Annals of clinical psychiatry : official journal of the American Academy of Clinical Psychiatrists2015eng
BACKGROUND: To date, there has been no investigation of dissemination outcomes for cognitive-behavioral training programs for body-focused repetitive behaviors (BFRBs).
METHODS: Sixty-three past participants of the Trichotillomania Learning Center (TLC) intensive professional training institutes completed an online survey assessing referral sources, skills utilization, and treatment outcomes before and after training. The intensive training focused on the treatment of trichotillomania (hair-pulling) disorder and excoriation (skin-picking) disorder using in-person or videotaped didactics, role

The study objective was to perform a confirmatory factor analysis of the SLEEP-50 Questionnaire (SLEEP-50) in Trichotillomania (Hair-Pulling Disorder) and Excoriation (Skin-Picking) Disorder and compare sleep complaints in adults with Trichotillomania, Excoriation Disorder and non-affected controls. Participants were 234 adults with Trichotillomania, 170 with Excoriation Disorder, and 146 non-affected controls. Participants rated sleep using the SLEEP-50 and Pittsburgh Sleep Quality Index (PSQI). Confirmatory factor analysis was used to assess fit of the originally-proposed SLEEP-50 factors wi

OBJECTIVE: While individual trials suggest benefit of interventions for excoriation (skin-picking) disorder (ExD), limited systematic evaluation of treatments, or their collective benefit, exists.
METHODS: The present study examined the current state of treatments for ExD in a systematic review and meta-analysis and explored potential treatment moderators. Twelve trials were identified for review, including five with a control condition. Of these, nine were eligible to be included in the meta-analysis (three with a control).
RESULTS: A fixed-effects meta-analysis found a large overall treatm

Excoriation disorder (ED) involves uncontrollable skin picking, not explained by a dermatologic or other psychological conditions. ED is associated with emotion regulation impairment, psychosocial distress, and psychiatric comorbidities. This study investigated the efficacy of psychodrama group therapy (PGT) in reducing ED symptoms. A randomized controlled trial compared psychodrama group therapy (PGT, n = 26) and support group therapy (SGT, n = 27) as a control. The comparison included ED severity, psychosocial impact, emotion regulation, depression, anxiety, social adjustment, and general cl

Body-focused repetitive behavior disorders (BFRBs) include Trichotillomania (TTM; Hair pulling disorder) and Excoriation (Skin Picking) Disorder (SPD). These conditions are prevalent, highly heterogeneous, under-researched, and under-treated. In order for progress to be made in optimally classifying and treating these conditions, it is necessary to identify meaningful subtypes. 279 adults (100 with TTM, 81 with SPD, 40 with both TTM and SPD, and 58 controls) were recruited for an international, multi-center between-group comparison using mixture modeling, with stringent correction for multiple

Trichotillomania (TTM) and skin-picking disorder (SPD) are body-focused repetitive behavior disorders (BFRBDs) associated with deficits in emotion regulation, distress tolerance, and experiential avoidance. Their differential relations to TTM, SPD, and their comorbidity, and relative importance in predicting BFRBD outcomes is unclear. Therefore, this study compared these constructs in adults (n = 311) diagnosed with TTM (n = 112), SPD (n = 87), comorbid TTM and SPD (n = 40), and healthy controls (n = 72), and examined their relative importance in association with BFRBD outcomes. Participants c

The present study assessed sleep functioning in Trichotillomania (TTM; Hair-Pulling Disorder) and Excoriation (Skin-Picking) Disorder (ExD), and a non-affected comparison group, and examined the prevalence and correlates of bedtime and sleep-related hair pulling and skin picking. Participants were adult internet survey respondents, who met diagnostic criteria for TTM (N=259), ExD (N=182), or did not meet criteria for these disorders (N=148). Individuals with TTM and ExD endorsed significantly greater sleep disturbance relative to the comparison group, even after controlling for internalizing (

Trichotillomania (TTM) and excoriation disorder (ED) are body-focused repetitive behaviors (BFRBs) characterized by recurrent hair-pulling or skin-picking; they may present with psychiatric comorbidities worsening outcomes. While cross-sectional and case-control studies report high comorbidity rates, longitudinal analyses remain limited. We looked to determine incidence of new psychiatric diagnoses, psychopharmacologic prescriptions, and psychotherapy referrals following BFRB diagnosis compared with controls and assess whether psychiatric history modified outcomes. Adults with ICD-10 diagnoses

