Association between patient unconscious or not alert conditions and cardiac arrest or high-acuity outcomes within the Medical Priority Dispatch System "Falls" protocol.

INTRODUCTION: Falls are one of the most common types of complaints received by 9-1-1 emergency medical dispatch centers. They can be accidental or may be caused by underlying medical problems. Though "not alert" falls patients with severe outcomes mostly are "hot" transported to the hospital, some of these cases may be due to other acute medical events (cardiac, respiratory, circulatory, or neurological), which may not always be apparent to the emergency medical dispatcher (EMD) during call processing. OBJECTIVES: The objective of this study was to characterize the risk of cardiac arrest and

Prehospital Workplace Violence (WPV) Dispatch Alerts: Anticipating Behavior and Preventing Violence.

OBJECTIVES: Within our emergency medical services (EMS) agency, workplace violence (WPV) is captured through a documentation feature in the electronic medical record. Leveraging this data, we implemented WPV dispatch alerts for addresses where physical violence occurred. Our primary objective was to assess the association of these alerts on the rate of WPV against EMS clinicians. METHODS: This observational cohort study took place 11/20/2022-11/20/2024 at a hospital-affiliated EMS agency with 23,300 average annual ground calls for service. Alerts were implemented on 12/26/2023 and consisted o

Drones providing live video to dispatch centers during time-critical incidents.

BACKGROUND: Early triage of emergency calls at dispatch centres relies mainly on verbal information, which may not reflect the complexity and severity of a situation, limiting dispatchers situational awareness and early decision-making. Real-time live-video could potentially be provided by drones before the arrival of emergency medical services and fire department. The aim was to evaluate feasibility and operational value of automated beyond visual line of sight (BVLOS) drones delivering early live-video to emergency dispatch centres during time-critical incidents. METHODS: This prospective i

Intentional drug poisoning care in a physician-manned emergency medical service.

OBJECTIVES: Severely poisoned patients can benefit from intensive and specific treatments. Emergency medical services (EMS) may therefore play a crucial role by matching prehospital care and hospital referral to the severity of poisoned patients. Our aim was to investigate EMS accuracy in this condition. METHODS: A 3-year retrospective study was conducted in a university hospital. Emergency telephone calls about adult patients with intentional drug poisoning (IDP) were included. In daily practice, an emergency physician answers such telephone calls and dispatches either first responders or a

A classification model for the telephonic triage of low acuity abdominal complaints in South Africa.

INTRODUCTION: Emergency medical services (EMS) dispatch emergency resources according to the appropriate patient acuity, ensuring those in need of urgent care receive prompt attention. Abdominal complaints have been highlighted as an incident type that is over-triaged in EMS. The development of a telephonic classification tool could improve emergency medical dispatch efficiencies by reducing the inappropriate prioritisation of responses to these cases. The study aimed to identify variables that classified abdominal complaints as low acuity at dispatch. METHODS: A retrospective cross-sectional

Public access defibrillator locations and details in Forsyth County, North Carolina: Results of a telephone survey canvas of large businesses to facilitate out-of-hospital cardiac arrest treatment.

BACKGROUND: Limited data are available on locations of public access defibrillation programs across communities in the United States, despite their widespread presence. Our goal was to determine publicly available AED locations of large businesses in a mixed urban-rural county. We then compared our survey results to a NC state-mandated AED registry and the county's emergency medical dispatch center AED registry. METHODS: We conducted structured phone surveys of all large businesses (>19 employees) and select small businesses (healthcare, government, childcare, educational, and religious organ

Evaluating stroke recognition and prehospital quality metrics in emergency dispatch: a retrospective cohort study comparing mimics and confirmed strokes.

OBJECTIVES: Accurate identification of acute stroke by the Emergency Medical Dispatch Centre (EMDC) is essential for timely treatment and efficient resource use. However, stroke mimics-conditions resembling stroke can often lead to overtriage, complicating emergency care. We aimed to quantify EMDC stroke overtriage and compare characteristics of stroke mimics with confirmed stroke cases. DESIGN: Retrospective cohort study. SETTING: Patients with suspected stroke identified by the EMDC in Western Norway between 1 January 2021 and 31 December 2022. PARTICIPANTS: A total of 8,261 patients with

Proposition and operational characteristics of a new dispatch scale to specifically identify acute strokes.

