Retrospective Cohort Studies in Craniofacial Outcomes Research: An Epidemiologist's Approach to Mitigating Bias.

Retrospective cohort studies, defined as a follow-up study in which outcome occurred prior to study onset, are common in craniofacial outcomes research and will continue to be prevalent given the increasing availability of secondary datasets and inherent prospective study limitations. However, if available data are not adequately measured, or necessary variables are absent, retrospective cohort studies can be particularly prone to bias. This brief communication aims to highlight the primary sources of bias, including measurement error, selection bias, and confounding. Each source is clearly de

The role of epidemiologists in addressing the public health consequences of the United States criminal legal system.

Exposure to the US criminal legal system-whether through contact with law enforcement, incarceration in a jail or prison, or community supervision-is associated with a range of adverse health outcomes. There is mounting evidence that mass incarceration drives health inequities, particularly for Black, Indigenous, and People of Color. However, relative to its outsized impacts on health and health inequities, the criminal legal system has received limited attention in epidemiology. In this commentary, we use a public health prevention framework to highlight opportunities for epidemiologic resear

An Increasing Scabies Incidence in Croatia: A Call for Coordinated Action Among Dermatologists, Physicians and Epidemiologists.

INTRODUCTION: The aim of our study was to examine the scabies incidence in the Croatian population and to analyse potential related factors. METHODS: This mixed ecological study is based on a retrospective medical record review. National data from communicable disease reports was sourced and analysed for an 11-year period (2007-2017), with more focus on the period 2014-2017. Descriptive statistics were used to calculate trends. Differences between the groups were studied using Chi-square test and Kendall's tau (τ) correlation coefficient. Levels of significance were set at p<0.05 or p<0.01.

Clinical manifestations of dengue hemorrhagic fever in Puerto Rico, 1990-1991. Puerto Rico Association of Epidemiologists.

The aim of the study reported here was to demonstrate that dengue hemorrhagic fever occurs in Puerto Rico, that it is underreported, and that this underreporting is due partly to underdiagnosis in hospitals. Surveillance for severe dengue identified 986 hospitalizations for suspected dengue in 1990-1991. At the time, on the basis of available clinical and laboratory data, the surveillance system routinely identified 20 DHF cases, including three with dengue shock syndrome (DSS). Our subsequent review of these 986 patients' hospital records identified 102 whose records supported a clinical diag

Cardiovascular risks related to increased diastolic, systolic and pulse pressure. An epidemiologist's point of view.

Since the introduction of the sphygmomanometer at the beginning to the 20th century, the significance of diastolic (DBP), Systolic (DBP) and pulse pressure (PP) as hypertensive cardiovascular risk factors has been controversial. These historical controversies are reviewed. Initially, DBP was thought to be the best measure of risk, but more recently both SBP and DBP, which ever is higher, are used in classifying hypertensive cardiovascular risk. There are problems with the present guidelines, in that SBP and DBP represent only two inflection points on the propagated pulse wave that is measured

"A wild and wondrous ride": CDC field epidemiologists in the east Pakistan smallpox and cholera epidemics of 1958.

In mid-April of 1958 the Government of Pakistan summoned the press to announce a grave need for international aid to cope with smallpox and cholera epidemics in East Pakistan. In response, and with the backing of the US State Department, Dr. Alexander D. Langmuir, chief epidemiologist of the CDC, led a team of epidemiologists to assist authorities in Dacca strengthen their immunization programs. Langmuir's superiors hoped for a Cold War advantage, but he saw an opportunity for trainees in the Epidemic Intelligence Service to learn about public health in a developing country. Langmuir later des

Surveillance for an emerging disease: dengue hemorrhagic fever in Puerto Rico, 1988-1997. Puerto Rico Association of Epidemiologists.

