Effective Epilepsy Surgery for Post-Traumatic West Syndrome Following Abusive Head Trauma.

West syndrome, an infantile developmental and epileptic encephalopathy with a deleterious impact on long-term development, requires early treatment to minimize developmental abnormality; in such cases, epilepsy surgery should be considered a powerful therapeutic option. We describe a 10-month-old female admitted with West syndrome associated with a hemispheric lesion following abusive head trauma. Her seizures were suppressed by hemispherotomy at 12 months of age, leading to developmental improvement. Surgical treatment of West syndrome following traumatic brain injury has not been reported pr

Post-Traumatic Epilepsy in Zebrafish Is Drug-Resistant and Impairs Cognitive Function.

Post-traumatic epilepsy (PTE) is acquired epilepsy after traumatic brain injury (TBI). Despite the availability of more than 20 antiseizure medications (ASMs), there is no way at present to prevent epileptogenesis in TBI survivors, and many cases of PTE become drug-resistant. Importantly, the adverse effects of ASMs can significantly affect patients' quality of life. Mammalian models are commonly used for studying refractory PTE, but are expensive and laborious. Zebrafish models have become popular for studying epilepsy, but most focus on larvae, and there have been no reports to date of pharm

Treatment options for post-traumatic epilepsy: an update on clinical and translational aspects.

INTRODUCTION: Post-traumatic epilepsy (PTE) accounts for 10% to 20% of all symptomatic epilepsies and 5% of all forms of epilepsy, and drug resistance is reported in up to 45% of cases. AREAS COVERED: This is a focused narrative review that discusses the available data on the current and new PTE treatments, giving particular attention to the last 10 years. EXPERT OPINION: Despite the disappointing results of many antiseizure medications (ASMs) in preventing epileptogenicity, it is still unclear whether the early intervention could lead to different clinical phenotypes in terms, for example,

The path to post-traumatic epilepsy: A review of emerging biomarkers and therapeutic targets.

BACKGROUND: Post-traumatic epilepsy (PTE) is a common, disabling sequela of traumatic brain injury (TBI), contributing to long-term morbidity. Short-term antiseizure prophylaxis reduces early provoked seizures, but no interventions prevent late unprovoked seizures or PTE. This gap arises from limited understanding of underlying mechanisms. OBJECTIVE: This review synthesizes PTE pathophysiology and emerging mechanism-based prevention strategies, emphasizing a shift from reactive management to personalized prophylaxis. EVIDENCE SYNTHESIS: PTE develops via epileptogenesis-a latent, multifactori

The prevalence of chronic traumatic encephalopathy in a historical epilepsy post-mortem collection.

Previous post-mortem epilepsy series showed phosphorylated tau (pTau) accumulation in relation to traumatic brain injury (TBI) rather than driven by seizure frequency. The Corsellis Epilepsy Collection, established in the mid-20th century, represents brain samples collected from patients living with a range of epilepsies from the 1880s to 1990s. Our aim was to interrogate this historical archive to explore relationships between epilepsy, trauma and tau pathology. AT8 immunohistochemistry for pTau was carried out in 102 cases (55% male, with mean age at death of 62 years) on frontal, temporal,

Mapping the evidence-practice gap in pediatric post-traumatic seizures and post-traumatic epilepsy: A national practice-pattern survey of pediatric neurologists.

OBJECTIVE: Pediatric post-traumatic seizures (PTS) and post-traumatic epilepsy (PTE) remain largely managed through extrapolation from adult evidence, with limited pediatric-specific guidance. This study aimed to characterize current clinical practice among pediatric neurologists in Türkiye, quantify evidence-practice divergence, assess whether clinician characteristics explain this variation, and identify priority areas requiring consensus. METHODS: A national cross-sectional web-based survey including 15 items was conducted among actively practicing pediatric neurologists in Türkiye. Post-t

Incidence of post-traumatic epilepsy following paediatric traumatic brain injury: protocol for systematic review and meta-analysis.

