Experience with the management of primary endodermal sinus tumor of the mediastinum.

Ten male patients with extensive primary endodermal sinus tumor of the mediastinum were treated with chemotherapy (with or without surgical excision and radiation therapy) between 1977 and 1985. Three patients, treated with cyclophosphamide-vincristine-based chemotherapy, died 1.5, 2.5, and 6.0 months, respectively, after initial diagnosis. Of the seven patients treated with cisplatin-based chemotherapy, one patient died of septicemia at 2.5 months after diagnosis and was disease-free at autopsy examination. Three other patients died of progressive disease at 7.0, 13.0, and 14.0 months, respec

Endodermal sinus (yolk sac) tumors of the anterior mediastinum.

Seven cases of endodermal sinus tumor (yolk sac tumor) primary in the anterior mediastinum are added to the literature bringing to 21 the number of reported cases. This germ cell tumor should be included in the differential diagnosis of anterior mediastinal tumors. It has occurred in males 13--49 years old who are often systemically ill. Since this tumor almost always produces elevated levels of serum alpha-fetoprotein, a strong presumptive diagnosis is possible prior to biopsy. The prognosis is poor. The two disease-free survivors of the 21 reported cases were both treated with a combination

Posterior mediastinal endodermal sinus (yolk sac) tumor in a female patient.

Primary endodermal sinus tumor (yolk sac tumor) of the mediastinum is uncommon. Most patients are young and male, and the great majority of tumors are found in the anterior mediastinum. We report a case of primary posterior mediastinal endodermal sinus tumor occurring in a female patient. Surgical excision was performed and three courses of combination chemotherapy were subsequently given. The serum alpha-fetoprotein level returned to normal.

Extragonadal endodermal sinus (yolk sac) tumor. Angiographic findings and literature review.

The angiographic and pathologic features of an endodermal sinus tumor (EST) arising from the retroperitoneum in a 19-month-old boy are presented. Although rare instances of extragonadal origin of EST have been reported, this is the first detailed description of the angiographic abnormalities. The angiographic appearance was that of a primarily retroperitoneal tumor with parasitized feeding arteries from the liver and right kidney, early filling venous radicles, sinusoidal pooling of contrast material, and inferior vena caval obstruction.

Disseminated peritoneal tuberculosis mimicking advanced-stage endodermal sinus tumor: a case report.

It is well known that peritoneal tuberculosis may mimic advanced-stage epithelial ovarian carcinoma because of similar clinical, radiologic, and laboratory findings. However, disseminated peritoneal tuberculosis mimicking advanced-stage endodermal sinus tumor (ESS) has not been reported previously. An 18-year-old nulliparous woman came with the complaint of pelvic pain and weight loss. Imaging studies demonstrated that she had multiple peritoneal implants and left adnexial mass. Also, laboratory studies showed elevated CA125 and alpha fetoprotein levels suggesting an initial diagnosis of ESS.

Endodermal sinus tumor of the vulva: successfully treated with high-dose chemotherapy.

Endodermal sinus tumors (EST) of the lower female genital tract are uncommon malignancies. Most of these tumors are found in the vagina or in the cervix. Only seven cases of EST involving the vulva are reported. We report an eighth case of vulvar EST. The patient recurred and was successfully treated with high-dose chemotherapy and peripheral blood cell transplantation. Because of their location, an early diagnosis of vulvar EST should be possible. Resection of the lesion by using a modified radical vulvectomy followed by cisplatin-based chemotherapy seems the best treatment for these tumors t

Successful multidisciplinary treatment of an endodermal sinus tumor of the pineal region.

The case presented is a 17-year-old male patient with a pineal region endodermal sinus tumor manifested by obstructive hydrocephalus and upward gaze limitation. The highly aggressive nature of this rare malignancy was evidenced by spontaneous tumor bleeding. The patient was treated with a cerebrospinal fluid (CSF) shunt and microsurgical tumor excision followed by irradiation and combination chemotherapy consisting of cisplatin, vincristine, methotrexate and bleomycin. The response to therapy was monitored by serial measurements of serum and CSF alpha-fetoprotein level. The patient remains fre

Successful treatment of metastatic extragonadal endodermal sinus (yolk sac) tumor in childhood.

Two children with metastatic extragonadal endodermal sinus tumor who had failed VAC chemotherapy were treated with combination chemotherapy consisting of cis-platinum, vinblastine, doxorubicin HCl (Adriamycin) and bleomycin (cis-VAB). There was a complete response in both patients with one child remaining tumor-free 3 1/2 years after metastasis and 2 1/2 years after stopping therapy. The main toxic reactions encountered were associated with the use of cis-platinum, but these were manageable and resolved upon completion of therapy. Cis-VAB appears to be a safe and effective regimen for the trea

Challenges in diagnosis of primary penile tumor of the endodermal sinus in infants.