BACKGROUND: Excoriation (skin-picking) disorder (SPD) is a relatively common psychiatric condition whose neurobiological basis is unknown.
AIMS: To probe the function of fronto-striatal circuitry in SPD.
METHOD: Eighteen participants with SPD and 15 matched healthy controls undertook an executive planning task (Tower of London) during functional magnetic resonance imaging (fMRI). Activation during planning was compared between groups using region of interest and whole-brain permutation cluster approaches.
RESULTS: The SPD group exhibited significant functional underactivation in a cluster e

Skin picking disorder (SPD) and trichotillomania (hair pulling disorder, or HPD) significantly overlap in terms of clinical features. However, few studies have directly compared structural brain data in these disorders. The aim of this study was to compare volumes of brain structures and cortical thickness in patients with SPD and HPD, and determine involvement of fronto-striatal pathways. Seventeen female SPD, 17 HPD and 15 healthy age-matched controls underwent clinical assessment and structural MRI imaging. Group differences were determined in brain volume and cortical thickness, controllin

A plethora of health-related concerns, including skin disorders, are particularly common during the critical periods of adolescence and young adulthood. Their reactions to skin-related disorders can be different from their reactions to other disorders. While medical problems could be physiologically impacting, skin disorders like excoriation disorder can, in addition, directly affect their physical appearance in ways that have psychological consequences like anxiety, low self-esteem, and impaired quality of life. The increasing prevalence of excoriation disorder has resulted in its inclusion i

Excoriation, or skin picking disorder (SPD), is a common and impairing condition, yet many individuals face barriers to accessing evidence-based care. Digital interventions offer a scalable option, but few have been evaluated in real-world settings. This study examined naturalistic outcomes among 2063 adult users of StopPicking, a self-guided online intervention for SPD. The program includes three modules: Module 1 focuses on self-monitoring and assessment, Module 2 delivers personalized behavioral strategies, and Module 3 supports maintenance and relapse prevention. Symptom severity was asses

BACKGROUND: Trichotillomania (TTM), excoriation disorder, onychophagia, and onychotillomania are categorized as body focused repetitive behavior (BFRB) disorders, causing damage to the skin, hair, and/or nails with clinically significant psychosocial consequences. Currently, there are no standardized treatments for these compulsive, self-induced disorders. Studies on treatment of these disorders using psychotropic drugs (i.e., selective serotonin reuptake inhibitors, tricyclic antidepressants, anticonvulsants) have shown variable efficacy. Recently, there is a growing interest in N-acetylcyste

Acceptance and commitment therapy (ACT)-enhanced behavior therapy (A-EBT) is the most empirically supported treatment of trichotillomania. Support for A-EBT for excoriation disorder is budding. In this article, we review the A-EBT model, including support for targeting psychological inflexibility, as well as the use of traditional behavioral procedures to reduce non-focused behaviors. To date there are 17 studies investigating A-EBT for trichotillomania and skin picking across ages and treatment delivery methods (individual, group, telehealth, and asynchronous web-based delivery). The work has

Psychogenic excoriation (also called neurotic excoriation, acne excoriée, pathological or compulsive skin picking, and dermatotillomania) is characterised by excessive scratching or picking of normal skin or skin with minor surface irregularities. It is estimated to occur in 2% of dermatology clinic patients and is associated with functional impairment, medical complications (e.g. infection) or substantial distress. Psychogenic excoriation is not yet recognised in the DSM. We propose preliminary operational criteria for its diagnosis that take into account the heterogeneity of behaviour associ

BACKGROUND: Psychogenic excoriation, characterized by excessive scratching or picking of the skin, is not yet recognized as a symptom of a distinct DSM-IV disorder. The purpose of this study was to provide data regarding the demographics, phenomenology, course of illness, associated psychiatric comorbidity, and family history of subjects with psychogenic excoriation.
METHOD: Thirty-four consecutive subjects were recruited from an outpatient dermatology practice and by advertisement. Subjects completed the Structured Clinical Interview for DSM-IV augmented with impulse control disorder modules

Despite growing interest in the link between sleep disturbance and psychopathology, much remains unknown regarding obsessive-compulsive and related disorders (OCRDs). The present review integrates contemporary research on the relations between sleep and OCRDs, including hoarding disorder, trichotillomania, and excoriation disorder in order to inform future programmatic research. Findings indicate a robust link between sleep disturbance and OCD; however, the impact of sleep on OCD treatment outcome is unclear. Extant work on sleep disturbance in hoarding disorder indicates a relationship indepe
