Epstein-Barr virus infections of the central nervous system.

OBJECTIVE: Epstein-Barr virus (EBV), a lymphotropic herpes virus causing infectious mononucleosis (IM), also causes various central nervous system (CNS) infections. In the present study, EBV CNS infections were investigated. PATIENTS AND METHODS: For adult inpatients in our hospital and related hospitals between 1984-2002, CNS syndromes with IM symptoms were examined, and serologic positives were assessed according to established criteria. Polymerase chain reaction (PCR) was performed for cerebrospinal fluid (CSF) from seven patients. RESULTS: Ten patients with EBV-related CNS infections wer

[Cluster headache, Epstein-Barr and Burkitt's lymphoma virus infections].

Thirteen patients affected by Burkitt's lymphoma were studied. Six out of them had cluster headache associated with Burkitt's lymphoma, and in this group we observed the absence of HLA-B14 antigen. The presence of HLA-B14 antigen in the remaining seven patients, confirm the hypothesis of a protective role of this antigen against cluster headache.

Latent Epstein-Barr virus infection in cottontop tamarins. A possible model for Epstein-Barr virus infection in humans.

The association of Epstein-Barr virus (EBV) with a growing number of human malignancies underlines the importance of efforts aimed at preventing the infection with this potential carcinogen and of establishing animal models for human virus-associated tumors. Cottontop tamarins have been used in EBV vaccine studies because virus infection regularly induces lymphomas similar to those seen in human immunocompromised individuals. In recent years, several vaccines based on the gp340/220 envelope protein of EBV have been developed and shown to prevent the development of EBV-associated lymphomas in t

[Epstein-Barr virus infections in adults: a diagnostic challenge].

The adult form of mononucleosis caused by Ebstein-Barr virus (EBV) is different from the disease in children and adolescents. In most adults there is no pharyngitis or lymphadenopathy, fever is much more prolonged, abnormal liver function is frequent and lymphocytosis and the presence of atypical lymphocytes are not common. Such an atypical disease presentation often results in delayed diagnosis and unnecessary treatments. We describe 2 adults with such atypical presentations and complications of EBV infection.

[Epstein-Barr virus infections. New pathogenic and clinical aspects].

The Epstein-Barr virus (EBV) is among the most widespread of human viruses. It causes several different diseases, such as acute infectious mononucleosis (IM), chronic active EBV-infection (cEBV), the x-linked lymphoproliferative syndrome (XLP), polyclonal and oligoclonal lymphomae in connection with immunologic disorders, as well as African Burkitt's lymphoma and nasopharyngeal carcinoma. Pathogenesis, clinical features and diagnosis are discussed. In this connection, special tests on the latency and multiplication of the Epstein-Barr virus are presented. --The Epstein-Barr virus persists in t

Primary Epstein-Barr-virus infections in acute neurologic diseases.

Infectious mononucleosis has been associated with Guillain--Barré syndrome, Bell's palsy, meningoencephalitis and transverse myelitis. Since it is not known that many children with infectious mononucleosis do not develop heterophil antibodies, we looked for evidence of current or recent Epstein-Barr virus infection in young patients with these neurologic diseases by using serodiagnostic procedures for detection and titration of antibodies to various antigens related to Epstein-Barr virus. Seven of 24 cases with Guillain-Barre syndrome and three of 16 with facial palsy were definitely associate

Epstein-Barr virus load in cerebrospinal fluid of patients with chronic active Epstein-Barr virus infection.

Chronic active Epstein-Barr virus (EBV) infection is a rare chronic mononucleosis syndrome involving clonally proliferating EBV-infected T-/NK-cells. EBV DNA was quantified in nonpleocytotic cerebrospinal fluid (CSF) of 9 patients. Three patients with neurologic and/or neuroimaging abnormalities showed high CSF copy numbers. In 1 patient, CSF copy number exceeded the peripheral blood value. CSF EBV-load may predict the central nervous system involvement of EBVT-/NK-cells.

Quantification of Epstein-Barr virus DNA is helpful for evaluation of chronic active Epstein-Barr virus infection.

