Neurophysiologic assessment of endolymphatic hydrops.

An electrophysiological method for assessing endolymphatic hydrops of the cochlea in an animal model of Meniere's disease is described. Eighth nerve gross action potentials (AP) were evoked by tone bursts presented at discrete phases of a simultaneously delivered 50-Hz pure tone. Hydropic cochleas showed significantly less modulation of AP amplitude and latency than normal cochleas. This method may be useful in objectively evaluating Meniere's disease with electrocochleography.

Effect of diuretics on endolymphatic hydrops.

The effect of diuretics on endolymphatic hydrops, which was produced by obliteration of the endolymphatic duct and sac in guinea pigs, was determined by statistical analysis of volumetric changes in the scala media. When acetazolamide was administered orally at 159 to 434 mg/kg per day for a period of 30 days, endolymphatic hydrops did not reach the extent of distension shown in animal ears without drug treatment. No difference was seen in animals with one month of drug treatment followed by three additional months without treatment. This result suggests that acetazolamide has the effect of re

Experimental endolymphatic hydrops in the rat.

In the rat, the epithelial lining of the endolymphatic duct and sac, and the contents of the sac, showed remarkable differences in comparison with these structures in the guinea pig and man. Obliteration of the endolymphatic duct resulted in endolymphatic hydrops of varying severity in 55% of the rats, after survival times varying from one to five months. The presence of a positive periodic acid-Schiff-stain for coagulum accumulating in the endolymphatic duct after prolonged saccus obliteration suggests active involvement of the endolymphatic duct in the migration of endolymph toward the endol

Effect of fistulae on endolymphatic hydrops.

An experiment was performed on 31 guinea pigs to study the effect of fistulae of the horizontal canal, superior canal, common crus, and utricle on the course of experimentally induced endolymphatic hydrops. The effect of fistulae on corresponding parts of normal ears was also studied in an additional 17 animals. The results indicated a remarkable consistency of healing of the fistulae in both groups of animals. Fistulae had no significant effect on the course of endolymphatic hydrops, irrespective of whether the fistulae were made immediately before or several months after obliteration of the

The vestibular aqueduct and endolymphatic sac and duct in endolymphatic hydrops.

The histologic features of the endolymphatic sac and duct in 23 serially sectioned temporal bones with idiopathic or secondary endolymphatic hydrops were blindly compared with 22 randomly selected, normal temporal bones. In idiopathic hydrops, the pars rugosa of the endolymphatic sac extended out of the vestibular aqueduct into the dura in 29% of bones, compared with none of normal bones (P less than .01). In the other 71%, the pars rugosa in the vestibular aqueduct was surrounded by dura more commonly than normal. Functional studies are required to assess the relationship of these findings to

Endolymphatic hydrops: can it be controlled?

While endolymphatic shunt procedures are conceptually logical, there are characteristics of underlying pathological conditions and host response that severely limit the probabilities of achieving functional shunts.

Endolymphatic hydrops. An overview and classification.

Endolymphatic hydrops (EH) is a pathological condition which is the final common manifestation of a variety of otologic insults. In this paper we develop a classification which, on the basis of clinical and pathological data, distinguishes symptomatic and asymptomatic forms. Clinical case histories and temporal bone studies are presented to illustrate and substantiate this classification. The symptomatic form becomes evident by the hallmark symptoms of episodic vertigo and fluctuating hearing loss. The asymptomatic form is silent. Interconversion from one form to another may occur over time. E

Delayed endolymphatic hydrops: a case study.

Delayed endolymphatic hydrops (DEH) is an unusual variation of Ménière's disease characterized by episodic vertigo that develops some time after the onset of a profound, typically unilateral sensorineural hearing loss. This case study describes a 48-year-old male who presented with complaints of episodic vertigo and disequilibrium 15 years following the onset of unilateral sensorineural hearing loss. The patient's history, audiologic findings, and vestibular evaluation led to the diagnosis of DEH. The case highlights the diagnostic and treatment challenges associated with this condition and fo

Autoimmune endolymphatic hydrops: five-year review.

This article reviews the authors' experience with autoimmune endolymphatic hydrops over the past 5 years. Twenty-seven of 52 patients with diagnosed autoimmune inner ear disease (52%) manifested endolymphatic hydrops, usually bilateral. Treatment consisted of low salt diet, diuretic, vestibular suppressant, and usually prednisone. Cytotoxic drugs and lymphocytoplasmapheresis were reserved for refractory cases. Audiometric followup (average, 23 months) was available in 12 patients. Using American Academy of Otolaryngology--Head and Neck Surgery guidelines, hearing improved or stabilized in at l

The development of endolymphatic hydrops following CMV inoculation of the endolymphatic sac.

