Restitution metrics in Brugada syndrome: a systematic review and meta-analysis.

BACKGROUND: Brugada syndrome (BrS) is an ion channelopathy that predisposes affected subjects to ventricular tachycardia/fibrillation (VT/VF) and sudden cardiac death. Restitution analysis has been examined in BrS patients but not all studies have reported significant differences between BrS patients and controls. Therefore, we conducted a systematic review and meta-analysis to investigate the different restitution indices used in BrS. METHODS: PubMed and Embase were searched until April 7, 2019, identifying 20 and 27 studies. RESULTS: A total of ten studies involving 178 BrS (mean age 38 ye

Interventional electrophysiology: catheter ablation for patients with cardiac arrhythmias.

Catheter ablative techniques have introduced an exciting new chapter of interventional therapy for patients with drug-resistant cardiac arrhythmias. A variety of techniques are presently available, and proper choice depends on proper localization of the tachycardia focus and mechanism of the arrhythmia. These techniques should be performed only in laboratories with personnel experienced in invasive electrophysiologic procedures. Over the past 5 years, we have learned that catheter ablation of the AV junction for patients with drug-resistant supraventricular tachycardia has replaced the need fo

Is it feasible to offer 'targeted ablation' of ventricular tachycardia circuits with better understanding of isthmus anatomy and conduction characteristics?

Successful mapping and ablation of ventricular tachycardias remains a challenging clinical task. Whereas conventional entrainment and activation mapping was for many years the gold standard to identify reentrant circuits in ischaemic ventricular tachycardia ablation procedures, substrate mapping has become the cornerstone of ventricular tachycardia ablation. In the last decade, technology has dramatically improved. In parallel to high-density automated mapping, cardiac imaging and image integration tools are increasingly used to assess the structural ventricular tachycardia substrate. The aim

A modern automated patch-clamp approach for high throughput electrophysiology recordings in native cardiomyocytes.

Crucial conventional patch-clamp approaches to investigate cellular electrophysiology suffer from low-throughput and require considerable experimenter expertise. Automated patch-clamp (APC) approaches are more experimenter independent and offer high-throughput, but by design are predominantly limited to assays containing small, homogenous cells. In order to enable high-throughput APC assays on larger cells such as native cardiomyocytes isolated from mammalian hearts, we employed a fixed-well APC plate format. A broad range of detailed electrophysiological parameters including action potential,

Comprehensive strategies to minimize radiation exposure during Interventional electrophysiology procedures: state-of-the-art review.

INTRODUCTION: Cardiac electrophysiology (EP) procedures are frequently performed in patients with cardiac arrhythmias, chronic heart failure, and sudden cardiac death. Most EP procedures involve fluoroscopy, which results in radiation exposure to physicians, patients, and EP lab staff. Accumulated radiation exposure is a known health detriment to patients and physicians. AREA COVERED: This review will summarize radiation exposure, dose metrics, complications of radiation exposure, factors affecting radiation exposure, minimizing radiation exposure, zero or near-zero fluoroscopy strategies, an

Accuracy of automatic abnormal potential annotation for substrate identification in scar-related ventricular tachycardia.

INTRODUCTION: Ultrahigh-density mapping for ventricular tachycardia (VT) is increasingly used. However, manual annotation of local abnormal ventricular activities (LAVAs) is challenging in this setting. Therefore, we assessed the accuracy of the automatic annotation of LAVAs with the Lumipoint algorithm of the Rhythmia system (Boston Scientific). METHODS AND RESULTS: One hundred consecutive patients undergoing catheter ablation of scar-related VT were studied. Areas with LAVAs and ablation sites were manually annotated during the procedure and compared with automatically annotated areas using

Clinical intracardiac electrophysiologic testing: technique, diagnostic indications, and therapeutic uses.

Clinical cardiac electrophysiologic testing has evolved rapidly since 1968, when the technique was first described. In an electrophysiologic study, electrode catheters are positioned within the heart to record electrical activity from the atrium, atrioventricular conduction tissue, and ventricle. Programmed stimulation is then performed, which involves pacing of the atrium or ventricle and introducing critically timed premature stimuli during sinus rhythm or paced rhythm. The use of programmed stimulation in conjunction with intracardiac recordings in electrophysiologic studies has facilitated

Hexagonal Multielectrode Design for Recording Local Electrograms With Superior Characteristics.

Electrograms recorded with a local coaxial reference offer advantages over bipolar electrograms for activation mapping but require more electrodes. We demonstrate that these coaxial electrograms can be recorded with a triangular local reference that can be efficiently created using hexagonal multielectrode grids. Triangular and cross-shaped local references recorded with a custom multielectrode were virtually indistinguishable. In contrast to bipolar electrograms, the morphology of the coaxial electrograms was insensitive to electrode orientation relative to the activation front, and the activ

Alterations in atrial electrogram amplitude as steady sinus rhythm transitions to paroxysmal atrial fibrillation during continuous monitoring in patients with implantable cardiac devices: Insights from the IMPACT study.

