Patulous eustachian tube syndrome: the relationship with sensorineural hearing loss. Treatment by eustachian tube diathermy.

The patulous Eustachian tube syndrome has previously been regarded as a troublesome but benign condition. Nine out of 13 patients reported here had evidence of cochlear damage similar to that caused by acoustic trauma. These patients also had vestibular symptoms which improved with treatment of the patulous tube. It is postulated that abnormal patency of the Eustachian tube may allow excessive middle ear pressure changes to occur which may be transmitted by abnormal ossicular movement to the cochlea. Eustachian tube diathermy using a ureteric diathermy probe is a safe and effective method of t

Eustachian tube function in tympanoplasty.

Prognostic value of preoperative Eustachian tube function was examined in 77 ears subjected to type 1 tympanoplasty. Eustachian tube function was evaluated by positive and negative pressure equalization tests, and clearance test. Based on the results of these three tests, the ears were classified into four grades. The outcome of the ear surgery was judged by postoperative hearing, presence of otorrhea, atelectasis and perforation of the eardrum. Results of the positive pressure equalization and clearance tests of the tube were correlated with the outcome of ear surgery, although the negative p

Endoscopic Evaluation of the Eustachian Tube: assessment of a novel tool for grading Eustachian tube inflammation.

BACKGROUND: Signs of inflammation are commonly encountered during endoscopic examination of the Eustachian tube (ET) region. The clinical applicability of these findings may be enhanced by use of a standardized assessment score. METHODS: Digital video recordings were obtained of 50 nasal endoscopy examinations of the nasopharyngeal portion of the ET. Four fellowship-trained rhinologists independently reviewed the videos with regard to specific physical findings: edema of the ET torus, erythema of the ET torus, exudate at the ET orifice, and presence of tubal tonsil. Scoring of this Endoscopic

Navigation-Assisted Balloon Eustachian Tuboplasty for Eustachian Tube Dilatory Dysfunction.

OBJECTIVES: Balloon Eustachian tuboplasty (BET) is a novel treatment method for Eustachian tube dilatory dysfunction (ETD). However, surgeons cannot identify the insertion depth of the catheter during BET, resulting in potential risks such as internal carotid artery (ICA) injury. Therefore, we developed an image-guided navigation balloon catheter to identify the insertion depth of the catheter and to establish awareness of the proximity of the ICA. This study aimed to evaluate the technical feasibility of this image-guided navigation balloon catheter system in patients with ETD. METHODS: Twen

Microbiome of the Eustachian tube in patients with chronic obstructive Eustachian tube dysfunction and healthy individuals.

OBJECTIVE: The Eustachian tube (ET) is essential for middle ear ventilation. Chronic obstructive ET dysfunction may lead to chronic otitis media and cholesteatoma, but its pathophysiology remains unclear. Commensal bacterial colonization of the ET has not previously been studied in healthy individuals or affected patients. STUDY DESIGN AND PATIENTS: Prospective cohort study was conducted at a tertiary academic referral center including 13 patients with chronic obstructive ET dysfunction and 37 controls. Between 2017 and 2020, intraoperative tissue samples and swabs were collected using a cont

Lipoma Arising in the Eustachian Tube.

Lipomas are among the most common soft tissue tumors. Surgical removal of lipoma is considered if the patient has symptoms or cosmetic challenges. Lipomas that develop from any fat tissue in the body and involve the eustachian tube are extremely rare. Herein, we report the case of a patient with a lipoma arising in the eustachian tube. We also summarized the literature on tumors originating from the eustachian tubes. A 62-year-old female presented to our department with a five-year history of left nasal congestion. Nasal endoscopy revealed a tumor in the left eustachian tube. The tumor was con

Assessment of the Eustachian tube: a review.

BACKGROUND: Dysfunction of the Eustachian tube (ET) can lead to negative pressure within the middle ear, which, in turn, causes various pathological changes. Multiple testing methods for ET function have been devised, each with its own advantages and disadvantages. Knowing the characteristics of each ET function test and the unique characteristics of ET dysfunction (ETD) in children are prerequisites for choosing the optimal assessment method. For a comprehensive diagnosis, assessment should also include the localisation of any sites of obstruction. This review aims to summarise the methods of

Compliance of the patulous eustachian tube.

The compliance and ventilatory functions of the eustachian tube (ET) of 17 ears with patulous ETs were examined and compared to those of 16 ears with traumatic perforation of the eardrum (controls) by means of the tubal compliance test, the inflation-deflation test, and the forced response test. The tubal compliance was significantly lower in the patulous ETs than in the controls. An excessive patency of the ET was confirmed among the patients with patulous ETs by a low opening pressure during the inflation test and a low passive resistance on the forced response test. Active ventilatory dysfu

Autophony and the patulous eustachian tube.

Autophony and the patulous Eustachian tube is a real but rare clinical entity often misdiagnosed because the symptoms so mimic those of middle ear effusion. The diagnosis is made by the history of fullness or blockage and hearing ones own voice and breath sounds in the ear. The drum is usually atrophic and moves with respiration but this may not be visible. The usual past history is of weight loss. The diagnostic test is to observe a regular increase or decrease in middle ear pressure when increasing or decreasing the ambient pressure while measuring the impedance in a pressure chamber. The us

Vidian nerve and the eustachian tube.

The vidian nerve was stimulated electrically in the dog and this resulted in a reduced patency of the Eustachian tube. The effect was atropine resistant. This physiological evidence in the dog corresponds to anatomical evidence in man of a parasympathetic, vasodilator nerve bundle passing through the vidian to the sphenopalatine ganglion and then via the pharyngeal nerve to the Eustachian tube mucosa. Additional parasympathetic routes to the tube may exist.

Salvage Eustachian Tube Cartilage Chip Insertion After Multiple Transnasal Shim Operations in Intractable Patulous Eustachian Tube.

This article presents 2 cases of extremely intractable patulous Eustachian tube following multiple transnasal shim insertion. These cases highlight the disadvantages of repeat transnasal shim operations, including enlargement of the Eustachian tube lumen, frequent dislocation, repeat surgery, recurrent middle ear infection, and shim misswallowing. The patients in these cases were successfully treated with Eustachian tube cartilage chip insertion through a postauricular approach. We describe the surgical technique and advantages of this promising management method.

Three-dimensional computed tomography imaging of the eustachian tube lumen in patients with patulous eustachian tube.

BACKGROUND: The eustachian tube (ET) has a complicated anatomy, which is related to middle ear pathology. The anatomy of the ET has been well investigated in cadavers, but may not accurately reflect that in living subjects. Three-dimensional (3D) computed tomography (CT) was used to examine the ET in patients with patulous ET. METHOD: The subjects were 35 patients (17 males, aged 59 +/- 16 years, and 18 females, aged 44 +/- 15 years) with patulous ET which could be continuously traced from the pharyngeal orifice to the tympanic orifice (53 ears). CT was performed in the sitting position at re
