbows and compare favorably with internal fixation in this same group of elderly patients. Advances in elbow arthroplasty for fractures will likely

Early mobilisation for elbow fractures in adults.

BACKGROUND: A fall on the outstretched arm can result in an elbow fracture. Loss of elbow function is a common problem with these fractures and can have major implications for functional capabilities. It is unknown whether early mobilisation can improve functional outcome without increasing complications. OBJECTIVES: To compare the effects (benefits and harms) of early mobilisation versus delayed mobilisation of the elbow after elbow fractures in adults. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (August 2010), the Cochrane Central Regi

Flexion-type supracondylar elbow fractures in children.

Flexion-type supracondylar elbow fractures are uncommon in children. Minimally displaced fractures were treated in plaster cast in 7 children, and 22 children with a displaced fracture were treated by closed reduction and percutaneous pinning. Results after an average follow-up of 6.3 years showed that the treatment of choice yielded excellent or good results in 86.2% of the patients. All patients were satisfied with the end result and had normal use of their elbow.

Relevancy of radiographic features in elbow fractures.

PURPOSE: To correlate radiographic and clinical findings of elbow fractures. MATERIAL AND METHODS: A retrospective study was made of 110 consecutive adult elbow fractures of various types involving the humerus, the ulna, and the radius. RESULTS: Twenty-seven fractures with radiographically conspicuous distortions healed. Of these only 15 had clinical sequelae. Five had clinical sequelae without showing conspicuous radiographic distortions after healing. CONCLUSION: For radiographic evaluation and classification purposes, it would seem to be important to note the status of the capitellum, th

[TOTAL ELBOW REPLACEMENT AS AN ALTERNATIVE TO SEVERELY COMMINUTED FRACTURES AROUND THE ELBOW].

BACKGROUND: Intra-articular elbow fractures are considered to be one of the most complex injuries in orthopedic trauma. Some are too comminuted for open reduction and internal fixation. Recently, total elbow replacement (TER) has gained popularity for the treatment of comminuted elbow trauma when other treatment options are not possible. METHODS: Since 2007-2013 we treated 18 patients with TER due to comminuted distal humeral fractures. We used the Disabilities of the Arm, Shoulder and Hand scoring system (DASH) to evaluate the patient's satisfaction. In addition we evaluated the elbow range

Elbow fracture dislocation with associated olecranon avulsion fracture.

Complex elbow fracture-dislocations typically include ulnohumeral joint dislocation, radial head and coronoid process fractures and ligamentous disruption and are termed a terrible triad injury. In this case report, we describe a mid-60s female patient with a direct posterior elbow dislocation with comminuted radial head and coronoid process fractures combined with an olecranon avulsion fracture without ligamentous disruption. Surgical intervention was indicated to restore elbow stability and extensor mechanism via posterior approach to the elbow. Given radial head and coronoid fracture commin

Adolescent elbow osteochondral lesions following prior elbow fracture pinning.

PURPOSE: Pinning of pediatric elbow fractures has been shown to be a safe procedure with a low complication profile. This study identified patients who underwent cartilage surgery for elbow osteochondral lesions or osteochondritis dissecans who had prior ipsilateral elbow pinning. METHODS: Records of patients who underwent ipsilateral cartilage surgery for osteochondritis dissecans and prior percutaneous pinning for elbow fractures were identified. Demographics were compiled and the clinical, radiographic, and surgical results were tabulated for patients with at least 1-year of follow-up from

Does Contralateral Elbow Radiography Improve Diagnostic Accuracy of Paediatric Elbow Fractures amongst Emergency Residents?

Background: Paediatric elbow fractures are difficult to diagnose because secondary ossification centres can obscure radiographic interpretation. Emergency physicians often request contralateral elbow radiographs to improve diagnostic accuracy, although the benefit of this practice remains controversial. This study aimed to evaluate the impact of contralateral elbow radiographs and a targeted educational intervention on the diagnostic accuracy of emergency residents interpreting paediatric elbow fractures. Methods: This diagnostic study included 28 emergency residents at a tertiary hospital. Pa

Heterotopic ossification formation after fractures about the elbow.

INTRODUCTION: Heterotopic ossification (HO) is a well-known sequela after an elbow injury and is widely studied given the associated morbidity. The anatomic location of HO development for specific elbow injuries has not been reported. The purpose of this study was to describe the precise, anatomic location of HO development after different peri-articular elbow injuries. METHODS: A retrospective chart review was performed for patients with peri-articular elbow fracture and/or dislocation who underwent an elbow contracture release. The injuries were grouped into coronal shear distal humerus/AO

Valgus deformity following supracondylar elbow fractures in children.

We reviewed the cases of 10 patients with cubitus valgus deformity who had a supracondylar elbow fracture during childhood, in order to identify the causes of valgus deformity. Cubitus valgus deformity occurred in 3 cases because of failure to recognize the deformity at the initial treatment. In 7 patients the deformity was recognized but was insufficiently treated. In this series there was no evidence of progressive valgus deformity after healing of the fracture. Incomplete correction is the main cause of valgus deformity. It is suggested that, in order to reduce the incidence of valgus defor

[Intra-articular fractures of the elbow in childhood].

Correct assessment of the prognosis is a special problem in intra-articular fractures of the elbow in children. Pseudarthrosis, which is other-wise not seen in children, is often observed in fractures of the radial condyle but, together with other post-traumatic dislocations (fishtail deformity), is of little functional significance. In principle, an operation should be performed even on slightly dislocated fragments at the distal humeral joint surface. Intra-articular fractures of the proximal ulna also tend to result in persistent dislocations and therefore require exact repositioning. Fract

Delayed treatment of supracondylar elbow fractures in children.

BACKGROUND: Supracondylar fractures are the most common elbow fractures in children and are usually treated on an emergency basis, using percutaneous pinning. However, the treatment is often delayed in areas where healthcare resources are scarce. HYPOTHESIS: Delaying treatment does not influence the perioperative complication rate. MATERIALS AND METHOD: We retrospectively reviewed the medical charts of 89 children aged 2 to 15 years in whom surgery for extension-type supracondylar elbow fractures was delayed by more than 48 hours. The 53 boys and 36 girls with a mean age of 6 years 9 months

Periprosthetic humeral fractures after shoulder and elbow arthroplasty.

Due to rapidly increasing numbers of arthroplasty surgeries of the upper extremity, periprosthetic humeral fractures after shoulder and elbow arthroplasty, formerly described as rare, may hence increase in the near future. Therefore the aim of the present work was to give an overview of the existing literature including possible classifications as well as an update on treatment concepts and experiences with own cases. After a literature research have been done, existing prevalence, classifications and treatment options, mostly described in case series, were processed to create an overview of t

Missed elbow fractures misdiagnosed as radial head subluxations.

Over a 36-month period, 11 children were treated in our department for fractures about the elbow which initially went undiagnosed, as they had first undergone reduction maneuvers for a suspected radial head subluxation, without prior imaging. The most frequent final diagnosis was supracondylar humerus fracture (n = 4). A fall was reported for six children (54.5%); the mechanism of injury remained unknown in four children (36.4%). In four of eleven children (36.4%) there was moderate or major soft tissue swelling at the time of first presentation, which is uncommon in radial head subluxation. R
