A 61-year-old man with critical aortic stenosis underwent aortic valve replacement. Mechanical ventilation was applied because of postoperative acute pulmonary edema accompanied by poor ventilation and poor oxygenation, and the patient was also recannulized to an extracorporeal membranous oxygenator. Bilateral pneumothorax was found 2 days later, and the right upper and left lower chest quadrants were drained with pigtail catheters. The patient did not regain consciousness 5 days after cessation of propofol. He remained in a deep coma; anisocoric pupils without light reflex and left upward eye

Entry of air into the venous system leading to intracardiac air and pulmonary air embolism (PAE) has been reported in various clinical settings such as neurosurgical interventions in the sitting position and in autopsies on patients with head and neck injuries. We report the case of a 29-year-old male who developed severe pulmonary dysfunction after severe head injury in a high-velocity car accident. Chest X-ray showed bilateral diffuse patchy infiltrates. Pneumothorax, haemothorax, pulmonary aspiration, various forms of pulmonary oedema and pulmonary contusion could be excluded. Furthermore,

We report a case of severe respiratory distress syndrome which required intermittent positive pressure ventilation and led to severe pulmonary interstitial emphysema (PIE) and massive air embolism.

We present a 59-yr-old Chinese male patient who developed acute pulmonary oedema and cardiovascular collapse following multiple episodes of venous air emboli while in the sitting position for removal of a cervical meningioma. The severity of the pulmonary oedema and cardiovascular disturbance were surprising. Postoperative ventilation and inotropic support were required and five litres of plasma were needed to replace the fluid lost as pulmonary oedema. We discuss the differential diagnosis of the pulmonary changes and review current ideas on the pathogenesis for pulmonary oedema following ven

Venous air embolism is a well-recognized complication of central venous catheterization. Although previous reports have documented venous air embolism occurring in a number of ways, including during initial catheterization, when catheters crack or are disconnected, and after catheter removal, no reports mention the possibility of air embolism occurring when a guide wire without a catheter was in place. A patient is presented who displayed signs and symptoms of venous air embolism (tachypnoea, chest pain, and arterial hypoxaemia) during central venous catheter manipulation while a guide wire al

The authors report a case of barotraumatic cerebral air embolism following scuba diving, in which air embolism was diagnosed by CT. This kind of disorder becomes more frequent with the greater popularity of scuba diving. A healthy 24-year old woman made a rapid ascent with breath hold after a scuba dive to 8 meters for 20 minutes. On surfacing, she felt chest pain radiating to the cervical region. Shortly thereafter, she developed visual obscuration and weakness in the right arm and leg. On admission, neurological examination revealed right hemiparesis with hemisensory disturbance. Visual acui

A clinical case of apparent air embolism through a radial artery line served as the impetus to conduct a study in primates to investigate the possibility of air embolism from the radial artery into the cerebral circulation. Using 133Xe cerebral scanning, we demonstrated that greater than 2 ml of air in the radial artery would result in retrograde passage of air into the vertebral system and then into the brain.

Doppler ultrasonic cardiac monitoring of patients undergoing trans-sphenoidal pituitary operations in the semisitting position has revealed the occurrence of venous air embolism in 3 of 31 consecutive cases. One such case is presented. Air may be drawn into the venous system whenever a gradient exists between the site of operation and the right heart. During trans-sphenoidal operations the most likely portals of venous air entry include the intercavernous connections within the sella, venous channels through nonpneumatized bone, inadequately sealed subnasal vessels, and vascularized metastatic

BACKGROUND: The origins and shapes of accessory digits in postaxial polydactyly of the foot were analyzed morphologically and radiographically, and their characteristics were determined. A simple classification method was then devised to assist in determining the most appropriate treatment options.
METHODS: We evaluated 113 feet of 95 patients who had surgery for the treatment of postaxial polydactyly between 1998 and 2002. Based on the morphologic, radiographic, and operative findings, the cases were classified according to the origin of the accessory digit: middle phalangeal, proximal phala

Cerebral air embolism is a rare clinical entity in day-to-day practice. The introduction of air into the venous or the arterial system can cause cerebral air embolism leading to severe neurological deficits. The common causes reported in the literature are iatrogenic; it can be caused by positive pressure maneuvers performed during cardiac resuscitation, lung biopsy, and the placement of venous catheters in the presence of a patent foramen ovale. We report a case of cerebral air embolism which has occurred secondary to lung laceration. The patient underwent intercostal drainage for hydro-pneum

Background. Nontraumatic cerebral air embolism cases are rare. We report a case of an air embolism resulting in cerebral infarction related to angioinvasive cavitary aspergillosis. To our knowledge, there have been no previous reports associating these two conditions together. Case Presentation. A 32-year-old female was admitted for treatment of acute lymphoblastic leukemia (ALL). Her hospital course was complicated by pulmonary aspergillosis. On hospital day 55, she acutely developed severe global aphasia with right hemiplegia. A CT and CT-angiogram of her head and neck were obtained demonstr
