Mycoplasma genitalium infection in Eswatini amid syndromic case management: prevalence, coinfections, diagnostic challenges and treatment gaps.

BACKGROUND: The global epidemic of Mycoplasma genitalium (MG) is marked by its widespread prevalence, varied resistance patterns, and significant impact on sexual health. This study aimed to understand the prevalence and interaction of MG infections with other sexually transmitted infections (STIs) in a low-resource setting, as well as the implications for routine STIs care. METHODS: This nested cross-sectional study was conducted from July 2022 to April 2023 across six outpatient care sites in Shiselweni, Eswatini. Participants completed a self-questionnaire, underwent syndromic case managem

Determinants of viral load suppression among adolescents on antiretroviral therapy in Eswatini: a cross-sectional study.

BACKGROUND: The goal of antiretroviral therapy (ART) is to achieve a sustained HIV suppressed viral load. However, adolescents often present poor adherence to ART which is associated with lower rates of viral load suppression (VLS). The objective of this study was to determine the viral load suppression levels and the associated factors among adolescents living with HIV (ALHIV) and on ART in Eswatini. METHODS: We conducted cross-sectional analysis of data from 911 adolescents aged 10 to 19 living with HIV and on ART between the period January 2017 and September 2022. We collected data of demo

Evaluating the impact of achieving cascade equality in Eswatini: a modeling study on the prevention impacts of antiretroviral therapy.

OBJECTIVE: Inequalities in the antiretroviral therapy (ART) cascade across subpopulations remain an ongoing challenge in the global HIV response. Eswatini achieved the UNAIDS 95-95-95 ART cascade targets by 2020, with differentiated programs to minimize inequalities across subpopulations, including for female sex workers (FSWs) and their clients. We sought to estimate the impacts of this achievement, through a retrospective impact evaluation of ART scale-up in Eswatini. DESIGN: Drawing on population-level and FSW-specific surveys, we developed a compartmental model of heterosexual HIV transmi

Enhanced Integration of TB Services in Reproductive Maternal Newborn and Child Health (RMNCH) Settings in Eswatini.

Tuberculosis (TB) primarily affects women during their reproductive years and contributes to maternal mortality and poor pregnancy outcomes. For pregnant women living with HIV (WLHIV), TB is the leading cause of non-obstetric maternal mortality, and pregnant WLHIV with TB are at increased risk of transmitting both TB and HIV to their infants. TB diagnosis among pregnant women, particularly WLHIV, remains challenging, and TB preventive treatment (TPT) coverage among pregnant WLHIV is limited. This project aimed to strengthen integrated TB and reproductive, maternal, neonatal and child health (R

Impact of using creative arts programming to support HIV treatment in adolescents and young adults in Eswatini.

BACKGROUND: In 2018, approximately 1.6 million adolescents (aged 10-19) were living with HIV worldwide, with the highest HIV prevalence found in Eswatini. Adolescents and young adults living with HIV are a vulnerable population due to unique psychosocial challenges that come with having a stigmatizing disease. This group struggles more than other age-groups with medication adherence and requires novel approaches to supporting treatment, including peer-group encouragement, and self-expression. METHODS: We piloted a theater camp for a group of adolescents and young adults enrolled at our HIV cl

Integrating HIV and primary healthcare for key populations: community-led models from Vietnam, Nigeria and Eswatini.

INTRODUCTION: Key populations (KP), including men who have sex with men, people who inject drugs, sex workers, transgender people and people in closed settings, are disproportionately affected by HIV and face structural and legal barriers to care. While community-led responses are central to reaching KP, services are often disease-specific and disconnected from national primary healthcare (PHC) systems. PHC, defined by WHO as a whole-of-society approach to delivering integrated and person-centred services, is rarely designed to meet the broader health needs of KP, who also experience high burd

Factors associated with stillbirth in four selected hospital maternity units: an unmatched case-control study in Eswatini.

