Effects of adjuvants to local anaesthetics on their duration. III. Experimental studies of hyaluronic acid.

The effects of addition of hyaluronic acid (sodium hyaluronate, Healon) to different local anaesthetics of the amide type on the duration of sensory or motor blocks following various regional anaesthetic procedures were studied in animal experiments. In the rat infra-orbital nerve block model, the addition of 0.1-0.5% hyaluronic acid (HA) to 2% prilocaine increased the duration of sensory block of varying degrees in a dose-dependent way by up to 500% of values obtained with plain prilocaine. The duration of degree 5 blocks produced by 0.5% etidocaine and 0.5% bupivacaine was also significantly

Effects of local anaesthetics on responses of human saphenous vein and bovine coronary artery to neurotransmitters, acetylcholine, noradrenaline and 5-hydroxytryptamine.

The effects of 5 different local anaesthetics, lignocaine, etidocaine, prilocaine, mepivacaine, bupivacaine, and 3 different neurotransmitters, acetylcholine (ACh), noradrenaline (NA), 5-hydroxytryptamine (5-HT) on tone and contractility of human saphenous vein and bovine coronary artery were studied and compared in vitro. The experiments were carried out to see if there were species or tissue differences in the action of local anaesthetics and neurotransmitters at vascular smooth muscle, i.e. saphenous vein and coronary artery. Another important aspect was to see if local anaesthetics modifie

Clinical pharmacokinetics of local anaesthetics.

The introduction of the new long acting local anaesthetics, bupivacaine and etidocaine, has stimulated an expansion of interest in regional anaesthesia, particularly for obstetrical applications and pain therapy. System toxicity following injection of local anesthetics occurs albeit infrequently, and tentative correlations have been made between the onset of CNS and cardiovascular effects and circulating drug concentrations in both adults and neonates. Amongst other factors, interpretation of these relationships depends upon blood distribution and plasma binding of the agents, sampling sites a

[The effect of local anaesthetics on uterine blood flow and O2-consumption in pregnant sheep (author's transl)].

The local anesthetics bupivacaine, etidocaine, lignocaine and chloroprocaine were infused intravenously for 30 min to pregnant sheep, prepared for chronic studies. The amide compounds reached highere plasma concentrations than chloroprocaine. All local anaesthetics decreased uterine blood flow and O2 consumption without marked alterations of arterial or uterine venous blood gases. These changes were within the wide physiological range of variation, which was established in controls during 24 h. The findings may be of significance during paracervical block or pathological states of UBF or fetal

Clinical pharmacokinetics of epidural and spinal anaesthesia.

Epidural and spinal anaesthesia results from the interaction of local anaesthetics with nerve structures, primarily those located within the subarachnoid space. Local anaesthetics can reach the sites of action along various distribution pathways. Uptake into extraneural tissues (in particular epidural fat) and systemic absorption compete with neural tissue distribution and thereby affect the clinical potency and duration of action. Consequently, epidural doses must be much higher than spinal doses. The systemic absorption of lignocaine (lidocaine), bupivacaine and etidocaine following lumbar e

[Alkalinization of local anesthetics: theoretically justified but clinically useless].

PURPOSE: In vitro studies have demonstrated the potential advantages of alkalinization on anaesthetic activity, by decreasing the ratio of ionized to nonionized molecules, there by permitting more rapid penetration of local anaesthetic through biological membranes, thus decreasing the onset time. The proportion of each form depends on the pKa of the agent and the ultimate pH of the solution. When NaHCO3 is mixed with local anaesthetics, CO2 is produced. Carbon dioxide has been reported to enhance local anaesthetic action by diffusion trapping of the cationic form in pH gradient combined with a

Effect of local anaesthetics on the stimulus-secretion coupling in bovine adrenal chromaffin cells.

This study was carried out to determine the relative potencies of local anesthetics to inhibit the cholinergic synaptic transmission using cultured bovine adrenal chromaffin cells, and to clarify if the inhibitory action would correlate with biophysical and pharmacological properties. Local anaesthetics (bupivacaine, etidocaine, tetracaine, lignocaine and procaine; 0.02-2 mM) inhibited carbachol-induced catecholamine release from the cells in a concentration-dependent manner. This inhibition was completely reversible. IC50 (concentration of 50% inhibition) of each anaesthetic showed no correla

A review of the metabolism of amide local anaesthetic agents.

