Three patients, a mother and her two sons, with mutilating palmoplantar keratoderma (Vohwinkel's syndrome) were successfully treated with oral etretinate (Tigason). All of them had keratotic constriction furrows of one or more digits (pseudo-ainhum) with threatening spontaneous amputation. A 36-year-old North American patient has been suffering from Olmsted syndrome since early childhood. Radiographic skeletal examinations were performed in sixteen adult patients who had received etretinate therapy for various keratinizing disorders (psoriasis vulgaris 13, pustular psoriasis 2, pustulosis palmoplantaris 1) over periods ranging from 0.7 to 4.5 years. Clinical, roentgenographic and biologic features of etretinate bone toxicity in a 13-year-old girl with pachyonychia congenita syndrome are reported. Etretinate is a synthetic derivative of vitamin A that infrequently induces bone and joint abnormalities in children. Aphthous ulcers are notoriously difficult to treat. We report repeated remission of recurrent aphthous ulceration in a female while taking etretinate for plantar pustular psoriasis. Although systemic retinoids are potent teratogens and are generally inappropriate for women of childbearing potential, this case report suggests further assessment of retinoids in severe aphthosis is warranted in selected patients.

Two children, 19 months of age, were independently treated with a compassionate protocol of etretinate therapy for recalcitrant, debilitating pustular psoriasis. Laboratory test results, roentgenograms of the spine, and bone age were periodically monitored. Keratinization defects can be very severe and disfiguring diseases. The development of retinoids such as etretinate has provided us with an effective symptomatic form of oral therapy for these skin conditions. A 63-year-old woman with eruptive keratoacanthomas on sun-exposed skin is reported. Attempts were made to produce lesions by the application of a tumour homogenate to the scratched skin, followed by UV-B irradiation; this was positive in two out of three sites, but these factors were probably non-specific stimuli. Treatment with oral etretinate led to most of the tumours disappearing within a month. Recurrence was seen on stopping the drug. Recommencement of therapy followed by a maintenance dose of 10 mg on alternate days, has been associated with freedom from the skin lesions.

Mucocutaneous side-effects from etretinate are common and are usually dose-dependent, whereas hypersensitivity drug eruptions are rare. Two patients are described in whom erythema multiforme-like eruptions appeared several days after oral intake of etretinate. In one patient etretinate rechallenge again produced the skin eruption. A brief review of the less common skin manifestations of etretinate therapy is provided.

A patient with psoriasis vulgaris developed multiple purpura on the extremities and hemorrhage at the oral mucosa and gingiva with marked thrombocytopenia. This thrombocytopenia was possibly induced by a retinoid agent, etretinate, from the clinical course and data.