Surveillance for emerging diseases is critically dependent on four factors: reporting methods, case definition, laboratory diagnosis, and knowledge of the disease among health-care professionals. The dengue hemorrhagic fever (DHF) surveillance system in Puerto Rico collects patient data from three sources: dengue case investigation (DCI) forms sent with diagnostic samples, clinical reports from hospital infection control nurses (ICNs), and hospital records. Recruitment of ICN reporting produced a marked increase in notifications (67 to 294). Hospital records of possible DHF cases showed that t

Community Assets as Protective Factors Buffering Health Effects of Environmental Hazards: Framework and Considerations for Environmental Epidemiologists.

Disparities in the adverse health effects due to ambient environmental exposures have long been documented in the environmental epidemiology literature. A growing body of environmental epidemiology literature has focused on how detrimental aspects of the physical environment (e.g., poor housing quality) and social environment (e.g., chronic social stressors) can exacerbate the adverse health effects of environmental exposures (e.g., air pollution, heat). However, the literature on protective factors which might mitigate adverse health effects of environmental exposure is more limited. We borro

Tikhon Efimovich Boldyrev (1900-1984): A Soviet epidemiologist's contributions to public health in 1950s China.

Tikhon Efimovich Boldyrev was a prominent Soviet epidemiologist. During the Sino-Soviet collaboration of the 1950s, he played a key advisory role in shaping health policies in the newly established People's Republic of China. This article explores Boldyrev's life and his journey to China, where he served as Group Leader of the Soviet Experts and Chief Expert at the Chinese Ministry of Health from 1954 to 1956. During his tenure, Boldyrev authored twenty reports and proposals that influenced China's public health policy. His notable contributions included introducing and adapting the Soviet hea

An evaluation of modified case definitions for the detection of dengue hemorrhagic fever. Puerto Rico Association of Epidemiologists.

The case definition for dengue hemorrhagic fever (DHF) requires fever, platelets < or = 100,000/mm3, any hemorrhagic manifestation, and plasma leakage evidenced by hemoconcentration > or = 20%, pleural or abdominal effusions, hypoproteinemia or hypoalbuminemia. We evaluated the specificity and yield of modified DHF case definitions and the recently proposed World Health Organization criteria for a provisional diagnosis of DHF, using a data base of laboratory-positive and laboratory-negative reports of hospitalizations for suspected dengue in Puerto Rico, 1994 to 1996. By design, all modificati

Dengue severity throughout seasonal changes in incidence in Puerto Rico, 1989-1992. The Puerto Rico Association of Epidemiologists.

To determine whether the proportion of severe dengue cases increased with the yearly seasonal increase in dengue incidence, we examined reports of disease symptoms in case surveillance data and laboratory testing results in Puerto Rico from January 1989 to July 1992. A computer algorithm was designed to identify severe cases, i.e., those that fulfilled three or all four of the World Health Organization criteria for dengue hemorrhagic fever (DHF). A monthly severity index (SI) was defined as the ratio of severe cases to laboratory-positive and indeterminate (all non-negative) cases for each mon

A comparison of quasi-experimental methods with data before and after an intervention: an introduction for epidemiologists and a simulation study.

BACKGROUND: As the interest in and use of quasi-experimental methods to evaluate impacts of health policies have dramatically increased in the epidemiological literature, we set out this study to (i) systematically compare several quasi-experimental methods that use data before and after an intervention and contrast their performance within a simulation framework while providing a brief overview of the methods; and (ii) discuss challenges that could arise from using these methods as well as directions for future research in the context of epidemiological applications. METHODS: We considered s

Assessing the Impact of the 2020 Council of State and Territorial Epidemiologists Case Definition for Pertussis on Reported Pertussis Cases.

BACKGROUND: In 2020, the Council of State and Territorial Epidemiologists (CSTE) pertussis case definition was modified; the main change was classifying polymerase chain reaction (PCR)-positive cases as confirmed, regardless of cough duration. Pertussis data reported through Enhanced Pertussis Surveillance (EPS) in 7 sites and the National Notifiable Diseases Surveillance System (NNDSS) were used to evaluate the impact of the new case definition. METHODS: We compared the number of EPS cases with cough onset in 2020 to the number that would have been reported based on the prior (2014) CSTE cas