BACKGROUND: Stillbirth is a substantially under-recognized adverse pregnancy outcome that predominantly occurs in low-middle-income countries. In 2019, Eswatini's stillbirth rate was 13.2 per 1,000 births, higher than global targets. However, there is limited research on stillbirths in Eswatini. This study analyzed factors associated with stillbirths among women giving birth in four selected hospitals. METHODS: A multi-center unmatched case-control study was conducted using secondary data sources from July 1 to December 31, 2021. Birth records of 268 stillbirths (cases) and 1,151 live births

Enhancing demographic survey protocols to characterise household dynamics that influence health - a participatory approach from rural Eswatini.

Standard Western demographic survey protocols fail to capture dynamics, such as circular migration and support networks, that profoundly influence the health of non-Western domestic social groups, typically called households. Enhanced protocols are needed because survey data provide the primary evidence base for health policy and planning globally. We present the participatory development, implementation and analysis of a novel demographic survey protocol, that aimed to better capture domestic social dynamics in rural Eswatini, southern Africa. The multiple-method study incorporated participat

A qualitative exploration of women's choices and experiences of using oral and vaginal HIV pre-exposure prophylaxis in Eswatini.

Understanding women's decision-making when offered different HIV pre-exposure prophylaxis (PrEP) options is critical for improving uptake. This qualitative study explored factors influencing PrEP choices among Swazi women in 2022-2023. In-depth interviews were conducted with 17 women who accepted or declined oral PrEP or the Dapivirine vaginal ring; and 6 healthcare workers who prescribed PrEP. One focus group discussion (FGD) with eight women eligible for PrEP and three FGDs with six to eight male community members explored attitudes to PrEP. Data were collected in Siswati, transcribed into E

"We will tell when we are ready": perinatally HIV-infected adolescents and self-disclosure of their status in Eswatini.

Access to antiretroviral therapy (ART) has promoted a significant decrease in mortality of vertically HIV-infected children. As a result, there has been an increasing growth of this population that reaches adolescence. These adolescents face problems such as self-disclosure and the stigma of the disease. This study aimed to determine the process followed by perinatally HIV-infected adolescents in self-disclosing their HIV status to significant others and the barriers and promoters of perinatally HIV-infected adolescents' disclosure of their HIV status to others. Data were collected from 15-19-

A stepped-wedge randomised trial on the impact of early ART initiation on HIV-patients' economic outcomes in Eswatini.

BACKGROUND: Since 2015, the World Health Organisation (WHO) recommends immediate initiation of antiretroviral therapy (ART) for all HIV-positive patients. Epidemiological evidence points to important health benefits of immediate ART initiation; however, the policy’s impact on the economic aspects of patients' lives remains unknown. METHODS: We conducted a stepped-wedge cluster-randomised controlled trial in Eswatini to determine the causal impact of immediate ART initiation on patients’ individual- and household-level economic outcomes. Fourteen healthcare facilities were non-randomly matched

Targeted next-generation sequencing implementation in Eswatini identifies rifampicin and bedaquiline resistance undetected by routine diagnostic testing.

In Eswatini, multidrug-resistant (MDR) Mycobacterium tuberculosis (Mtb) strains harbouring the rifampicin-resistance (RR) rpoB I491F mutation are missed by routine diagnostics, including GeneXpert MTB/RIF Ultra (Xpert Ultra), line probe assays (LPA), and mycobacterium growth indicator tube (MGIT) phenotypic drug susceptibility testing (pDST). To address this diagnostic gap, Eswatini introduced targeted next-generation sequencing (tNGS) in 2019. We analysed 234 patient samples enrolled from June 2021 to December 2024 with isoniazid and/or rifampicin resistance detected by routine diagnostics, o

Social complexities of informed consent and assent among young males undergoing voluntary medical male circumcision in Eswatini.

INTRODUCTION: As part of an effort to meet ambitious male circumcision targets in Eswatini, programme implementers have increasingly focused on young males, raising questions about informed consent. Males aged 10-19 years account for more than two-thirds of those circumcised since 2008 when internationally funded circumcision campaigns began in Eswatini. The ethical guidelines of these programmes conform to international standards, requiring that informed consent or assent be obtained prior to surgery. This article examines clients' levels of circumcision-related knowledge following the assent

Universal test and treat in relation to HIV disease progression: results from a stepped-wedge trial in Eswatini.

