Emergency Medical Services Prenotification is Associated with Reduced Odds of In-Hospital Mortality in Stroke Patients.

OBJECTIVE: Arrival by emergency medical services (EMS) and prenotification among ischemic stroke patients are well-established to improve the timeliness and quality of stroke care, yet the association of prenotification with in-hospital mortality has not been previously described. Our cross-sectional study aimed to assess the association between EMS prenotification and in-hospital mortality for patients with acute ischemic stroke or transient ischemic attack. METHODS: We analyzed data from the Massachusetts Paul Coverdell National Acute Stroke Program registry. Our study population included a

Services for emergency department patients experiencing early pregnancy complications: A survey of Ontario hospitals.

OBJECTIVES: Women experiencing complications of early pregnancy frequently seek care in the emergency department (ED), because most have not yet established care with an obstetrical provider. The primary objective of this study was to explore the services available (ED management, ultrasound access, and follow-up care) for ED patients experiencing early pregnancy loss or threatened early pregnancy loss in Ontario hospitals. METHODS: The emergency medicine chiefs of 71 Ontario hospital EDs with an annual census of more than 30,000 ED patient visits in 2017 were invited to complete a 30-item, o

Characteristics of the Highest Users of Emergency Services in Veterans Affairs Hospitals: Homeless and Non-Homeless.

Efforts are underway to understand recent increases in emergency department (ED) use and to offer case management to those patients identified as high utilizers. Homeless Veterans are thought to use EDs for non-emergent conditions. This study identifies the highest users of ED services in the Department of Veterans Affairs and provides descriptive analyses of these Veterans, the diagnoses for which they were seen in the ED, and differences based on their homeless status. Homeless Veterans were more likely than non-homeless Veterans to have >10 visits in the 2014 calendar year (12% vs. <1

Conventional versus task-based package organization for out-of-hospital emergency kits: an emergency medical services simulation study.

BACKGROUND: Emergency Medical Service crews are equipped with comprehensive emergency kits for routine care and to provide life-saving interventions in severely ill patients. While guidelines on contents and packing strategies of emergency kits for specific tasks and specialized situations exist, data for the design of out-of-hospital emergency kits in a general urban population is lacking. It may be possible to transfer the promising results of modern in-hospital packing strategies such as task-based package organization (TPO) to an Emergency Medical Service setting. METHODS: Four types of e

Community Need Index (CNI): a simple tool to predict emergency department utilization after hospital discharge from the trauma service.

BACKGROUND: Emergency department (ED) visits after hospital discharge may reflect failure of transition of care to the outpatient setting. Reduction of postdischarge ED utilization represents an opportunity for quality improvement and cost reduction. The Community Need Index (CNI) is a Zip code-based score that accounts for a community's unmet needs with respect to healthcare and is publicly accessible via the internet. The purpose of this study was to determine if patient CNI score is associated with postdischarge ED utilization among hospitalized trauma patients. METHODS: Level 1 trauma pat

Emergency medical service and in-hospital vital signs as predictors of subsequent PTSD symptom severity in pediatric injury patients.

BACKGROUND: The present study investigated the extent to which heart rate (HR) levels soon after a traumatic event predicted posttraumatic stress disorder (PTSD) symptom severity assessed 6 weeks and 6 months later in child trauma victims. METHODS: Participants consisted of 82 children (56 boys, 26 girls) aged 8-18 who were admitted to a Midwestern trauma center. HR data were recorded from emergency medical services (EMS) records, upon admission to the emergency department (ED), for the first 20 minutes following admission, and upon discharge. Subsequent PTSD and depressive symptoms were asse

Diagnostic agreement between prehospital and in-hospital diagnoses for dyspnea: a retrospective study in a French prehospital emergency medical service.

BACKGROUND: Dyspnea is a common, high-acuity presentation in emergency medicine and is associated with substantial short-term mortality. Early etiological diagnosis remains challenging in the prehospital setting, particularly in physician-staffed mobile intensive care unit (MICU) systems. METHODS: We conducted a retrospective, single-center study of adults managed by a physician-staffed MICU for dyspnea in Orléans, France, between 2023 and 2024. Agreement between the prehospital MICU working diagnosis and the first in-hospital coded diagnosis was assessed among patients with paired diagnostic

[Integration of the hospital coronary care unit with the first aid service for the domiciliary treatment of cardiovascular emergencies].

To evaluate the feasibility and the utility of an integrated service for the pre-hospital diagnosis and care of cardiovascular emergencies a new pre-hospital intensive care system has been developed. Such an emergency medical service relies on the telephone transmission of ECG and the conversation between the first aid service and the cardiologist of the hospital coronary care unit. It also implies early therapeutic intervention performed at home under the responsibility of the chief physician of the referring centre. From March '86 to December '88, 311 telephone ECG transmissions were obtaine
