Gonadotropin-releasing hormone agonists and estrogen-progestogen replacement therapy.

Gonadotropin-releasing hormone agonists are effective in the treatment of endometriosis and myomas, both of which are estrogen-dependent processes, but there is a high clinical recurrence rate after therapy is discontinued. Long-term continuous therapy (2 years or more) has a cumulative effect on bone loss and causes other uncomfortable or harmful side effects. Noninvasive assessments of disease response in patients with myomas have shown that bone changes might be prevented and other side effects of long-term therapy can be alleviated by adding back small amounts of estrogen or progestin. Estrogen replacement therapy and physical function in rheumatoid arthritis.

OBJECTIVES: Postmenopausal women with RA experience worsening functional decline, independent of disease activity. Estrogen replacement therapy (ERT) has been proposed to mitigate musculoskeletal deterioration, but its role in RA remains unclear. This study investigated the association between ERT use and physical function in postmenopausal women with RA. METHODS: This longitudinal study analysed data from 'Forward, the National Databank for Rheumatic Diseases' (2000-22). Postmenopausal women with RA who initiated ERT were matched 1:1 to non-users based on menopause year or ERT initiation. Th

Estrogen-progestogen replacement therapy and ovarian cancer: an update.

Hormone replacement therapy in menopause has been associated with a moderate increase in ovarian cancer risk. Data on combined estrogen-progestin therapy, based on one randomized trial, two cohort and four case-control studies, do not provide definite evidence of an association between combined hormone replacement therapy and ovarian cancer. These data do not suggest, however, substantial differences between the effect of estrogen only or unspecified hormone replacement therapy and combined hormone replacement therapy on ovarian cancer risk.

Timing of estrogen replacement therapy for optimal osteoporosis prevention.

To determine whether estrogen initiated at age 60 yr or later reduces rates of bone loss and fracture incidence, we performed a prospective cohort study of 6910 nonosteoporotic women, 65 yr of age or older. Estrogen use, medical history, lifestyle, and anthropometric data were obtained by questionnaire, interview and examination. We identified five patterns of estrogen use: never users (67%); past early users (started under age 60 yr with no current use; 23%); past late users (started at age 60 or later with no current use; 2%); current early users (started under age 60 yr with use both at bas

Estrogen replacement therapy and risk of epithelial ovarian cancer.

Estrogen replacement therapy (ERT), in common use among menopausal women, has been inconsistently related to risk of ovarian cancer. Part of the inconsistency may be due to differences in risk according to tumor histology. We therefore compared, within histologic groups, 367 cases of invasive epithelial ovarian cancer to 564 population controls on history of ERT practices. In multivariate logistic regression analyses adjusted for age, parity, oral-contraceptive usage, and other factors, the relative odds of nonmucinous ovarian cancer increased by 5.1% for each year of ERT use [i.e., adjusted o

Estrogen replacement therapy and protection from acute myocardial infarction.

As part of a longitudinal study of a southern California retirement community begun in June 1981, 8841 women 44 to 101 years old completed a health survey questionnaire. As of January 1, 1987, 1019 deaths had occurred among these women, who had contributed 40,919 years of follow-up. Women who had used estrogen replacement therapy had a relative risk of death due to all causes of 0.80 compared with women who had never used estrogens (p = 0.0005). Much of this reduced mortality rate was due to a marked reduction in the death rate of acute myocardial infarction among users of estrogen (55 deaths)

Effects of long-term estrogen replacement therapy. II. Neoplasia.

Two groups of hypoestrogenic women are analyzed by retrospective comparison. Patients were observed by a single group of physicians for at least five years -- 301 patients treated with replacement estrogen and 309 untreated patients. Of each group, 207 women had uteri in situ. Incidence figures for neoplasia (gynecologic, breast, and all sites) were compared between the two groups and with the Third National Cancer Survey, yielding a risk ratio for the development of adenocarcinoma of the endometrium among estrogen-treated women of 3.8 and 9.3, respectively. There was no increase among any oth

Estrogen and estrogen-androgen replacement in postmenopausal women dissatisfied with estrogen-only therapy. Sexual behavior and neuroendocrine responses.

OBJECTIVE: To investigate the efficacy of esterified estrogens alone and combined with oral androgen on sexual function and menopausal symptoms in postmenopausal women. STUDY DESIGN: Twenty postmenopausal women dissatisfied with their estrogen or estrogen-progestin therapy volunteered to enter a double-blind, randomized trial in which they received either oral esterified estrogens or esterified estrogens + androgen for eight weeks after a single-blind, placebo, lead-in period. Sexual function was assessed with a questionnaire used in the Yale midlife survey, and plasma levels of estradiol, es

Drospirenone in combination with estrogens: for contraception and hormone replacement therapy.