OBJECTIVES: Universal test and treat (UTT) is recommended for people living with HIV (PLHIV) to reduce morbidity/mortality and minimize transmission. However, concerns exist that this strategy may lead to more crowded hospitals, longer wait times and poorer service, adversely impacting health outcomes for clients with severe disease. We assessed how UTT was related to markers of disease progression in PLHIV overall and specifically among clients with low CD4 count/high World Health Organization (WHO) stage. METHODS: The analysis was conducted using data from a stepped-wedge trial of UTT in 14

"We know this will be hard at the beginning, but better in the long term": understanding PrEP uptake in the general population in Eswatini.

Information regarding HIV pre-exposure prophylaxis (PrEP) for the general population is largely lacking, because the majority of PrEP demonstration projects have focused on key populations. This qualitative study examines barriers and facilitators to PrEP uptake and adherence among the general population in Eswatini, where PrEP is offered through public-sector primary-care clinics. We analysed 106 semi-structured in-depth interviews with healthcare workers (n = 26), stakeholders (n = 30), and clients who initiated, continued, declined or discontinued PrEP (n = 50). Some healthcare workers and

Projected burden of hypertension-associated cardiovascular disease in people living with HIV versus HIV-negative adults in Eswatini.

BACKGROUND: Having achieved the UNAIDS 95-95-95 targets, Eswatini faces a growing burden of non-communicable diseases which are major contributors to morbidity and mortality. Hypertension-associated cardiovascular disease (CVD) is rising among people living with HIV (PLHIV) as survival improves and metabolic risks - including those linked to dolutegravir (DTG) - increase. We projected CVD burden among PLHIV and HIV-negative adults (PLWHIV) through 2045 to inform integrated HIV-CVD planning. METHODS: EMOD-HIV, an agent-based model calibrated to Eswatini's epidemic, generated HIV prevalence tra

Psychometric validation of a novel community norms measure among youth, Eswatini Violence Against Children and Youth Survey, 2022.

Social norms define what is acceptable and appropriate for women and men, boys and girls, in a given group or society. Restrictive social norms around women and men's roles and responsibilities have proven harmful for both women and men, particularly in adolescence and young adulthood, and are associated with increased risk of violence. In global settings, measurement of social norms tends to rely on proxy measures capturing attitudes, beliefs, and perspectives. Measurement of social norms on women's and men's roles and responsibilities is particularly limited among adolescents and young adult

Predictive risk assessment and targeted testing could enhance STI diagnosis in a high HIV prevalence setting in Eswatini.

Sexually transmitted infections (STIs) present significant public health challenges in resource-limited settings where syndromic management, which often leads to misdiagnosis and over-treatment, remains standard care. We retrospectively evaluated hypothetical STI risk assessment-only (Triage-only) and sequential risk assessment plus STI diagnostic testing (Triage&Test) approaches as potential enhancements to syndromic management in a high-burden setting. A retrospective analysis was conducted using historic data from a cross-sectional study of adults (≥ 18 years) accessing six outpatient care

Association of tuberculosis active case finding with knowledge, attitudes, stigma and health-seeking behaviour among patients in Eswatini.

BACKGROUND: Tuberculosis (TB) poses a significant global public health challenge, with >10 million new infections reported annually, making it one of the top 10 causes of death worldwide. Eswatini has one of the world's highest TB incidence rates, with an estimated rate of 398 cases per 100 000 population in 2019, as reported by the World Health Organization. In Eswatini, despite the implementation of TB active case finding (ACF) interventions aimed at improving TB case detection and reducing community transmission, there is limited empirical evidence on their association with improved knowled

Associations between forced non-consensual first-sex and HIV among sexually active women in the Kingdom of Eswatini.

Studies continue to underscore the profound impact of sexual violence on women's health. Yet, little is known about the impact, via a complex matrix of behavioural and social factors, of first intercourse, namely forced non-consensual on HIV status, particularly among sexually active women (SAW) in low-income countries where HIV prevalence remains high. Using a national sample from Eswatini, we employed multivariate logistic regression modelling to estimate the associations between forced first-sex (FFS), subsequent sexual behaviour and HIV status among 3555 SAW aged from 15 years to 49 years.
