Compartment syndrome: Neuromuscular complications and electrodiagnosis.

Compartment syndrome (CS) is a treatable condition characterized by elevated intracompartmental pressure and may be acute or chronic in nature. Sustained elevated compartment pressure can lead to ischemia and necrosis of muscle as well as injury to peripheral nerves, creating a deficit that may be neuropathic, myopathic, or mixed. While electrodiagnostic assessments are of limited utility in the diagnosis of acute/traumatic CS, they can assist with prognosticating return of function after surgical fasciotomy or in clarifying the injury pattern in cases in which a traumatic injury results in su

[Electrodiagnosis of ALS: its practical aspects].

Electrodiagnostic methods, especially needle EMG plays an important role for the ALS diagnosis. Existing diagnostic criteria such as revised El Escorial criteria (R-EEC) and Awaji algorithm have a drawback of low sensitivity. Our study revealed that the percentage of patients classified as confirmed ALS, i.e. clinically probable (laboratory supported) or higher, was 43% using the R-EEC and 37% using the Awaji algorithm. Needle EMG can strongly suggest ALS beyond these criteria. Fasciculation potentials (FPs) are sufficiently specific for ALS, and we have argued its diagnostic utitlity in ALS d

Electrodiagnosis of mild carpal tunnel syndrome.

Electrophysiologic tests have been reported to detect mild carpal tunnel syndrome (CTS). Such tests include (i) absolute palmar latency of median wrist segment; (ii) comparison of median and radial distal sensory latencies in digit I; (iii) comparison of median and ulnar distal sensory latencies in digit IV; (iv) comparison of median and ulnar palmar latencies; (v) comparison of median and ulnar sensory potential amplitudes in digits II and V. To clarify the clinical utility of these tests, the parameters of all five tests were determined across four carefully established patient subgroups: gr

Electrodiagnosis of striated urethral sphincter dysfunction.

In many centers urethral striated sphincter electromyography has become an essential component of urodynamic testing. The urologist who uses this modality of investigation in an attempt to understand the pathophysiology of striated sphincter dysfunction must have some basic knowledge of electromyography. Attempts have been made to provide such basic information, especially relating to the methods of evaluation and interpretation. We also have incorporated our own views based on our experience with more than 200 subjects with neuropathic vesicourethral dysfunctions.

Cervical radiculopathy for neurologists: the role of electrodiagnosis.

Cervical radiculopathy (CR) is a common condition encountered in the general population, usually related to a musculoskeletal degenerative condition. Conventional electroneuromyography (ENMG) consists of nerve conduction studies (NCS) and needle electromyography (EMG), and it is regarded as the most specific diagnostic evaluation in this scenario. Although CR is commonly encountered in clinical practice, ENMG as a diagnostic tool is not often discussed in neurology residency programs. Electromyography has demonstrated modest sensitivity (50-71%) but excellent specificity (approaching 100%) for

Late Responses in the Electrodiagnosis of Cervical Radiculopathies.

BACKROUND AND AIM: This paper aims to assess the parameters of late responses and then determine their usefulness in patients with cervical radiculopathy. PATIENTS AND METHODS: We studied a total of 114 patients with bilateral assessment of median and ulnar nerves and of F and A waves parameters. RESULTS: We draw attention to the need of bilateral electrodiagnostic examination due to changes occurring in a third of cases also in the asymptomatic limb. Pluriradicular injuries occurred in one third of cases. The root that was most commonly affected in cases of uniradicular lesion was C7. The p

Electrodiagnosis in persons with amyotrophic lateral sclerosis.

Electrophysiology remains an important tool in the evaluation of patients presenting with signs and symptoms of motor neuron disease. The electrodiagnostic study should include peripheral nerve conduction studies and needle electromyography to both exclude treatable disease and gather evidence regarding a diagnosis of amyotrophic lateral sclerosis (ALS). The recent changes in the revised El Escorial criteria, recommended by the Awaji-shima consensus group, have increased the diagnostic significance of fasciculation potentials to equal that of fibrillation and positive sharp-wave potentials in

Pathogenesis and electrodiagnosis of cubital tunnel syndrome.

