We have treated 11 patients with cultured epidermal autograft (CEA) at the Indiana University Medical Center between April 1989 and January 1991. Seven of these patients had acute burns. We have followed up on five of them for at least 5 months; the remaining two are still hospitalized. Two patients had CEA applied to cover giant hairy nevi, and two were treated for hypertrophic burn scars, one of which was treated too recently to report follow-up. The disastrous effects of loss of epidermal cell adhesion are epitomized by the life-threatening blistering skin diseases pemphigus foliaceus and pemphigus vulgaris. In four instances, metastases to epidermis from primary cutaneous malignant melanomas at different sites showed histological features similar to those of cutaneous malignant melanoma primary in the epidermis. In these metastases, atypical melanocytes were present within the epidermis and in the upper part of the dermis much as in primary cutaneous malignant melanoma. In previous work we described a novel culture technique using a cholera toxin and PMA-free medium (Mel-mix) for obtaining pure melanocyte cultures from human adult epidermis. In Mel-mix medium the cultured melanocytes are bipolar, unpigmented and highly proliferative. Large numbers of CD30-positive tumor cells are not typically observed in mycosis fungoides (MF), but CD30 expression may occur on large cells of MF that have transformed into high-grade large cell lymphoma.