OBJECTIVES: We aimed to evaluate whether device measured amplitudes of atrial electrogram (AEGM) would change when measured in sinus rhythm (SR) transitioning to paroxysmal atrial fibrillation (AF) from previous steady SR, and significance of such change. METHODS: From the IMPACT trial's database we selected two groups; (A) those who developed AF (n = 164), and (B) propensity-matched control (n = 459) who stayed in SR during continuous Home Monitoring (HM) to compare AEGMs amplitudes at baseline SR and transition phase. RESULTS: During 420.0 ± 349.2 days (mean ± SD) from first postenrollment

Catheter ablation targeting complex fractionated atrial electrograms for the control of atrial fibrillation.

PURPOSE OF REVIEW: Catheter ablation of persistent atrial fibrillation has a lower success rate than for paroxysmal atrial fibrillation, due to structural, electrical and anatomical remodeling of the left atrium. Ablation strategies targeting complex fractionated atrial electrograms (CFAEs) are commonly employed to identify and eliminate the substrate responsible for atrial fibrillation maintenance in persistent cases. This review discusses the pathophysiology as well as the role of targeting CFAEs in catheter ablation of atrial fibrillation. RECENT FINDINGS: As our understanding regarding th

Effects of cromakalim, pinacidil and nicorandil on cardiac refractoriness and arterial pressure in open-chest dogs.

The cardiac electrophysiologic effects of the potassium channel activators cromakalim, pinacidil and nicorandil were determined in anesthetized open-chest normotensive dogs using conventional surface electrogram recording techniques. Intravenous administration of cromakalim (0.025-0.5 mg/kg), pinacidil (0.1-2.0 mg/kg) and nicorandil (0.1-2.5 mg/kg) produced large dose-dependent decreases in arterial blood pressure accompanied by smaller reductions of atrial and ventricular effective refractory periods. The shortening of refractoriness was more pronounced in the atrium than in the ventricle and

Electrophysiologic characteristics of the spontaneous onset and termination of atrial fibrillation.

Electrophysiologic characterization of the onset and termination of atrial fibrillation (AF) is poorly defined. Our study population consisted of 21 consecutive patients (mean age 58 +/- 9 years, 6 women) with intermittent (10 patients) or persistent (11 patients) AF. Mapping of the left atrium (LA) and the right atrium (RA) during initiation and termination of AF was performed with a 64-electrode basket catheter. A total of 92 spontaneous AF onsets (in 16 patients) and 63 spontaneous AF terminations were analyzed. Irrespective of the origin of the triggering atrial premature complex (APC), th

Ventricular voltage-gated ion channels: Detection, characteristics, mechanisms, and drug safety evaluation.

Cardiac voltage-gated ion channels (VGICs) play critical roles in mediating cardiac electrophysiological signals, such as action potentials, to maintain normal heart excitability and contraction. Inherited or acquired alterations in the structure, expression, or function of VGICs, as well as VGIC-related side effects of pharmaceutical drug delivery can result in abnormal cellular electrophysiological processes that induce life-threatening cardiac arrhythmias or even sudden cardiac death. Hence, to reduce possible heart-related risks, VGICs must be acknowledged as important targets in drug disc

Optogenetic current in myofibroblasts acutely alters electrophysiology and conduction of co-cultured cardiomyocytes.

Interactions between cardiac myofibroblasts and myocytes may slow conduction and generate spontaneous beating in fibrosis, increasing the chance of life-threatening arrhythmia. While co-culture studies have shown that myofibroblasts can affect cardiomyocyte electrophysiology in vitro, the extent of myofibroblast-myocyte electrical conductance in a syncytium is unknown. In this neonatal rat study, cardiac myofibroblasts were transduced with Channelrhodopsin-2, which allowed acute and selective increase of myofibroblast current, and plated on top of cardiomyocytes. Optical mapping revealed signi

Radiation safety and ergonomics in the electrophysiology laboratory: update on recent advances.

PURPOSE OF REVIEW: Risks associated with exposure to ionizing radiation in patients undergoing electrophysiology procedures and interventional cardiac electrophysiologists performing these procedures are a serious concern. Strategies to reduce radiation exposure are of obvious importance. In addition, interventional cardiac electrophysiologists have to perform procedures wearing heavy lead protection for prolonged periods, making them prone to cervical and lumbar spinal injuries. RECENT FINDINGS: Recently developed technologies, such as low-exposure radiographic imaging, novel radiographic im

Clinical value of robot-assisted catheter navigation for cardiac electrophysiology ablation: current evidence and future directions.

The success of catheter ablation depends not only on identification of arrhythmia mechanisms, mapping, and energy strategy, but also on catheter accessibility and stable manipulation. Robot-assisted catheter navigation enables remote control through magnetic fields, robotic sheaths, or mechanical actuation. However, with the development of three-dimensional electroanatomic mapping, low- and zero-fluoroscopy workflows, and newer ablation technologies, the key question has shifted from technical feasibility to whether these systems can overcome practical procedural constraints in specific clinic