INTRODUCTION: Germ cell tumors (GCTs) are an uncommon group of malignant neoplasms in the pediatric population. The endodermal sinus tumor, also known as yolk sac tumor, is the most frequent subtype of GCT and typically originates in the gonads. Extragonadal presentation is rare, particularly in the penis. OBJECTIVE: To describe the case of a male infant with an extragonadal penile endodermal sinus tumor, emphasizing the diagnostic challenges and the role of alpha-fetoprotein (αFP) as a monitoring biomarker. CLINICAL CASE: A previously healthy 18-month-old male infant presented with a one-mo

Successful treatment of mediastinal germ cell/endodermal sinus tumors.

Three patients with endodermal sinus tumor of the mediastinum and two with endodermal sinus tumor mixed with other histologies were treated with CEBA (cisplatin, etoposide, bleomycin, and doxorubicin [Adriamycin]) and by cytoreductive surgery. Three patients treated with CEBA followed by surgery achieved a complete remission and remain free of disease at 36+, 35+, and 28+ months, respectively, from diagnosis. Two patients were treated initially with vinblastine, bleomycin, cisplatin, and surgery, and neither achieved a complete remission. Both then received CEBA, and one achieved a complete re

Extragonadal endodermal sinus tumors in children: a review of 24 cases.

Endodermal sinus tumors (EST) are highly malignant lesions with a tendency to recur locally and with a high incidence of metastatic disease at the time of diagnosis. The clinical and radiographic spectra of 24 patients with extragonadal EST are reported. There were 17 females and seven males and the mean age of presentation was 21 months. The primary lesion occurred in the sacrococcygeal region in 16 patients; in the vagina in two patients; in the anterior mediastinum in two patients; and in the bladder, liver, nasopharynx and posterior cranial fossa in one patient each. Computed tomography (C

Metrorrhagia in a child with an endodermal sinus tumor of the vagina: a case report.

In a female child presenting with vaginal bleeding or a mass protruding through the vagina, it is mandatory to perform an endoscopic and laparoscopic examination and biopsy to establish the correct diagnosis and precise extent of the lesion. An endodermal sinus tumor (EST) of the vagina is a rare, malignant germ cell tumor that occurs primarily in infants. Clearly, the vagina is a critical site for treatment, and the surgeon should seek to preserve the reproductive and sexual function, if possible. We report our experience of a case of EST of the vagina in a 9-month-old female with a history o

Effusion cytology of endodermal sinus tumor of the colon. Report of a case.

A three-year-old boy presented with a peritoneal effusion due to an occult endodermal sinus tumor in the ascending colon. Cytologic examination of the ascitic fluid revealed clusters of round plump cells that had large hyperchromatic nuclei, finely stippled chromatin, multiple prominent nucleoli and vacuolated or mucin-containing cytoplasm, features suggesting an adenocarcinoma. The colonic primary was discovered during emergency laparotomy performed due to suspected acute hemoperitoneum. Endodermal sinus tumor should be included in the differential diagnosis when cytologic features reminiscen

Ataxia-telangiectasia and endodermal sinus tumor of the ovary: report of a case.

Ataxia-telangiectasia, an inherited disorder characterized by progressive cerebellar ataxia and telangiectasias, is often associated with primary immunodeficiency and high incidence of malignancies, mostly of the lymphoreticular type. Endodermal sinus tumor is a rare germ cell tumor of the ovary characterized by an extremely rapid growth and poor prognosis. Both these diseases are associated with an abnormal production of alpha-fetoprotein. Primary tumors of the ovary in patients with ataxia-telangiectasia are extremely rare and the association of an endodermal sinus tumor and ataxia-telangiec

Value of tumor markers in the treatment of endodermal sinus tumors and choriocarcinomas in the pineal region.

A case of embryonal carcinoma in the pineal region of a 17-year-old boy is presented. The tumor included elements of choriocarcinoma and endodermal sinus tumor, and the use of human chorionic gonadotropin and alpha-fetoprotein as tumor markers is discussed. The markers were demonstrated both within the tumor and in the cerebrospinal fluid (CSF) and blood. The patient was treated with a postoperative program of irradiation and cancer chemotherapy, and at follow-up examination 20 months after operation no signs of residual tumor were present. It is suggested that human chorionic gonadotropin and

Mayer-Rokitansky-Kuster-Hauser syndrome associated with endodermal sinus tumor of the ovary.

Both EST and Mayer-Rokitansky-Kuster-Hauser syndrome are uncommon disorders. This report illustrates a case of these two rare conditions occurring in the same patient. Patients with Mayer-Rokitansky-Kuster-Hauser syndrome, although not appearing to be at increased risk for germ cell tumors, unlike testicular feminization patients, are not immune to ovarian cancer. Therefore, individuals with Mayer-Rokitansky-Kuster-Hauser syndrome should still be followed gynecologically because they are at least at the normal risk of developing ovarian as well as other gynecological neoplasms.
