Comparative Diagnostic Accuracy of Pleural Effusion Classification Methods in Cats: An Analysis of Naturally Occurring Cases.

BACKGROUND: Pleural effusion (PE) in cats can result from transudative or exudative processes. Transudates are caused by decreased colloid osmotic pressure (↓COP) or elevated hydrostatic pressure (↑HP) gradient, while exudates arise from increased pleural capillary permeability. Diagnostic classification approaches traditionally rely on pleural effusion total protein (TPPE) and total nucleated cell counts (TNCCPE). In contrast, Light's criteria employing pleural effusion lactate dehydrogenase (LDHPE), LDHPE divided by serum LDH (LDHratio), and pleural effusion to serum protein ratio (TPratio)

Soluble leukocyte selectin in the analysis of pleural effusions.

STUDY OBJECTIVES: To determine if soluble leukocyte selectin (sL-selectin) levels in serum and pleural fluid (PF) are an inflammatory marker that differentiates pleural effusion transudates from exudates. DESIGN: sL-selectin PF and serum levels were measured in consecutive patients and compared to established criteria. SETTING: A tertiary-care military medical center. PATIENTS: One hundred twenty patients undergoing diagnostic or therapeutic thoracentesis. INTERVENTIONS: PF and serum samples were collected during thoracentesis and analyzed separately for sL-selectin levels. Results were co

Simplified Light's Criteria and Acute Phase Proteins Reflect Aetiology of Feline Body Cavity Effusions Better than the Traditional Classification Scheme.

The traditional veterinary classification (TVC) of effusions based on cell count and total protein (TP) does not adequately reflect the aetiology. Light's criteria (LC) (activity of lactate dehydrogenase [LDH] in the effusion [LDHef], effusion/serum LDH ratio [LDHr], effusion/serum TP ratio [TPr]), serum-effusion albumin gradient (ALBg), acute phase proteins (APPs) [serum amyloid A (SAA), α1-acid glycoprotein (AGP), haptoglobin] might aid classification. The aim was to evaluate the utility of these parameters except LDHr in differentiating exudates from transudates. Sixty-five cats with effusi

Evaluation of pleural and peritoneal effusions.

Certain diseases cause an increase in the amount of fluid present in the pleural and/or peritoneal cavity (an effusion). Uroperitoneum subsequent to kidney, ureter, bladder, or urethra rupture also can cause an increased amount of fluid in the abdomen. Evaluation of fluid samples often is helpful in identifying the mechanism causing the effusion and, occasionally, results in a specific diagnosis. The TP, TNCC, and general cytologic examination can be performed easily, quickly, and inexpensively in-house. The TP and TNCC are used to classify effusions as transudates, modified transudates, or ex

New strategies to classify canine pleural effusions and the diagnostic value of acute phase proteins, amylase, and adenosine deaminase in pleural exudates.

BACKGROUND: In dogs, simplified Light's criteria can discriminate transudates from exudates. Other tests used in human medicine are pleural effusion cholesterol (CHOLPE) and butyrylcholinesterase [BChEPE], the pleural effusion/serum ratio of these analytes (CHOLratio and BChEratio), and the serum albumin minus pleural effusion albumin gradient (SEAG). OBJECTIVES: We aimed to assess the diagnostic accuracies of different biomarkers in dogs with pleural effusion in differentiating exudates from transudates. Secondarily, we evaluated the potential diagnostic utility of pleural effusion acute pha

Diagnostic Performance of the Cancer Ratio in Malignant Pleural Effusion in Combination with the Light's Criteria.

The cancer ratio (CR), defined as serum lactate dehydrogenase to pleural fluid adenosine deaminase ratio (sLDH/pADA), has emerged as a diagnostic marker for malignant pleural effusion (MPE). However, previous studies have primarily compared MPE with exudative effusions, excluding transudates. The aim of this study was to determine the diagnostic accuracy of CR in differentiating MPE from benign pleural effusions (BPE), including both exudates and transudates. An observational analysis was conducted on patients with pleural effusion (PE) evaluated in a Portuguese center between January 2017 and

Association between echogenicity of ascites and its classification in dogs.

BACKGROUND: Ascites in dogs results from a range of systemic and localized disorders. Early distinction between transudative and exudative peritoneal effusions, and between septic and non-septic exudates, is crucial for guiding emergency interventions and treatment strategies. OBJECTIVE: To evaluate the relationship between echogenicity of ascites and results of fluid analysis, and to determine whether echogenicity can help differentiate transudates from exudates. METHODS: Retrospective study including 100 client-owned dogs with ultrasonographically confirmed peritoneal effusion and fluid an

ACE/ACE2 ratio and MMP-9 activity as potential biomarkers in tuberculous pleural effusions.

OBJECTIVE: Pleural effusion is common problem, but the rapid and reliable diagnosis for specific pathogenic effusions are lacking. This study aimed to identify the diagnosis based on clinical variables to differentiate pleural tuberculous exudates from other pleural effusions. We also investigated the role of renin-angiotensin system (RAS) and matrix metalloproteinase (MMPs) in the pathogenesis of pleural exudates. EXPERIMENTAL DESIGN: The major components in RAS and extracellular matrix metabolism, including angiotensin converting enzyme (ACE), ACE2, MMP-2 and MMP-9 activities, were measured

Profile of Metalloproteinases and Their Association with Inflammatory Markers in Pleural Effusions.

BACKGROUND: Matrix metalloproteinases (MMPs) are responsible for the breakdown of the extracellular matrix and play an important role in the inflammatory processes of pleural exudates. The imbalance between MMPs and their inhibitors (TIMPs) is present in various pathological processes. OBJECTIVE: To evaluate the profile of MMPs and TIMPs in pleural effusions of different etiologies correlated with inflammatory markers. METHODS: The patients with pleural effusion due to tuberculosis (TB), cancer (CA) or transudate were prospectively evaluated. Pleural fluid was submitted to cytological, bioch

Pleural Fluid Analysis: Are Light's Criteria Still Relevant After Half a Century?

Fifty years from their initial description, Light's criteria are still unhesitatingly accepted as the default reference test for separating pleural transudates and exudates. Efforts should be focused not so much on trying to find an even more reliable technique for categorizing PEs but on improving the misclassification rate of transudates that characterize Light's criteria. Despite their shortcomings, Light's criteria may well continue their reign for another 50 years. Long live the Light's criteria!

Pleural fluid and serum soluble interleukin-2 receptors in pleural effusions.

Pleural fluid and serum soluble Interleukin-2 receptors (sIL-2R) were measured by an enzyme-immunoassay in 13 patients with tuberculous pleurisy, in 28 patients with carcinomatous pleurisy and in 17 transudates from patients with congestive heart failure. Significantly higher values of sIL-2R were observed in exudative than in transudative (mean +/- SEM = 713 +/- 111 U/ml) pleural fluid samples, the highest being found in tuberculous (3777 +/- 501 U/ml) and the intermediate in carcinomatous exudates (1981 +/- 160 U/ml) (p less than 0.0001; on way ANOVA). Serum sIL-2R were significantly higher

Computed tomography attenuation value for the characterization of pleural effusions in dogs: A cross-sectional study in 58 dogs.

CT attenuation value can help to differentiate exudate from transudate in people. The aim of this cross-sectional study was to assess the utility of CT in characterizing pleural effusions based on attenuation values in a population of dogs having CT and diagnostic thoracentesis within 48 h of each other. The CT attenuation values were determined using four circular, same size, regions of interest (ROIs) placed on the same CT slice with the greatest quantity of fluid. Values of each ROI were recorded and the mean of the four ROIs mean values (mean of the means) was calculated and considered as
