Weekend Effect in Carotid Endarterectomy.

Background and Purpose- Patients undergoing surgery on the weekend may experience worse outcomes, but this weekend effect has not been studied in carotid endarterectomy (CEA). Methods- We identified patients undergoing isolated CEA in the Vascular Quality Initiative between 2003 and 2018. Our primary outcome was in-hospital stroke or perioperative death (stroke/death), stratified by symptom status. For asymptomatic patients, we also compared rates of the Centers for Medicare and Medicaid Services quality metric prolonged length of stay (>2 days or failed discharge home). We calculated propensi

[Pharmacological preconditioning in carotid endarterectomy].

The study was aimed at examining efficacy of preoperative preparation (pharmacological preconditioning) for carotid endarterectomy in patients with chronic cerebrovascular insufficiency. For this purpose, we analysed the outcomes of surgical treatment in a total of 80 patients presenting with haemodynamically significant unilateral and bilateral lesions of carotid arteries. Of these, 40 patients were operated on immediately and a further 40 patients underwent surgery after pharmacological preconditioning with Actovegin taken at a daily dose of 1,200 mg for 1.5 months. It was demonstrated that

Vernet's syndrome after carotid endarterectomy.

Unilateral paresis of cranial nerves IX to XI is defined as Vernet's syndrome. We retrospectively assessed cranial nerve symptoms from the clinical records of 143 carotid endarterectomy patients. A flexible nasolaryngoscope was used to examine vocal fold movements in 73 patients. If vocal fold paresis (VFP) was confirmed, the patient also underwent magnifying laryngoscopy (for correct diagnosis of injury to the glossopharyngeal and vagus nerves). It was found from clinical records that 8 patients (6%) were confirmed to have cranial nerve symptoms corresponding to Vernet's syndrome; 7 patients

Carotid endarterectomy in female patients.

OBJECTIVES: To evaluate early and late results of carotid endarterectomy (CEA) in female patients in a large single center experience. METHODS: Over a 12-year period ending in December 2007, 4009 consecutive primary and secondary CEAs in 3324 patients were performed at our institution. All patients were prospectively enrolled in a dedicated database containing pre-, intra-, and postoperative parameters. Patients were female in 1200 cases (1020 patients; Group 1) and male in the remaining 2809 (2304 patients, Group 2). Early results in terms of intraoperative neurological events and 30-day str

Carotid artery stenting vs. endarterectomy.

Randomized clinical trials have demonstrated that carotid endarterectomy (CEA) is superior to medical management for stroke prevention in patients with symptomatic and, to a lesser degree, asymptomatic internal carotid artery stenosis. However, large-scale registries have shown that the adverse event rates following CEA are commonly higher than observed in the trials. In the last decade, carotid artery stenting (CAS) has emerged as a less invasive alternative to surgery. In order to address the efficacy of CAS, we performed a meta-analysis of 10 randomized trials comparing CAS with CEA in 4648

Selective shunting during carotid endarterectomy.

Carotid endarterectomy (CEA) is an effective treatment for significant carotid atherosclerosis. Perioperative stroke, a devastating complication, may be partially circumvented by shunting. However, routine shunt use is not without complications and does not benefit every patient. Our study is designed to determine whether CEA under general anesthesia, without cerebral monitoring, can be safely done with shunting only in the presence of poor internal carotid artery back-bleeding or contralateral carotid occlusion or critical stenosis. The medical records of 995 carotid operations were reviewed.

Hyperperfusion syndrome after carotid endarterectomy.

The hyperperfusion syndrome is a rare delayed postoperative complication of carotid endarterectomy (CEA) characterized by headache and seizure, with or without intracranial edema or hemorrhage. Between January 1996 and December 2003, 1,602 CEAs were performed. Six patients (0.4%) developed symptoms of hyperperfusion within 2 weeks of surgery. All patients had critical stenoses, five > or =90% and one 80-90%, with poor backbleeding from the distal internal carotid artery noted at operation in all cases. Five patients were asymptomatic prior to operation; one had a hemispheric transient ischemic

Early carotid endarterectomy after stroke.

The authors sought to determine if patients with stroke and a high-grade carotid stenosis benefited from a delay before carotid endarterectomy. A retrospective study of 45 patients undergoing carotid endarterectomy after stroke is presented. The patients were divided into two groups: group I (early group, n = 20), composed of patients who had carotid endarterectomy less than 6 weeks after stroke, and group II (late group, n = 25), comprised of patients who had carotid endarterectomy more than 6 weeks after stroke. As assessed by cerebral angiography, 100% of patients in group I and 64% of pati

[Embolism detection after carotid endarterectomy].

Embolic phenomena, early after endarterectomy of the internal carotid artery, were observed in 8 of 28 cases. No patient suffered from ischemic events. 24 h later, no further embolic signals could be detected. These and other observations of an increased risk of perioperative thrombosis and cerebral infarction, in the case of persistent embolic signals during the early recovery period, confirm the probably important role of the transcranial emboli detection in the monitoring of carotid surgery.

Airway problems after carotid endarterectomy.

