Surgical treatment and long-term follow-up in chronic pancreatitis.

In the past two decades our knowledge of the pathophysiology and surgical treatment options in chronic pancreatitis have improved substantially. Surgical treatment in chronic pancreatitis has evolved from radical to organ-preserving procedures. The classic Whipple resection is no longer indicated in chronic pancreatitis, and operations like the duodenum-preserving pancreatic head resection and the pylorus-preserving Whipple have replaced it as surgical standards. These procedures allow the preservation of exocrine and endocrine pancreatic function, provide pain relief in up to 90% of patients,

Surgical therapy of chronic pancreatitis: indications, techniques and results.

In chronic pancreatitis (CP) a benign inflammatory process in the pancreas results in progressive structural changes with replacement of functional exocrine and endocrine parenchyma by a fibrotic and inflammatory tissue, often evident as an inflammatory mass. The consequences are diabetes mellitus, exocrine insufficiency, and severe recurrent upper abdominal pain, often resulting in a significant reduction in the quality of life. The inflammatory process or the formation of pseudocysts can cause local complications such as obstruction of the pancreatic duct, bile duct or the duodenum. In spite

[Current indications for thyroidectomy].

Thyroid surgery, one of the most common interventions in endocrine surgery, is practiced by many specialists who perform this procedure exclusively. It accounts for the bulk of work even in reference centers that treat rare endocrine tumors (e.g. adrenal and gastrointestinal tract cancer). Better results are obtained by experienced and skilled operators. Surgeons who correctly perform thyroid surgery can achieve excellent outcomes even in other areas of endocrine surgery. So it is surprising that not more is being done to teach the procedure, which has always been considered something of an ar

[Rare pancreatic endocrine tumors].

Functional pancreatic endocrine tumors other than gastrinoma and insulinoma are quite rare. Some of these tumors may be part of multiple endocrine neoplasia type one (MEN-1) syndrome or phakomatoses. Depending on their cell type, functional pancreatic endocrine tumors may cause distinct clinical endocrine syndromes, such as the 'glucagonoma syndrome', Verner-Morrison syndrome and the 'somatostatinoma syndrome'. The significant progress made in recent years, especially in the field of imaging procedures, has brought about great improvement in the identification and differentiation of these neop

Synchronous thyroid pathology in patients presenting with primary hyperparathyroidism.

BACKGROUND: Thyroid and parathyroid pathology may coexist; classically, this is described in multiple endocrine neoplasia. Other nonsyndromic scenarios result in pathologies of these endocrine organs coexisting. This can create a challenge in the clinical decision making and management of these patients. It is optimal to deal with both problems in one operative procedure, if surgery is required. METHODS: A retrospective review of cases of synchronous thyroid and parathyroid pathology that were managed surgically over a 3-year period by a single endocrine surgeon at a single university medical

Scarless Surgery: Clinical Indications for Transoral Endocrine Surgery and Implications for Pathologists.

Transoral endocrine surgery (TES) is a scarless approach to thyroidectomy and parathyroidectomy for well-selected patients. Criteria for the TES approach to thyroidectomy include thyroid diameter less than or equal to 10 cm, benign nodule less than or equal to 6 cm, or confirmed or suspected malignant nodule less than or equal to 2 cm. Although fragmentation of surgical specimens has been reported in TES, additional studies are needed to evaluate the implications of TES on pathologic examination.

The impact of laparoscopy in the management of non-palpable testes.

Undescended testis is one of the most common congenital abnormalities of the genitourinary system. In 10-20% of these boys, a testis is located completely inside the abdominal cavity. Multiple operative techniques have been described and are associated with a variable success rate. In this review we evaluate the use of laparoscopy in the management of the non-palpable testis to determine what advantages it might offer over the open technique. On the basis of current evidence it seems that laparoscopy is the only diagnostic and therapeutic procedure that is accurate enough to elucidate the diag

European Society of Endocrine Surgeons (ESES) consensus statement on standardization and research requirements for intraoperative nerve monitoring in endocrine surgery.