(1) Amide local anaesthetics are almost completely metabolized before excretion in both animals and man. (2) Considerable interspecies variability occurs in the quantitative excretion of local anaesthetic metabolites; however, qualitative similarities often exist. (3) The secondary amine metabolite of lignocaine, MEGX, is capable of reacting in vivo with acetaldehyde, formed as a metabolite of ethyl alcohol, to produce a cyclic condensation product. (4) Although hydrolysis in man of the secondary amine metabolite of lignocaine, MEGX, appears extensive, the same is not true for the metabolic an

In vitro controlled release kinetics of local anaesthetics from poly(D,L-lactide) and poly(lactide-co-glycolide) microspheres.

Poly(D,L)lactide and polylactide-co-glycolide drug-loaded microspheres were prepared with lipopholic (bupivacaine and etidocaine) and hydrophilic (mepivacine and lidocaine) local anaesthetics. Formulations of drug-loaded microspheres were characterized by the drug content, the in-vitro release kinetics and by the physical state of the drug within the microspheres. Release rates of the local anaesthetics from the microspheres were different and could not be accounted for by the intrinsic dissolution rates of the drugs. The encapsulation efficiency was highly dependent on the lipophilicity of th

Diffusive transport properties of some local anesthetics applicable for iontophoretic formulation of the drugs.

As part of a general study to improve the iontophoretic delivery of local anaesthetics of the amide type, the diffusion properties of the hydrochloride salts of bupivacaine, etidocaine, lidocaine, mepivacaine, prilocaine and ropivacaine, were studied in a 1% w/w agarose hydrogel. A source drug solution (25 mM) was placed in contact with the gel and, after an appropriate time, the drug concentration profile in the gel was analyzed to give a diffusion coefficient, D. The values of Dx10(10) expressed in m(2) s(-1) were: (bupi) 6.71, (eti) 6.71, (ropi) 6.39, (mepi) 7.31, (lido) 7.49 and (prilo) 7.

[The effect of local anaesthetics on uterine blood flow and O2-consumption in non-pregnant sheep (author's transl)].

11 non-pregnant sheep were prepared for chronic studies with an electromagnetic flow transducer placed around one uterine artery and with catheters in both uterine veins, aorta and inferior vena cava. The 4 local anaesthetics bupivacaine, etidocaine, lignocaine and chloroprocaine were infused intravenously on consecutive days in a randomized manner. During the infusions of all 4 local anaesthetics, uterine blood flow decreased due to an increase of uterine vascular resistance. The uterine arteriovenous O2-difference increased, while the uterine O2-consumption decreased without development of a

Study of anaesthetic agents for their ability to elicit porphyrin biosynthesis in chick embryo liver.

Effects of some anaesthetic drugs on the activity of delta-aminolevulinate synthetase and on the formation of porphyrins and cytochrome P-450 were studied in 18-day-old chick embryo livers in ovo. The drugs were either tested alone or with a small dose of 1,4-dihydro-3,5-dicarbethoxycollidine, which reproduces in the embryo liver a partial block in the heme biosynthesis pathway similar to that found in cells of human patients with porphyrias. Two series of local anaesthetics were tested: procaine and its derivatives (proxymetacaine, oxybuprocaine, butacaine and tetracaine) had no (or very slig

[Systemic toxicity of local anesthetics. Pharmacokinetic and pharmacodynamic factors].

Local anaesthetics can have systemic adverse effects, mostly affecting the central nervous system and the heart. The physicochemical characteristics of the different local anaesthetics are recalled, for they determine the relationship between structure, activity and toxicity. The pharmacokinetic factors involved in the toxic effects of local anaesthetics, whether the drug is given in a single extravascular dose or, accidentally, within a blood vessel, are discussed. The toxic effects of repeated administrations of local anaesthetics depend on the metabolism of the drug as well as on pharmacoki

High-performance liquid chromatographic determination of bupivacaine in plasma samples for biopharmaceutical studies and application to seven other local anaesthetics.

A sensitive analytical procedure for bupivacaine dosing in plasma samples by reversed-phase high-performance liquid chromatography is described. After a two-step extraction, the analysis was performed using a C18 column and a mobile phase of 0.01 M sodium dihydrogen-phosphate (pH 2.1)-acetonitrile (80:20, v/v). The extraction yield of bupivacaine from plasma was 73.5 +/- 5.1% (mean +/- S.D., n = 10). The within-day and between-day reproducibilities at a concentration of 100 ng/ml were 2.1% and 5.6%, respectively (n = 10). Calibration curves were linear (r2 = 0.9996) between 5 and 1000 ng/ml. T
