Environmental health : a global access science source2026eng
The exposome encompasses all environmental exposures humans undergo from conception. We frame exposome studies according to four broad aims essential for environmental health research. First, a descriptive aim, consisting in assessing exposure patterns, including correlations between exposures and their within-subject variability. This descriptive aim includes "environmental justice" studies of associations between the exposome and sociodemographic factors. A second - etiologic - aim is to describe the subclinical and clinical effects of the exposome (hazard identification). A third aim is the

Sleep is fundamental for infant development and health, playing a critical role in cognitive, socio-emotional, and physical growth. However, environmental factors can impact the quality and duration of sleep in infants. This review synthesises current evidence on the associations between environmental chemical exposures and infant sleep outcomes, with a focus on the first 1000 days of life. Infants may be exposed to environmental pollutants before birth, through the placenta, or after birth, via breastfeeding, diet, and external sources such as inhalation, dust contact, or hand-to-mouth exposu

Rosacea is a chronic, inflammatory dermatosis driven by a complex interplay of genetic, environmental and lifestyle factors, collectively known as the exposome. This review explores how intrinsic contributors such as genetic susceptibility, immune dysregulation, microbiome alterations, hormonal influences and psychosocial stress intersect with extrinsic triggers like ultraviolet radiation (UVR), air pollution, dietary factors, and climate variability to shape rosacea pathogenesis. Recent advances in single-cell transcriptomics have identified fibroblasts as key components of inflammatory and v

Fragile X syndrome (FXS) is the leading known monogenetic cause of autism with an estimated 21-50% of FXS individuals meeting autism diagnostic criteria. A critical gap in medical care for persons with autism is an understanding of how environmental exposures and gene-environment interactions affect disease outcomes. Our research indicates more severe neurological and metabolic outcomes (seizures, autism, increased body weight) in mouse and human models of autism spectrum disorders (ASD) as a function of diet. Thus, early-life exposure to chemicals in the diet could cause or exacerbate disease

Pregnancy complications including fetal growth restriction, preeclampsia, and preterm birth, as well as gestational diabetes, affect one in every four to five pregnancies. Accumulating evidence indicates that increased production of reactive oxygen species accompanies these complications. Given that reactive oxygen species are cell stress-inducing agents, they may have a causal role in disease pathophysiology, although the exact mechanisms by which they contribute to pregnancy complications are not completely understood. Since many environmental and lifestyle factors and exposures are known to

The development and plasticity of the endocrine pancreas responds to both the intrauterine and postnatal exposome in a constant attempt to predict and respond to alterations in nutritional availability and metabolic requirements. Both under- and over-nutrition in utero, or exposure to adverse environmental pollutants or maternal behaviors, can each lead to altered β-cell or function at birth, and a subsequent mismatch in pancreatic hormonal demands and secretory capacity postnatally. This can be further exacerbated by metabolic stress postnatally such as from obesity or pregnancy, resulting in

Diabetes mellitus arises from complex interactions between biological susceptibility and diverse environmental influences that extend beyond traditional "genes versus lifestyle" models. The exposome is defined as the aggregate of non-genetic environmental exposures and their biological consequences across the life course. It provides a systems-level framework to evaluate this interaction, spanning chemical, physical, social, and behavioral domains while integrating them with internal molecular responses to decode how environments shape metabolic health. Emerging assessment strategies, includin

Environmental risk factors are recognized as essential determinants of the cardiovascular disease burden but are still poorly represented in clinical guidelines. External environmental exposures such as air pollution, traffic noise, heat, light pollution, chemical contamination of soil and water as well as urban planning, our social environment and lifestyle contribute significantly to the global disease burden and premature deaths. In particular, cardiovascular morbidity and mortality are significantly influenced by these environmental stressors. The exposome concept provides an overarching f

Government policies targeting key risk factors for cognitive decline are potential levers for preventing or delaying dementia. Policy is therefore a central component of the exposome-the cumulative exposures shaping dementia risk across the life course. While extensive research links individual risk factors to dementia, less attention has been paid to how public policies structure these risks. We introduce the concept of the policy exposome and summarize findings from a structured expert consultation conducted by the Gateway Exposome Coordinating center (GECC) to identify priority policy resea