Respiratory obstruction has been reported as a complication of carotid endarterectomy; the causes include traumatic mucosal oedema, direct tracheal compression by haematoma and oedema secondary to lymphatic and venous congestion. We report four cases of acute respiratory obstruction complicating carotid endarterectomy. Two of these cases suffered respiratory arrest in the postoperative ward and required emergency tracheal intubation in difficult circumstances. All of these patients had developed wound haematomas and all required surgical intervention.

Carotid stenosis and carotid endarterectomy.

This review focuses on the natural history of symptomatic and asymptomatic carotid stenosis and on the risk/benefit ratio of carotid endarterectomy. Five randomized trials for symptomatic stenosis and four trials for asymptomatic carotid occlusive disease are reviewed. Carotid endarterectomy has been proved effective in reducing stroke risk in symptomatic 70-99% stenosis. Conclusive results are still pending for moderate symptomatic stenosis and the asymptomatic lesion. Surgical controversies and techniques are discussed. Algorithms for management of symptomatic and asymptomatic stenosis are o

Hemodynamic effect of carotid endarterectomy.

Cerebral blood flow was measured by the intravenous xenon-133 technique at rest and during cerebral vasodilation with acetazolamide in 32 patients before and after uncomplicated carotid endarterectomy. The results were compared with the internal carotid artery perfusion pressure measured during surgery. A significant improvement in side-to-side cerebral blood flow asymmetry occurred in 6 patients studied at baseline and in 11 patients during provoked cerebral vasodilation. These patients all belonged to a group of 14 patients who, in addition to a severe stenosis of the internal carotid artery

Carotid endarterectomy. Who needs it?

Although carotid endarterectomy is one of the most frequently performed operations in this country, recent evidence casts doubt on its advisability, particularly for patients with ocular manifestations of cerebral ischemia. The following evidence is that: the risk of future stroke in untreated patients with amaurosis fugax, retinal plaques, and infarcts is less than 3% per year, far lower than that expected for cerebral (hemispheric) transient ischemic attacks (TIAs); the perioperative risk of stroke and death after endarterectomy may be much higher than previously suspected; and aspirin is a

Intracerebral haemorrhage after carotid endarterectomy.

Among 662 consecutive carotid endarterectomies eight cases of postoperative ipsilateral intracerebral haemorrhage were identified, occurring into brain areas which, preoperatively were without infarction. As blood pressures across the stenosis were routinely measured during surgery, the internal carotid artery (ICA) perfusion pressure could be related to the occurrence of haemorrhage. In addition, cerebral blood flow (CBF) was studied with the intravenous xenon-133 technique in four patients and histopathologic examination of the brain was available in four patients who died subsequent to thei

Controlled hypothermia in carotid endarterectomy.

Successful carotid endarterectomy under controlled hypothermia, induced by shell cooling, is described. The patient, whose ipsilateral vertebral and contralateral internal carotid arteries were obstructed, and whose contralateral external carotid artery was stenosed, had presented with recurrent TIA's and amaurosis fugax. Some observations on the indication for and technique of hypothermia are made.

Postoperative occlusion after carotid endarterectomy.

Noninvasive testing (Doppler imaging and spectral analysis of carotid flow) was performed in 70 symptomatic patients who required unilateral carotid endarterectomy. Angiographic correlations were made with the noninvasive tests before surgery, and postoperatively hemodynamic changes were determined during a three-year follow-up period. Postoperative stenosis occurred in 9% (3% early and 6% late) of the patients. Clinical progression on the unoperated side (new symptoms or bruits) occurred in 15 patients. Ten of these 15 patients had significant hemodynamic progression of disease in the unopera

Recurrent stenosis after carotid endarterectomy.

Thirteen of 1,250 patients required a second operation for recurrent stenosis following carotid endarterectomy performed at the Cleveland Clinic between 1958 and 1978. Two other patients underwent reoperation because of recurrent stenosis following primary operations at other institutions. Thirteen of the 15 patients experienced neurologic symptoms caused by recurent stenosis, while two patients remained asymptomatic. Atherosclerosis was responsible for recurrent stenosis in 12 patients and appeared to be related to hypercholesterolemia. Three of the patients had myointimal fibroplasia. Eleven

Stroke associated with carotid endarterectomy.

Between 1966 and 1976, eleven strokes occurred in association with 509 carotid endarterectomies performed at Walter Reed Army Medical Center. Contralateral carotid arterial occlusion with unilateral stenosis, bilateral carotid stenoses, or multiple extracranial (with or without intracranial) stenoses were present in all patients in whom stroke developed. Preventable technical factors contributing to or directly causing stroke were identifiable in six of the eleven patients. Pulmonary endarterectomy: technique and pitfalls.

Chronic thromboembolic pulmonary hypertension (CTEPH) remains a rare and underdiagnosed disease. After one or several episodes of acute pulmonary embolism, around 3% of patients develop CTEPH and two-thirds of these patients are potential surgical candidates. Besides surgery, additional treatment modalities are pulmonary arterial hypertension medication and balloon pulmonary angioplasty. Patients should be evaluated in CTEPH expert centers to ensure the most appropriate therapy. Pulmonary endarterectomy (PEA) is a complex, but standardized surgical procedure aiming to clear the obstructed pulm