Progesterone has a high affinity for the mineralocorticoid receptor and is an antagonist of aldosterone. Almost all synthetic progestogens are devoid of an antimineralocorticoid effect, and are unable to antagonize the salt-retaining effect of estrogens. This property could be one cause of the weight gain and increase in blood pressure associated with the use of combined oral contraceptives and, in some susceptible women, with postmenopausal hormone replacement therapy (HRT). This review illustrates the results of clinical studies with drospirenone, a new progestogen with antialdosterone activ

The risks and benefits of estrogen replacement therapy: Leisure World.

A women spends about one-third of her life in her postmenopausal years. Some women supplement this period of decreased estrogen production with estrogen replacement therapy (ERT). Many epidemiologic studies have examined the long-term effect of postmenopausal estrogen deprivation and of ERT. Since the 1970s, we have evaluated the risks and benefits of ERT in one population of older women in the California retirement community of Leisure World. ERT is the most effective method for preventing osteoporotic bone loss and fractures in postmenopausal women. In Leisure World, ERT reduced the risk of

Estrogen replacement therapy. A survey of women's knowledge and attitudes.

Because the significant health risks related to postmenopausal osteoporosis can be reduced by estrogen replacement therapy (ERT), this study assessed women's attitudes toward ERT to assess factors that might increase its use. Results showed that women taking ERT were more likely to know that decreased estrogen levels lead to osteoporosis, to perceive that menopause is a medical condition, to believe that natural approaches to menopause are less preferable, to be seeing a gynecologist for care, and to believe that women should take ERT for hot flashes. The study suggests that a systematic educa

Estrogen replacement therapy in the patient treated for endometrial cancer.

Adenocarcinoma of the endometrium is considered to be an estrogen-dependent neoplasia and as such, hormone replacement therapy is said to be contraindicated. The authors are unaware of any data to substantiate that statement. Patients, who had completed their therapy for stage I carcinoma of the endometrium, were placed on estrogen hormone replacement therapy in a nonrandomized fashion. Between 1975 and 1980, 221 patients with stage I adenocarcinoma of the endometrium were managed at the Duke University Medical Center. Forty-seven patients received estrogen after their cancer therapy, whereas

Prolactinoma and estrogens: pregnancy, contraception and hormonal replacement therapy.

The stimulatory role of estrogen on prolactin secretion and on proliferation of lactotropic cells is well established in terms of physiology but could this phenomenon be extended to include harmful effects of estrogens on prolactinoma? The aim of this review is to provide an up-to-date assessment of this subject with regard to pregnancy, use of contraceptive pills and postmenopausal hormone replacement therapy. Dopamine agonists allow women presenting prolactinoma to recover their ovulation cycles and become pregnant. There is no adverse data concerning the safety of dopamine agonists such as

Estrogen replacement therapy in women with a history of breast cancer.

Estrogens are known as potent mammary mitogen substances and are the major stimulus for the growth of hormone-dependent tumors and clearly implicated in the pathogenesis of breast cancer. Therefore it is a general belief that hormone replacement therapy (HRT) after breast cancer will increase the risk of developing recurrences, though there are no clear data available to support this suggestion. No prospective study with a large number of patients and a long treatment period was performed concerning this issue. On the other hand it may not be justifiable to withhold hormone replacement therapy

Recommendations regarding estrogen replacement therapy after treatment of endometrial cancer.

A history of endometrial cancer has long been considered a contraindication to estrogen replacement therapy. Yet women with such a history, like other women when postmenopausal, often suffer vasomotor symptoms that could easily be relieved with estrogen. In fact there is no good evidence that estrogen significantly increases the risk of recurrence after treatment for endometrial cancer. Some studies now suggest that estrogen plus a progestin may actually decrease the risk of cancer recurrence in these patients. The benefits of estrogen in preventing osteoporosis and cardiovascular disease in p

Estrogen replacement therapy and fibrocystic breast disease in postmenopausal women.

The association between estrogen replacement therapy and fibrocystic breast disease was assessed in a hospital-based case-control study undertaken in Connecticut from 1979 to 1981. The cases were 143 postmenopausal women with biopsy-confirmed fibrocystic breast disease, and the controls were 355 postmenopausal women with other surgical conditions. Use of estrogen replacement therapy was positively associated with fibrocystic breast disease; the odds of disease increased with duration of use, reaching an approximately fivefold excess odds for those who had taken menopausal estrogens for 10 or m