The early-life exposome influences future health and accelerated biological aging has been proposed as one of the underlying biological mechanisms. We investigated the association between more than 100 exposures assessed during pregnancy and in childhood (including indoor and outdoor air pollutants, built environment, green environments, tobacco smoking, lifestyle exposures, and biomarkers of chemical pollutants), and epigenetic age acceleration in 1,173 children aged 7 years old from the Human Early-Life Exposome project. Age acceleration was calculated based on Horvath's Skin and Blood clock

Growing evidence supports the importance of lifestyle and environmental exposures-collectively referred to as the 'exposome'-for ensuring immune health. In this narrative review, we summarize and discuss the effects of the different exposome components (physical activity, body weight management, diet, sun exposure, stress, sleep and circadian rhythms, pollution, smoking, and gut microbiome) on immune function and inflammation, particularly in the context of the current coronavirus disease 2019 (COVID-19) pandemic. We highlight the potential role of 'exposome improvements' in the prevention-or

The worldwide rise of neurological and psychiatric conditions poses major challenges. However, current global research remains fragmented, dominated by limited cohorts and poorly integrated datasets that disconnect whole-body health, exposome, and brain health. Theories rarely unify brain measures with extracerebral factors or capture heterogeneity in individual trajectories. We introduce multimodal diversity, a non-linear, non-simplistic causal and ecological construct integrating data representation, whole-body and exposomic factors, and computational modeling to address this situated, embed

INTRODUCTION: We evaluated the risk factors associated with gastrointestinal disorders (GD) among the soldiers of the Army and Marine Aviation community (AMAC) using an exposomic approach. Specifically, we aimed to determine the medical and operational factors associated with reported GD in the Military Health System.
METHODS: Longitudinal data were obtained from the Medical Assessment and Readiness System (MARS) housed at Womack Army Medical Center, Fort Liberty, NC, for a retrospective cohort study that included 79,249 active duty United States AMAC active duty service members (ADSMs) from

BACKGROUND: Sleep health and depression are multidimensional constructs, yet their shared determinants remain obscure. We aim to identify the key exposome factors in predicting sleep-related depression (SRD).
METHODS: We integrated regularized canonical correlation analysis with machine-learning models within a nested cross-validation to identify a shared phenotype (i.e., sleep-related depression (SRD)) and to predict SRD from exposome factors in the UK Biobank (n = 64,781). The best-performing model was validated in an independent subsample at baseline and follow-up (n = 8,139) and in a clin

Biological aging clocks offer estimations of aging and dementia, yet scalability is limited. We introduce a large-scale, cross-national speech clock derived from 2928 individuals across five Latin American countries, spanning healthy controls (HCs), mild cognitive impairment (MCI), Alzheimer's disease (AD), and non-language/language-dominant frontotemporal dementia (nldFTD/ldFTD). Multimodal acoustic and linguistic features were trained with supervised models to estimate chronological age, generating speech age gaps (SAGs) as cross-sectional markers of deviations from chronological age, with p

INTRODUCTION: Age-related macular degeneration (AMD) is a multifactorial disease, but it remains unclear how external exposures - the exposome - promote its development. Metabolomics can provide insights to better understand AMD pathophysiology. Previous work to date has focused primarily on associations between endogenous metabolites and AMD.
OBJECTIVES: This study aimed for the first time to investigate associations of exogenous metabolites in plasma and urine and AMD.
METHODS: Cross-sectional study including patients with AMD and a control group (> 50 years) from Boston, US and Coimbra, P

BACKGROUND: While ultraviolet radiation (UVR) present in sunlight is recognized as the main etiological agent of skin cancer, the most frequent form of which is basal cell carcinoma (BCC), other exposome factors like pollution, diet, and lifestyle may also contribute. This study aimed to investigate the association of BCC and exposome-related factors in the Spanish population.
METHODS: BCC cases (n = 119) and controls (n = 127) with no history of skin cancer were recruited between April 2020 and August 2022 by 13 dermatologists throughout Spain in this prospective multicenter case-control stu
