Exercise capacity in COPD patients with exercise-induced pulmonary hypertension.

BACKGROUND: Pulmonary hypertension (PH) in patients with COPD is associated with reduced exercise capacity. A subgroup of COPD patients has normal mean pulmonary artery pressure (mPAP) at rest, but develops high mPAP relative to cardiac output (CO) during exercise, a condition we refer to as exercise-induced pulmonary hypertension (EIPH). We hypothesized that COPD patients with EIPH could be identified by cardiopulmonary exercise test (CPET) and that these patients have lower exercise capacity and more abnormal CPET parameters compared to COPD patients with normal hemodynamic exercise response

Exercise induced complete atrioventricular block: Utility of exercise stress test.

Exercise induced complete atrioventricular block (EIAVB) is a relatively uncommon condition. This phenomenon is clinically important because it can mimic symptoms of other cardiovascular conditions and may be associated with exercise intolerance and subsequent syncope. A 76year old man with long-standing hypertension and diabetes mellitus presented with recurrent episodes of lightheadedness and syncope with physical activity. ECG showed sinus rhythm with first degree atrioventricular block. Echocardiography did not show any valvular disease causing his symptoms. Coronoary angiographic evaluati

Exercise-induced bronchoconstriction and exercise-induced respiratory symptoms in nurses.

In order to assess prevalence and characteristics of exercise-induced respiratory symptoms (EIRSs) and exercise-induced bronchoconstriction (EIB) in health care workers, we performed a cross-sectional study including 48 female nurses from primary care settings and an equal number of female office workers studied as a control group. The evaluation of examined groups included completion of a questionnaire, skin prick tests to common inhalant allergens, spirometry, and exercise and histamine challenge. We found a similar prevalence of EIRSs and EIB in both groups. EIB was closely related to asthm

Post-exercise vitamin C supplementation and recovery from demanding exercise.

The aim of this study was to investigate whether post-exercise vitamin C supplementation influences recovery from an unaccustomed bout of exercise. Sixteen male subjects were allocated to either a placebo (P; n=8) or vitamin C (VC) group ( n=8). Subjects performed a prolonged (90-min) intermittent shuttle-running test, and supplementation began after the cessation of exercise. Immediately after exercise the VC group consumed 200 mg of VC dissolved in a 500 ml drink, whereas the subjects in the P group consumed the drink alone. Later on the same day and then in the morning and evening of the fo

Exercise at the Extremes: The Amount of Exercise to Reduce Cardiovascular Events.

Habitual physical activity and regular exercise training improve cardiovascular health and longevity. A physically active lifestyle is, therefore, a key aspect of primary and secondary prevention strategies. An appropriate volume and intensity are essential to maximally benefit from exercise interventions. This document summarizes available evidence on the relationship between the exercise volume and risk reductions in cardiovascular morbidity and mortality. Furthermore, the risks and benefits of moderate- versus high-intensity exercise interventions are compared. Findings are presented for th

Quantification of exercised-induced myocardial deformation with exercise CMR.

BACKGROUND: Physiological stress may reveal myocardial deformations not evident at rest. Single-beat myocardial tagging in exercise cardiovascular magnetic resonance (Ex-CMR) enables assessment of myocardial deformation at both rest and during exercise. In this study, we developed the myocardial torsional reserve (MTR), a quantitative metric of exercise-induced rotational deformation, and evaluated its potential as a marker of abnormal myocardial mechanics in patients with overt or exercise-induced heart failure with preserved ejection fraction (HFpEF). METHODS: Free-breathing balanced steady

Pre-Exercise Caffeine Ingestion Leads to Greater Exercise-Induced Torque Reduction by Increasing Eccentric Load During Resistance Exercise.

PURPOSE: This study examined the impact of pre-exercise caffeine supplementation on post-exercise torque reduction, an indirect marker of exercise-induced muscle damage (EIMD), while focusing on the changes in eccentric load induced by the ergogenic effects of caffeine. METHODS: In double-blind, crossover trials, 15 healthy young men ingested 6 mg/kg caffeine or placebo, followed by an eccentric exercise task at 30-120° of knee extension: 100 isokinetic eccentric maximal voluntary knee extensions in Study 1 (i.e., variable eccentric load by caffeine) and 100 isoload (fixed at 120% of concentr

Exercise for depression.

RATIONALE: Depression is a common cause of morbidity and mortality worldwide. Depression is often treated with antidepressants or psychological therapy, or both, but some people may prefer alternative approaches such as exercise. This review updates one first published in 2008 and last updated in 2013. OBJECTIVES: To determine the effectiveness of exercise in the treatment of depression in adults compared with no intervention, waiting list control or placebo, or where exercise is used as an adjunct to an established treatment that is received by both exercising and non-exercising groups. Primary Exercise Headache.

PURPOSE OF REVIEW: Primary exercise headache has gone through many descriptors in the past but generally is a headache that is precipitated by strenuous exercise without significant intracranial pathology. Its presentation can remain vague, often confused with other primary and secondary headache disorders and thus undertreated. This review aims to discuss primary exercise headache in the context of epidemiology, presentation, pathophysiology, differential diagnosis, and treatment. RECENT FINDINGS: Two large epidemiological studies in Iran and Japan have further characterized a predilection f

Cardiopulmonary Exercise Test.

The cardiopulmonary exercise test (CPX) is an essential examination for detecting pathophysiological derangement and determining treatment policy because it clarifies not only the changes of hemodynamics but also abnormality in the whole body during exercise where heart disease patients often feel symptoms. To utilize CPX effectively, we must understand each parameter, such as peak oxygen uptake (peak VO2), peak VO2/HR, and VE/VCO2. In addition, comparison of each parameter, for example, peak VO2 and VE/VCO2, and peak VO2 and peak VO2/HR, is useful to detect the pathophysiological abnormalities

Exercise-induced pneumomediastinum.

BACKGROUND: A sudden onset of chest pain, which often reflects a life-threatening disease, requires prompt diagnosis in the emergency department. FINDINGS: A 12-year-old boy presented with sustained chest pain and dyspnea after diving into a swimming pool and was transferred to our emergency department. A chest examination noted a crunching and rasping sound at the precordium, synchronous with the heartbeat. Chest radiography showed lucent streaks and the mediastinal pleura at the left cardiac outline. Additionally, computed tomography showed massive pneumomediastinum surrounding the heart. T

Exercise-induced anaphylaxis.

Exercise-induced anaphylaxis is an uncommon disorder in which anaphylaxis occurs in response to physical exertion. Food-dependent exercise-induced anaphylaxis is a disorder with similar symptoms, although symptoms develop only if exercise takes place within a few hours of eating and, in most cases, only if a specific food is eaten. Management includes education about safe conditions for exercise, the importance of ceasing exercise immediately if symptoms develop, appropriate use of epinephrine, and, for patients with food-dependent exercise-induced anaphylaxis, avoidance of the culprit food fo

Exercise-induced anaphylaxis.

A 23-year-old man presented with acute flushing, pruritus and warmth followed by collapse after vigorous exercise in a gymnasium. After resting for 30 min and receiving a rapid infusion of 0.9% sodium chloride, he was finally stable. He admitted that he had a similar experience 5 years earlier during exercise. Based on the patient's history, his symptoms were attributed to exercise-induced anaphylaxis. None of his episodes was associated with any suspicious co-triggers of anaphylaxis. He was successfully discharged from hospital without any complications after receiving guidance on how to prev