The effect of inoculation of cytomegalovirus (CMV) into the endolymphatic sac was examined in CMV-seronegative and seropositive animals. Seronegative animals developed hearing loss, infection in the epithelial cells of the endolymphatic sac and perisaccular connective tissue, and endolymphatic hydrops. Control animals inoculated with inactivated CMV showed no hearing loss, viral infection, or endolymphatic hydrops. Seropositive animals showed complete protection from hearing loss and viral infection, yet a monocytic infiltrate was seen surrounding the endolymphatic sac. Associated with this lo

[Menière's disease, endolymphatic hydrops and MRI].

Menière's disease is characterized by episodic vertigo associated with fluctuating hearing loss, tinnitus and fullness of the ear on the affected side. Endolymphatic space enlargement - or endolymphatic hydrops (EH) - is a histological hallmark of the disease that does not explain all its clinical manifestations. Magnetic resonance imaging improvements now allow in vivo visualization of inner ear liquid spaces and appreciation of potential EH. This article discusses these advances and their potential diagnostic implications in the context of Menière's disease and more generally in cochleovesti

Intratympanic injection in delayed endolymphatic hydrops.

CONCLUSIONS: The present study showed that intratympanic dexamethasone injection (ITD) is a promising approach for the treatment of contralateral and ipsilateral delayed endolymphatic hydrops (DEH). Moreover, intratympanic gentamicin injection (ITG), as a chemical labyrinthectomy, is a simple alternative for controlling vertigo in patients with ipsilateral DEH. OBJECTIVE: This study examined the effect of ITD or ITG on DEH. METHODS: Fourteen patients with DEH completed the clinical and audio-vestibular evaluation. Among them, 10 cases (ipsilateral type: nine cases, contralateral type: one ca

MR volumetric assessment of endolymphatic hydrops.

OBJECTIVES: We aimed to volumetrically quantify endolymph and perilymph spaces of the inner ear in order to establish a methodological basis for further investigations into the pathophysiology and therapeutic monitoring of Menière's disease. METHODS: Sixteen patients (eight females, aged 38-71 years) with definite unilateral Menière's disease were included in this study. Magnetic resonance (MR) cisternography with a T2-SPACE sequence was combined with a Real reconstruction inversion recovery (Real-IR) sequence for delineation of inner ear fluid spaces. Endolymphatic hydrops: pathophysiology and experimental models.

It is well established that endolymphatic hydrops plays a role in Ménière disease, even though the precise role is not fully understood and the presence of hydrops in the ear does not always result in symptoms of the disease. It nevertheless follows that a scientific understanding of how hydrops arises, how it affects the function of the ear, and how it can be manipulated or reversed could contribute to the development of effective treatments for the disease. Measurements in animal models in which endolymphatic hydrops has been induced have given numerous insights into the relationships betwee

Unusual cases of delayed endolymphatic hydrops.

The objective of this report is to emphasize the clinical features of delayed endolymphatic hydrops in comparison with Ménière's disease. Four cases were selected from a series of 16 examined between the years 1993 and 2005, due to their unusual characteristics. A short clinical history and significant tests are presented for each of the four cases. The cases of delayed endolymphatic hydrops discussed in this article suggest that hydrops is the most important underlying pathology that causes the hearing loss and the vestibular symptoms both in the better ear and in the ear with severe hearing

Laboratory experience with experimental endolymphatic hydrops.

Experimental endolymphatic hydrops refers to the creation of hydrops in the experimental animal and represents a histologic correlate for Meniere's disease. Details of the surgical means to create this condition in guinea pigs are provided. Alterations in endolymph fluid composition and relative endolymph pressure occur with hydrops. Deterioration in auditory and vestibular function secondary to hydrops can develop.

Treatment of ipsilateral delayed endolymphatic hydrops.

Nine patients with ipsilateral delayed endolymphatic hydrops were treated. All cases were initially treated by conservative therapy using isosorbide or other drugs and 4 patients have been doing well without surgical intervention, though they occasionally have slight dizziness. Five patients required surgery to control vertigo. Three patients first underwent cochleosacculotomy to maintain the vestibular function in the diseased ear, but vertigo recurred after 6 months in all 3 cases. These 3 patients and another patient who wanted definitive treatment right from the start underwent transmastoi

Antidiuretic hormone (ADH) and endolymphatic hydrops.

Plasma antidiuretic hormone (p-ADH) concentrations were determined with a radioimmunoassay, using a reversed-phase C18 silica column, in 300 patients with vertigo, dizziness and/or deafness; 119 of them had a diagnosis of Menière's disease. The p-ADH level was significantly elevated in patients with Meniere's disease and others with endolymphatic hydrops, e.g. cochlear Menière's disease or delayed hydrops. By contrast, the p-ADH level was not so high in cases without the endolymphatic hydrops. The increase in the p-ADH level was closely linked to vertigo attacks, the glycerol test results and
