In contrast to the obstructive sleep apnoea syndrome (OSA) the obesity-hypoventilation syndrome (OHS) is characterized by persistent hypercapnia during the day. After positive pressure ventilation (PPV) patients with OHS the daytime blood gases normalize after a short time. The aim of this study was to investigate whether blood gases at the end of a standardized exercise test separate both OSA and OHS after 3 months of PPV. Fourteen patients with OHS (12 males, 53.2 +/- 9.5 years, BMI: 41.7 +/- 9.6 kg/m2, PCO2: 50.7 +/- 4.5 mmHg) and 28 patients with severe OSA (27 males, 54.5 +/- 8.3 years, B

In order to investigate the influence of the level of the spinal cord injury (SCI) on anaerobic or short-term power production and propulsion technique, 23 male SCI subjects performed a 30-second sprint test on a stationary wheelchair ergometer. Kinematic parameters were studied both inter- and intra-individually. Subjects with a cervical lesion showed a lower mean power output (21.5 Watt, one-sided) than the other subjects; whereas, no differences were found between subjects with a thoracic or lumbar injury (46.9, 63.7, and 49.1 Watt, one-sided). Unexpectedly, no differences were found for th

Assessment of dyspnoea severity during incremental cardiopulmonary exercise testing (CPET) has long been hampered by the lack of reference ranges as a function of work rate (WR) and ventilation (V' E). This is particularly relevant to cycling, a testing modality which overtaxes the leg muscles leading to a heightened sensation of leg discomfort.Reference ranges based on dyspnoea percentiles (0-10 Borg scale) at standardised work rates and V' E were established in 275 apparently healthy subjects aged 20-85 years (131 men). They were compared with values recorded in a randomly selected "validati

BACKGROUND: Cholinergic urticaria (CholU) is a common condition in which active or passive warming leads to development of pinpoint wheals, flares and itch.
OBJECTIVE: To develop a standardized protocol for diagnosing CholU and measuring trigger thresholds, independent of patients' fitness levels.
METHOD: Ten CholU patients and matched healthy controls underwent pulse-controlled incremental ergometry for 30 min (stationary bicycle) increasing their pulse rate by 15 beats every 5 min. Non-invasive, heat-flux double-sensor and conventional electronic thermometers measured core and skin surface

OBJECTIVE: To determine whether a submaximal one-arm cranking test could be used to predict an individual's upper body aerobic capacity. This issue has potential importance for the fitness assessment of individuals with neurological disease or damage who have hemiplegia.
METHODS: Nine healthy male volunteers (33+/-2.4 years) and nine female volunteers (27+/-1.9 years) performed a two-arm maximal, two-arm submaximal test and a one-arm submaximal arm crank ergometry test. Heart rate (HR) was monitored via a three-lead electrocardiogram (ECG) and expired air was analysed every 30 seconds through

One hundred patients presenting with exercise intolerance or rhabdomyolysis episodes have been examined successively by 31P Nuclear Magnetic Resonance Spectroscopy (MRS) of leg plantar flexor muscles with exercise test. In all cases a muscle biopsy was performed. At the end of investigations, diagnosis of a metabolic myopathy was made in 33 patients: glycogenolysis or glycolysis deficiency in 8 cases, mitochondrial myopathy in 24 cases and CPT II deficiency in one case. Muscular dystrophy or congenital myopathy were diagnosed in 6 cases. No precise etiology could be found in 30 patients with e

OBJECTIVES: To derive sex-specific peak oxygen uptake (VO2peak) reference equations for cycle ergometer cardiopulmonary exercise testing (CPET) that integrate estimations of lean body mass (eLBM) and body fat percentage (eBF) based on sex, age, height and weight.
METHODS: We pooled data from apparently healthy adults who performed cycle ergometry in Belgium (FLEMENGHO and iCOMPEER cohorts) and the USA (FRIEND registry). Per subgroup of sex, age and body mass index (BMI), 70% of the data was used for training the equations and 30% for testing them.
RESULTS: The cohort consisted of 1778 female

Activity-related dyspnea in chronic lung disease is centrally related to dynamic (dyn) inspiratory constraints to tidal volume expansion. Lack of reference values for exertional inspiratory reserve (IR) has limited the yield of cardiopulmonary exercise testing in exposing the underpinnings of this disabling symptom. One hundred fifty apparently healthy subjects (82 males) aged 40-85 underwent incremental cycle ergometry. Based on exercise inspiratory capacity (ICdyn), we generated centile-based reference values for the following metrics of IR as a function of absolute ventilation: IRdyn1 ([1-(

In-bed cycle ergometry is a safe, feasible, and reliable method of providing in-bed physical rehabilitation for patients with critical illness in the intensive care unit. It is a complex intervention that requires specific clinical decision-making skills to identify suitable patients, close coordination with the team to deliver the intervention, and ongoing evaluation to guide treatment progression. Protocols for the delivery of in-bed cycle ergometry are presented according to the Rehabilitation Treatment Specification System. Specifically, the components of in-bed leg cycle ergometry are des

BACKGROUND: Individuals with spinal cord injury (SCI) are at an increased risk of pressure ulcer development. Electrical stimulation of adjacent muscles may be underutilized as a tool for pressure ulcer healing in individuals with SCI.
METHOD: Single case study.
SUMMARY: A 27-year-old man with C4 tetraplegia developed a left ischial pressure ulcer that for 23 months responded slowly and inconsistently to conventional treatment. Electrically induced lower extremity ergometry (EILEE) was introduced to facilitate wound healing. The pressure ulcer healed completely in 6.5 months.

Measuring lactate during moderate exercise is a useful tool in the diagnosis of mitochondrial disorders. It was the aim of this study, to develop reference limits for lactate at rest, during exercise and after the exercise. We investigated 18 healthy subjects and 6 patients with a mitochondrial disorder. In controls, serum lactate levels were comparable to already reported findings. In 4 patients lactate levels were markedly increased during the exercise. Measurement of serum lactate is a simple and useful step in the diagnosis of mitochondrial disorders.

A 53-year-old male mountain biker received a total artificial heart and remained in the cardiovascular intensive care unit for 56 days. To reduce functional decline caused by inactivity, he performed a six-session cycle ergometer exercise program in his hospital room. Traditional cardiac responses for exercise prescription were not applicable; therefore, a symptom-limited, monitored progression scheme in conjunction with recommendations for the artificial heart was implemented to modify duration and intensity. Over the six sessions, the patient improved his distance pedaled by 320% and functio

In patients suffering from primary pulmonary hypertension (PPH), a raised pulmonary vascular resistance may limit the ability to increase pulmonary blood flow as work rate increases. We hypothesised that oxygen uptake (VO2) may not rise appropriately with increasing work rate during incremental cardiopulmonary exercise tests. Nine PPH patients and nine normal subjects performed symptom-limited maximal continuous incremental cycle ergometry exercise. Mean peak VO2 [1.00 (SD 0.22) compared to 2.58 (SD 0.64) l x min(-1)] and mean VO2 at lactic acidosis threshold [LAT, 0.73 (SD 0.17) compared to 1

This study examined the effectiveness of a short-duration but high-intensity exercise countermeasure in combination with a novel oral volume load in preventing bed rest deconditioning and orthostatic intolerance. Bed rest reduces work capacity and orthostatic tolerance due in part to cardiac atrophy and decreased stroke volume. Twenty seven healthy subjects completed 5 wk of -6 degree head down bed rest. Eighteen were randomized to daily rowing ergometry and biweekly strength training while nine remained sedentary. Measurements included cardiac mass, invasive pressure-volume relations, maximal