OBJECTIVE: Prehospital recognition of an acute stroke improves the time from onset to thrombolysis and rates of reperfusion therapy. Studies conducted to evaluate paramedic and dispatcher performance in suspecting stroke are disappointing. This study addresses the specific issue of stroke recognition by dispatchers, taking into account delay in reporting the onset of symptoms (<5 h). METHODS: This is an observational analysis conducted over a 12-month period. Dispatchers used a modified Cincinnati Stroke Scale to specifically identify acute strokes in a criteria-based dispatch. Data were extr

Mobilisation of emergency services for chemical incidents in Sweden - a multi-agency focus group study.

BACKGROUND: In chemical incidents, infrequent but potentially disastrous, the World Health Organization calls for inter-organizational coordination of actors involved. Multi-organizational studies of chemical response capacities are scarce. We aimed to describe chemical incident experiences and perceptions of Swedish fire and rescue services, emergency medical services, police services, and emergency dispatch services personnel. METHODS: Eight emergency service organizations in two distinct and dissimilar regions in Sweden participated in one organization-specific focus group interview each.

Dual-process theory of racial isolation, legal cynicism, and reported crime.

Why is neighborhood racial composition linked so strongly to police-reported crime? Common explanations include over-policing and negative interactions with police, but police reports of crime are heavily dependent on resident 911 calls. Using Sampson's concept of legal cynicism and Vaisey's dual-process theory, we theorize that racial concentration and isolation consciously and nonconsciously influence neighborhood variation in 911 calls for protection and prevention. The data we analyze are consistent with this thesis. Independent of police reports of crime, we find that neighborhood racial

Reorganisation of the Basic Life Support Segment in a Physician-Led Emergency Medical Service: A Retrospective Evaluation of Operational and Clinical Characteristics.

Background: Increasing demand for emergency medical services (EMS) challenges prehospital resource availability, particularly for time-critical emergencies requiring advanced life support (ALS). Although tiered BLS and ALS response models are established in many non-physician-led EMS systems, evidence regarding qualification-based dispatch stratification within physician-led EMS systems remains limited. This study evaluated the clinical and operational characteristics of an expanded BLS dispatch segment in Berlin EMS. Methods: A retrospective observational study analysed 2,132,246 EMS missions

[Evaluation of measures to reduce the number of emergency physician missions in Tyrol during the COVID-19 pandemic].

BACKGROUND: During the peak of the COVID-19 pandemic in spring 2020, the entire emergency rescue system was confronted with major challenges. Starting on 15 March, all tourists were asked to leave the State of Tyrol, Austria. The main goal of the efforts was to ensure the usual quality of emergency medical care while reducing the physical contact during emergency interventions on site. METHODS: The Austrian Emergency Medical Service is physician-based, meaning that in addition to an ambulance team, an emergency physician (EP) is dispatched to every potential life-threatening emergency call. How many emergency dispatches occurred per cardiac arrest?

BACKGROUND: The Medical Priority Dispatch System (MPDS) is an emergency medical dispatch (EMD) system that is widely used to prioritize 9-1-1 calls and optimize resource allocation. Calls are assigned an MPDS determinant, which includes a number (1-32) representing chief complaint and priority (Alpha through Echo) representing acuity. OBJECTIVE: This study evaluates the number of emergency dispatches per cardiac arrest (NOD-CA) in cardiac arrest and non-cardiac arrest MPDS determinants. METHODS: All patients assigned a determinant by MPDS from January 1, 2008 to June 30, 2009 in a large metr

Recognition of stroke symptoms indicative of anterior circulation large-vessel occlusion via telephone and video calls: a simulation study.

BACKGROUND: Identifying suspected anterior circulation large-vessel occlusion (aLVO) strokes during emergency calls could enhance dispatch efficiency, particularly in rural areas. However, data on emergency medical dispatchers' (EMDs) ability to recognize aLVO symptoms remain limited. This simulation study aimed to evaluate the feasibility of identifying side-specific arm paresis, side-specific conjugate eye deviation (CED), and aphasia during emergency calls by instructing layperson callers to perform brief, standardized examination steps. Two communication methods were compared: (1) telephon

Design and rationale of the TANGO2 trial: A randomized controlled non-inferiority trial of bystander compression-only CPR versus standard CPR in out-of-hospital cardiac arrest.

RATIONALE: Bystander cardiopulmonary resuscitation (CPR) before emergency medical services arrival is associated with improved survival after out-of-hospital cardiac arrest (OHCA). Compression-only CPR (CO-CPR) is simpler to teach and perform, eliminates interruptions in chest compressions for rescue breaths, and has been associated with higher bystander CPR rates. For adults with OHCA of presumed medical etiology, clinical equipoise remains regarding the optimal CPR strategy for previously trained bystanders. PRIMARY HYPOTHESIS: Bystander CO-CPR is non-inferior to standard CPR (S-CPR) for 30