BACKGROUND: Cubital tunnel syndrome is a well-recognized clinical condition and is the second most common peripheral compression neuropathy. This study was designed to investigate the causes of cubital tunnel syndrome by surgical means and to assess the clinical value of the neurophysiological diagnosis of cubital tunnel syndrome. METHODS: Twenty-one patients (involving a total of 22 limbs from 16 men and 5 women, aged 22 to 63, with a mean age of 49 years) with clinical symptoms and signs indicating a problem with their ulnar nerve underwent motor conduction velocity examinations at differen

The electrodiagnosis of ulnar nerve entrapment at the elbow.

Entrapment of the ulnar nerve at the elbow is the second most common focal peripheral neuropathy. Recent advances have facilitated the electrodiagnosis of this common nerve entrapment. The goals of electrodiagnosis are to localize ulnar nerve dysfunction, confirm that the disturbance is confined to the ulnar nerve, and assess the severity of ulnar nerve dysfunction. The goal of this review is to highlight the important advances in anatomy, neurophysiology and methodology that impact upon the electrodiagnosis of entrapment of the ulnar nerve at the elbow, illustrate the limits of electrodiagnos

Electrodiagnosis in entrapment neuropathy by the arcade of Struthers.

Two cases of high ulnar nerve neuropathy are reported. Lesions were localized at the midarm level by electrophysiologic studies. In the first case, the lesion was found mainly to be a prolonged neurapraxia, and neurolysis was effective. The ulnar nerve was swollen 1 cm in length under the arcade of Struthers. After neurolysis, the palsy recovered rapidly. In the other case, the lesion seemed to be a mild injury to the myelin sheath. Delayed segmental conduction velocity and partial conduction block were found at the midarm level. The paresis improved slightly during the 11-month followup witho

[Prognostic value of electrodiagnosis of Bell's palsy].

Many papers report on a poor rate of complete restitution of Bell's palsy if signs of degeneration can be detected in neuromyography (NMG) or electromyography (EMG). In 119 patients who underwent infusion therapy (as developed by Stennert) 39% showed signs of degeneration in EMG or NMG. Complete restitution was achieved in 93% of these patients. Degeneration was more frequent in elderly patients (< 20 years: 20%, > 60 years: 55%). This did not affect the rate of complete restitution, which was constantly high for every age. If infusion therapy was started within 7 days after onset of the disea

Electrodiagnosis reliability in the diagnosis of infant botulism.

Infant botulism is confirmed by isolation of Clostridium botulinum from stool culture or by toxin assay. Although electrodiagnosis has been described as a diagnostic tool in infant botulism, our 11-year review of toxin-confirmed cases suggests that electrodiagnosis is not a reliable tool. In the case report presented, results of electrodiagnosis were negative but enema effluent contained adequate concentrations of organism and toxin to confirm the diagnosis.

Clinical features and electrodiagnosis of ulnar neuropathies.

In this review, we delineate clinical, electrodiagnostic, and radiographic features of ulnar mononeuropathies. Ulnar neuropathy at the elbow (UNE) is most commonly due to lesions at the level of the retroepicondylar groove (RTC), with approximately 25% at the humeroulnar arcade (HUA). The term 'cubital tunnel syndrome' should be reserved for the latter. The diagnostic accuracy of nerve conduction studies is limited by biological (e.g. low elbow temperature) and technical factors. Across-elbow distance measurements greater than 10 cm improve diagnostic specificity at the expense of decreased se

Electrodiagnosis of botulism and clinico-electrophysiological correlation.

OBJECTIVE: An electrophysiological study can help to confirm the diagnosis of botulism. This study was aimed at validating a simple and reliable electrodiagnostic test and at correlating the findings with clinical severity. METHODS: Pre- and post-exercise single supramaximal stimulations (SSSs) were performed in 63 patients with botulism. The sensitivity and specificity of amplitude of compound muscle action potential (CMAP) and percentage increment (PI) of SSS were determined. These two parameters were then correlated with respiratory failure. The relationship between the amplitude of CMAP a

[Relevance of combined electrodiagnosis in facial hyperkinesia].

We present a combined electrophysiological diagnostic procedure for patients with facial hyperkinesia. With the new method of magnetic stimulation a central cortical stimulation is possible to represent the total way of the facial nerve. The combination of magnetic stimulation with other methods such as the blink reflex may lead to topodiagnostic statements in facial hyperkinesias.