INTRODUCTION: Post-traumatic epilepsy (PTE) is a recognised complication of traumatic brain injury (TBI), and is associated with higher rates of mortality and morbidity when compared with patients with TBI who do not develop PTE. The majority of the literature on PTE has focused on adult populations, and consequently there is a paucity of information regarding paediatric cohorts. Additionally, there is considerable heterogeneity surrounding the reported incidence of PTE following childhood TBI in the current literature. The primary aim of our study is to summarise reported PTE incidences in pa

Molecular mechanism analyses of post-traumatic epilepsy and hereditary epilepsy based on 10× single-cell transcriptome sequencing technology.

BACKGROUND: Single-cell RNA sequencing analysis has been usually conducted on post-traumatic epilepsy (PET) and hereditary epilepsy (HE) patients; however, the transcriptome of patients with traumatic temporal lobe epilepsy has rarely been studied. MATERIALS AND METHODS: Hippocampus tissues isolated from one patient with PTE and one patient with HE were used in the present study. Single cell isolates were prepared and captured using a 10× Genomics Chromium Single-Cell 3' kit (V3) according to the manufacturer's instructions. The libraries were sequenced on an Illumina NovaSeq 6000 sequencing

Multi-glial inhibitory plasticity in epilepsy and sleep: implications for post-traumatic epileptogenesis.

Traumatic brain injury (TBI) is a leading cause of acquired epilepsy, and the latent period before the first spontaneous seizure offers a window for anti-epileptogenic intervention. Glial cells are active participants in the pathophysiology of epilepsy and play a role beyond the classical supportive roles and in the control of inhibitory networks. Cumulatively, astrocytes, microglia and oligodendrocytes contribute to the GABAergic signal transduction via orchestrated pathways comprising neurotransmitter uptake, cytokine-mediated inflammation and myelin remodeling. GABA transporters (GAT-1, GAT

[Early post-traumatic epilepsy in childhood: significance and short term prognosis (author's transl)].

In the last five years, 244 children with head trauma were observed. An early post-traumatic epilepsy was present in 11% of the patients. This complication was more frequent in children less than six years of age or presenting a previous neurological history. Early posttraumatic epilepsy was rarely associated with brain concussion and frequently with cerebral contusion. An intracranial haematoma was not encountered in the absence of the classical signs of expanding space occupying lesion.

Post-traumatic epilepsy: An historical review from ancient observations to modern molecular mechanisms.

Post-traumatic epilepsy (PTE) represents a significant neurological complication of traumatic brain injury (TBI), accounting for approximately 5% of all epilepsies and 10-20% of structural/symptomatic epilepsies in population studies. This review traces the evolution of PTE research from ancient medical observations through 140+ years of modern scientific investigation, with emphasis on the historical, epidemiological and clinical-trial literature that established the field's current conceptual and diagnostic framework. Molecular and cellular mechanisms are introduced only briefly as a bridge

EEG biomarkers for the prediction of post-traumatic epilepsy - a systematic review of an emerging field.

Traumatic brain injury (TBI) is often followed by post-traumatic epilepsy (PTE), a condition often difficult to treat and leading to a substantial decline in quality of life as well as increased long-term mortality. The latent period between TBI and the emergence of spontaneous recurrent seizures provides an opportunity for pharmacological intervention to prevent epileptogenesis. Biomarkers capable of predicting PTE development are urgently needed to facilitate clinical trials of putative anti-epileptogenic drugs. EEG is a widely available and flexible diagnostic modality that plays a fundamen

The Role of Decreased Levels of Neuronal Autophagy in Increased Susceptibility to Post-traumatic Epilepsy.

Post-traumatic epilepsy (PTE) caused by mild TBI (mild traumatic brain injury, mTBI) has a high incidence and poor prognosis, but its mechanisms are unclear. Herein, we investigated the role of reduced levels of neuronal autophagy during the latency period in the increased susceptibility to PTE. In the study, a gentle whole-body mechanical trauma rat model was prepared using Noble-Collip drums, and the extent of injury was observed by cranial CT and HE staining of hippocampal tissue. The incidence of epilepsy and its seizure form were observed 7-90 days after mTBI, and electroencephalography (