Chronic active Epstein-Barr virus infection (CAEBV) presents with chronic or recurrent infectious mononucleosis-like symptoms, such as low-grade fever, liver dysfunction, lymphadenopathy, and hepatosplenomegaly. Immunological methods are useful for the diagnosis of viral infections. However, CAEBV patients do not necessarily have high titers of Epstein-Barr virus (EBV)-specific antibodies. Hosts that are immunocompromised after hematopoietic stem cell transplantations sometimes suffer from systemic EBV-associated hemophagocytic lymphohistiocytosis (EBV-HLH) and EBV-positive lymphoma. Patients

Elevated IgA antibodies to Epstein-Barr virus in children with chronic active Epstein-Barr virus infection.

Anti-Epstein-Barr virus (EBV) antibodies were tested in 11 children with chronic active EBV infection. Anti-virus capsid antigen (VCA)-IgG antibody titers ranged from 1:640 to 1:10,240. Anti-VCA-IgM antibody was consistently positive in 5 of the 11 patients; anti-VCA-IgA antibody was consistently positive in 6 of the 10 patients; anti-early antigen (EA)-IgG antibody was consistently positive in 10 of the 11 patients and anti-EA-IgA antibody was consistently positive in 4 out of the 7 patients. Anti-EBV nuclear antigen (EBNA) antibody was not detected in two patients. Consistently positive anti

[Epstein-Barr virus infection--a lympho- and epitheliotropic infection].

Epstein-Barr virus (EBV) has long been thought to be primarily a B-lymphotropic virus. This tropism becomes obvious in the association of the virus with diseases that become manifest in lymphoproliferative conditions, such as acute infectious mononucleosis or endemic Burkitt's lymphoma. In the course of mononucleosis, however, viraemia cannot be detected and B-lymphocytes infected with EBV in vitro produce only small amounts of the virus. In contrast, recent studies document that EBV replicates in the epithelial cells in the mouth, and pronounced secretion of virus can also be detected. Cells

NIH conference. Epstein-Barr virus infections: biology, pathogenesis, and management.

Epstein-Barr virus (EBV) encodes genes that ensure its persistence in human B lymphocytes. Some of the genes encourage B-cell proliferation; others are poised to evade or defeat immune recognition. Immune restraints on the virus, however, are typically so effective that most infections are never symptomatic. In contrast, acute infectious mononucleosis, a self-limited lymphoproliferative illness, is common in adolescents and young adults. Unbridled proliferative illnesses arise when cellular immunity is grossly defective. Treatment of EBV-associated syndromes is largely supportive. Antiviral dr

[The spectrum of infections caused by the Epstein-Barr virus].

The observation by Epstein and Barr (EB) of virus particles in Burkitt lymphoma cells in 1964 led 4 years later to the discovery that this virus was the long sought agent responsible for infectious mononucleosis. Pathophysiologically the EB virus specifically infects B lymphocytes of the immune system and induces perpetual multiplication of these cells if infection remains uncontrolled. The T lymphocytes, once sensitized against these virus infected cells, either kill them or suppress their continuous growth. During acute infectious mononucleosis, these stimulated T cells appear in the bloodst

The biology and serology of Epstein-Barr virus (EBV) infections.

The Epstein-Barr virus (EBV), carried by the majority of the world's adult human population, is closely associated with three known human diseases; infectious mononucleosis IM), Burkitts lymphoma (BL) and nasopharyngeal carcinoma (NPC). It is now generally accepted that EBV plays an active role in the etiology of BL and NPC rather than simply being a "passenger". Considerable evidence also argues for the participation of "co-factors" in these two diseases. Environmental co-factors appear needed to explain the peculiar geographical distribution of BL. Among various possibilities that must be co

Epithelial cell infection by Epstein-Barr virus.

Epstein-Barr Virus (EBV) is etiologically associated with multiple human malignancies including Burkitt lymphoma and Hodgkin disease as well as nasopharyngeal and gastric carcinoma. Entry of EBV into target cells is essential for virus to cause disease and is mediated by multiple viral envelope glycoproteins and cell surface associated receptors. The target cells of EBV include B cells and epithelial cells. The nature and mechanism of EBV entry into these cell types are different, requiring different glycoprotein complexes to bind to specific receptors on the target cells. Compared to the B ce
