Endothelial cell counts after epikeratophakia surgery.

We used specular microscopy to study the corneal endothelium of eight aphakic patients who had undergone epikeratophakia. The average post-operative period was 16 months with a range of 12 to 18 months. Corneal thickness increased from 0.52 mm +/- 0.01 mm to 0.88 mm +/- 0.04 mm with the addition of the onlay graft. The average pre-operative endothelial cell count was low (1,450 per mm2 +/- 218) and did not differ significantly (p greater than .9) from the average postoperative cell count (1,438 per mm2 +/- 218). Despite the increase in corneal thickness and low endothelial cell counts, we saw

[A preliminary report of epikeratophakia in the treatment of keratoconus].

7 eyes of keratoconus were treated with epikeratophakia in which the authors used self-made corneal lenses by a rapid method. The corneal curvature was markedly flattened to reduce myopia and irregular astigmatism was eliminated, resulting in much improved visual acuity in 6 eyes.

[Epikeratophakia surgery for refractive errors and keratoconus].

Epikeratophakia surgery is a relatively new surgical technique which involves suturing of prelathed corneal tissue on top of the recipient's cornea. The epikeratophakia lens is lathed according to the recipient's refractive needs and therefore can correct myopia and hyperopia; a plano lenticle can be used to correct keratoconus. In the past 3 years we operated on 25 patients (27 eyes). There were 11 cases of myopia, 7 of aphakia and 7 of keratoconus. The aphakic (hyperopic) group included patients with aphakia and traumatic aphakia in whom secondary implantation of an intraocular lens was cont

Epikeratophakia following rotational autokeratoplasty in a child.

A 3-year-old child with posttraumatic aphakia and a corneal scar was treated with a rotational penetrating ipsilateral autokeratoplasty followed by epikeratophakia 2 months later. His final best corrected visual acuity was 20/30, with 1.37D of corneal astigmatism. There were no significant operative or postoperative complications.

The nationwide study of epikeratophakia for aphakia in children.

In the nationwide study of epikeratophakia, 97 surgeons performed a total of 335 procedures in 314 eyes for the correction of aphakia in children under the age of 8 years 1 month. Fifteen children underwent bilateral surgery. Thirty-six tissue lenses were removed and 21 of these eyes underwent a second epikeratophakia procedure. Overall, the success rate for procedures was 89%, and with repeated surgery it was 95% for eyes. Seventy-three percent of the patients were within 3 diopters of emmetropia after surgery. Visual acuity results in patients able to provide verbal responses to the illitera

Histopathology of a case of epikeratophakia (aphakic epikeratoplasty).

An epikeratoplasty button was removed by penetrating keratoplasty 31 months following an aphakic epikeratoplasty. The epithelium over the donor cap was thickened with numerous surface abnormalities. Bends and fractures in the donor Bowman's membrane were found throughout the length of the specimen. A significantly decreased number of keratocytes were found in the anterior half of the donor lenticule; the posterior half of the lenticule was acellular. The interfibrillar collagen distance of the donor lenticule was significantly increased compared with that of the recipient host cornea. The cent

Surgical and visual results of pediatric epikeratophakia.

We performed epikeratophakia for the correction of aphakia in 31 children, ages 2 months to 7 years, who had unilateral congenital or traumatic cataracts. In 27 patients with more than 4 months of follow-up, 23 of 30 (77%) grafts were successful. In the 15 patients with 6 months or more followup, the average increase in corneal curvature was 12.39 diopters. The average overrefraction needed to achieve emmetropia was + 2.14 diopters spherical equivalent. Two patients operated on within the first year of life have achieved 6/15 (20/50) visual acuities, as measured by visual evoked potentials. On

Long-term follow-up of pediatric epikeratophakia.

BACKGROUND: Epikeratophakia is a potentially reversible corneal surgical procedure that can correct refractive errors in children who are aphakic and poor candidates for intraocular lens implantation. The correction of aphakia in the pediatric population poses specific problems because of associated amblyopia. METHODS: The clinical records of 61 consecutive patients (82 eyes) treated for pediatric aphakia by epikeratophakia were reviewed retrospectively with a follow-up of 3 to 5 years. For the purpose of analysis, the patients were divided into seven groups. RESULTS: The overall success rat

Physiologic analysis of corneal healing after epikeratophakia.

The evaluation of epithelial permeability, stromal transparency, and endothelial cell density is essential to determine the resumption of normal function in each corneal physiologic unit after refractive surgical procedures. The authors report the results of a prospective study conducted in 55 consecutive patients undergoing epikeratophakia using prelathed, lyophilized tissue lenses. Epithelial permeability was evaluated by means of fluorophotometry preoperatively and 1, 2, 4, 8, 12, 24, and 52 weeks after epikeratophakia. With few exceptions, Scheimpflug photography also was performed at the

The nationwide study of epikeratophakia for aphakia in adults.

In the nationwide study of epikeratophakia, 154 ophthalmic surgeons who had attended a training course performed 519 procedures for the correction of aphakia in adults: 310 of the eyes had 30 or more days of follow-up after suture removal. Of 229 eyes, 172 (75%) were within 3 diopters of emmetropia after surgery. Of 259 eyes, 245 (95%) demonstrated improved uncorrected visual acuity; 138 (53%) improved by four or more Snellen lines. Of 265 eyes, 209 (78%) achieved within two lines or improved their best corrected visual acuity. Of the 119 patients who achieved or improved their preoperative be

Epikeratophakia for control of pediatric bullous keratopathy.

A child who had had a congenital cataract phacoemulsified at age four months became contact lens intolerant, and at age two years had an implantation of a Worst metal-looped iris-clip intraocular lens. He developed painful pseudophakic bullous keratopathy three years later. Since the child remained with dense deprivation amblyopia and contact lens intolerance, when the IOL was removed an epikeratophakia graft was applied. The bullous keratopathy resolved and the patient has remained asymptomatic for 22 postoperative months. The proposed mechanism for relief of the bullous keratopathy is to inc

Keratometric and refractive results of pediatric epikeratophakia.

Keratometric and refractive results of pediatric epikeratophakia showed that patients under 1 year of age had steeper corneas preoperatively and required more correction, as estimated by the Sanders-Retzlaff-Kraff regression formula. The average spherical equivalent of refractive error six months postoperatively was +6.92 +/- 4.67 diopters in patients under 1 year, and -0.72 +/- 4.22 D in patients over 1 year. Three of 14 younger patients and 35 of 54 older patients were within 3 D of emmetropia. Since March 1982, significant undercorrection has occurred only in patients 6 months old or younge

Histopathological findings in failed human epikeratophakia lenticules.

Six human epikeratophakia lenticules were removed 1 to 23 months after refractive keratoplasty for aphakia (n = 2) and myopia (n = 4). The myopic group included one epi-lenticule treated with ArF-excimer laser for hyperopia. Reasons for removal were epithelial defect (n = 4), edema of host cornea (n = 1), and overcorrection (n = 1). The epi-lenticule stroma showed centrally a regular fibrillar structure with a low keratocyte density. Electron microscopy (n = 1) revealed preserved cytoplasmic organelles and nuclear appearance. Mononuclear cells were absent. Polysaccharide was demonstrated in th

Simplified formulas for lathing aphakic epikeratophakia lenses.

Geometrically based formulas for lathing aphakic epikeratophakia lenses have been simplified to enable easy calculation of the radii of cut for the optical and the wing zones. The formulas compensate for postoperative thinning of the lens on host cornea and allow for any desired amount of pre-lathing dehydration. Clinical results to date show that over 80% of lenses are made within 3 diopters of the expected power.

[Removal of epikeratophakia lenticules and implantation of intraocular lenses].

Two epikeratophakia lenticules were removed from a highly myopic eye after 13 years and from an aphakic eye after 15 years, respectively. After subsequent refraction compensation, the following results were achieved. Fixation of the epikeratophakia lenticules in the corneal tissue led to a change of the K-value following the removal of the corneal transplants. The refraction compensation can be performed by an intraocular or sclera-fixated lens implantation following the complete closure of the keratotomy wounds. Neither of the two host corneas had undergone an optically relevant change so tha