BACKGROUND: Intraoperative nerve monitoring (IONM) is widely used in endocrine surgery, yet substantial heterogeneity exists in its documentation, interpretation, and reporting. This limits comparability across studies and hampers the evaluation of clinical outcomes. METHODS: A consensus process was conducted by the European Society of Endocrine Surgeons (ESES), combining a systematic literature review, analysis of Eurocrine® registry data, a survey among ESES members, and a structured Delphi consensus process. RESULTS: Considerable variability was identified in monitoring techniques, signal

Autofluorescence-guided hemithyroidectomy in a low-volume thyroid institution with no experience in parathyroid surgery: randomized clinical trial.

BACKGROUND: Recent studies suggest that low-volume surgeons with no experience in parathyroid surgery are at increased risk of causing parathyroid gland damage during thyroid surgery. The aim of this RCT was to evaluate the impact of using autofluorescence in hemithyroidectomy on parathyroid gland identification and preservation in a low-volume institution with no experience in parathyroid surgery. METHODS: Patients referred for hemithyroidectomy were randomized 1 : 1 to either autofluorescence-guided hemithyroidectomy (the near-infrared autofluorescence group) or conventional hemithyroidecto

[Indication and performance of endocrine surgery. The significance of molecular genetic examination].

The molecular genetic changes from certain endocrine tumors are already understood, reflecting as they do the etiology of these sporadic familial disorders. This already has clinical consequences to the treatment of familial endocrine tumors, which often appear in the course of syndromatic disorders. These consequences consist in slight changes to surgical technique, the search for other active and usually endocrinal tumors, and examination of family members for other gene carriers (of disease-specific mutations) and the most suitable prophylactic tumor therapy. In contrast, for sporadic endoc

Lateral pancreatojejunostomy in the surgical management of chronic pancreatitis. Current concepts and future perspectives.

Chronic pancreatitis is a medical disease, but surgery will be required for a selected subgroup of patients. Main indications for surgery in chronic pancreatitis are severe truly incapacitating pain refractory to medical therapy, local complications of pancreatitis or suspicion of malignancy. Surgical options include resective and drainage procedures. The choice of operation depends on the structural anatomy of the gland and the adjacent organs. Lateral pancreatojejunostomy is the preferred drainage procedure, because it is safe and effective in relieving the pain, while it preserves whatever

[Pituitary and adrenal gland surgery].

Laparoscopic adrenalectomy and transsphenoidal pituitary surgery are currently established as the surgical procedures of choice with a low complication rate. Beyond potential surgical complications, one has to consider endocrine sequelae such as adrenal insufficiency and hypopituitarism. Without adequate endocrine treatment patients are prone to develop potentially lethal complications such as Addisonian crises or pituitary coma. Therefore, all patients should be seen by an endocrinologist before and after surgery. Patients with bilateral adrenalectomy require lifelong substitution of glucocor

Surgical approach in patients with hyperparathyroidism in multiple endocrine neoplasia type 1: total versus partial parathyroidectomy.

Usually, primary hyperparathyroidism is the first endocrinopathy to be diagnosed in patients with multiple endocrine neoplasia type 1, and is also the most common one. The timing of the surgery and strategy in multiple endocrine neoplasia type 1/hyperparathyroidism are still under debate. The aims of surgery are to: 1) correct hypercalcemia, thus preventing persistent or recurrent hyperparathyroidism; 2) avoid persistent hypoparathyroidism; and 3) facilitate the surgical treatment of possible recurrences. Currently, two types of surgical approach are indicated: 1) subtotal parathyroidectomy wi

Volume, outcomes, and quality standards in thyroid surgery: an evidence-based analysis-European Society of Endocrine Surgeons (ESES) positional statement.

INTRODUCTION: Continuous efforts in surgical speciality aim to improve outcome. Therefore, correlation of volume and outcome, developing subspecialization, and identification of reliable parameters to identify and measure quality in surgery gain increasing attention in the surgical community as well as in public health care systems, and by health care providers. The need to investigate these correlations in the area of endocrine surgery was identified by ESES, and thyroid surgery was chosen for this analysis of the prevalent literature with regard to outcome and volume. MATERIALS AND METHODS:
