Ocular and eyelid involvement in collagen vascular diseases. Part II. Dermatomyositis, scleroderma, and sarcoidosis.

Collagen vascular disease is a heterogeneous group of autoimmune diseases that affect multiple organ systems. Sjögren syndrome, dermatomyositis, scleroderma, systemic lupus erythematosus, and sarcoidosis are collagen vascular diseases that often present with characteristic cutaneous manifestations. Although less known, various ocular manifestations that affect both external and internal structures of the eye can also be seen in these conditions. Multidisciplinary management between dermatologists and ophthalmologists is essential in the early diagnosis and management of collagen vascular disea

Full thickness eyelid transsection (blepharotomy) for upper eyelid lengthening in lid retraction associated with Graves' disease.

AIM: To report on a modified technique for upper eyelid lengthening in lid retraction associated with Graves' disease. METHODS: A prospective consecutive interventional case series. 41 patients, 38 women and three men, with a mean age of 55 years (range 32-75 years) were included. Full transsection of the upper eyelid with optional transsection of the lateral horn was performed in 60 lids. A central bridge of conjunctiva was left intact in the pupillary axis. Before and after surgery, the skin crease, palpebral aperture, and scleral show were measured and the contour of the lid margin was ass

A Review of Applications and Intracellular Mechanisms of Intense Pulsed Light in Eyelid Inflammatory Diseases.

Objective: To evaluate relevant clinical outcomes and conclude possible mechanisms of intense pulsed light (IPL) in eyelid inflammation. Background: IPL devices were primarily applied in cutaneous vascular malformations and have been used in ocular diseases for about 20 years, mostly including meibomian gland dysfunction (MGD), blepharitis, and ocular rosacea. Recent findings: Seventy-two original clinical researches were included, 57 for MGD, 4 for blepharitis or blepharitis-related keratoconjunctivitis, and 11 for rosacea. Dry eye symptoms, (tear) break-up time (BUT), and meibomian structure

Presence of Eyelid Disease in Habitual Scleral Lens Wearers.

Background/Objectives: As scleral lens (SL) use continues to expand, particularly for medically necessary management of corneal irregularity and ocular surface disease, it is important to determine if wear of these lenses is associated with eyelid disease. This study evaluated habitual SL wearers for meibomian gland obstruction, meibum quality, and signs of lid margin inflammation. Methods: Eligible participants wore scleral lenses in one or both eyes for any indication and had been wearing their current lens design(s) for at least six months, with habitual wear of ≥5 h per day, six days per w

Effects of Teprotumumab on Eyelid Retraction in Thyroid Eye Disease.

PURPOSE: This study evaluates the efficacy of teprotumumab in reducing eyelid retraction in thyroid eye disease (TED) patients. METHODS: This retrospective study included patients with active or chronic moderate-to-severe TED who completed at least 4 cycles of teprotumumab. Patients with upper and/or lower eyelid retraction, defined as margin-to-reflex distance (MRD) 1 and/or MRD2 of more than 5 mm, in one or OU were included. The main outcome measure was a change in MRD1 and MRD2 after treatment. Changes in MRD1 and MRD2 were each analyzed for correlation (r) with changes in exophthalmolmetr

Pyoderma gangrenosum of the eyelid associated with inflammatory bowel disease.

PURPOSE: Pyoderma gangrenosum (PG) of the eyelid can be difficult to diagnosis and may mimic other, more common pathologies, thereby delaying proper treatment and management. PG may be associated with systemic disorders that have significant comorbidities. OBSERVATIONS: The authors present two cases of pyoderma gangrenosum of the eyelid associated with inflammatory bowel disease. CONCLUSIONS AND IMPORTANCE: This case series highlights the importance of early recognition of eyelid pyoderma gangrenosum to avoid local and systemic comorbidities with timely and appropriate management.

Intraepithelial sebaceous carcinoma of the eyelid misdiagnosed as Bowen's disease.

BACKGROUND: Sebaceous carcinoma (SC) is well known for its ability to masquerade clinically and histologically as a variety of periocular conditions resulting in a delayed diagnosis. We present a series of periocular SC cases and discuss the difficulties in histopathological diagnosis when this tumor presents with a Bowenoid pattern of intraepithelial spread. METHODS: A retrospective case study of all patients with SC of the eyelid treated in our Hospital, from 1997 to 2004, was conducted. RESULTS: Eight patients were identified (four females and four males). Seven cases involved the upper e

Nonsurgical management of upper eyelid retraction in thyroid eye disease.

Upper eyelid retraction (UER) is the most common sign of thyroid eye disease (TED) and may result in lagophthalmos and exposure keratopathy. Measures to address UER are varied and include conservative treatment, surgical intervention, and injections of botulinum toxin, hyaluronic acid (HA) filler, and triamcinolone acetonide (TA). Our article will discuss the various nonsurgical aspects of managing TED-related UER, focusing on the injections of botulinum toxin, HA filler, and TA to the upper eyelid, which have all been reported to be effective in improving UER in both active and inactive state

Kimura Disease Presenting As an Eyelid Mass in a Young Asian Male.

The diagnosis of Kimura disease should be strongly considered in any patient of Asian descent who presents with painless subcutaneous nodules predominantly involving the head and neck region in association with marked eosinophilia and elevated immunoglobulin E. Surgical resection of the lesions can be performed to prevent relapse. Here, the authors report a case of Kimura disease presenting as an eyelid mass in a 13-year-old Asian boy with pronounced eosinophilia and elevated immunoglobulin E levels.

Genetic diseases affecting the eyelids: what should a clinician know?

PURPOSE OF REVIEW: The molecular basis of a number of inherited diseases that affect the eyelids has been elucidated over the last two decades. Due to the vast number of these diseases, a clinician may become overwhelmed by the volume of data, making it difficult to incorporate newer information into his or her clinical practice. This article intends to review the recent developments of inherited diseases that affect the eyelids that a typical oculoplastic surgeon will encounter. RECENT FINDINGS: This review proposes categorizing genetic diseases affecting the eyelids on rarity and whether th

Acquired lower eyelid epiblepharon in patients with thyroid eye disease.

PURPOSE: To describe a series of patients with acquired epiblepharon secondary to thyroid-related orbitopathy. METHODS: Retrospective case series of 9 patients with thyroid-related orbitopathy who had epiblepharon. RESULTS: Seven patients had epiblepharon in both lower eyelids and 2 patients had it in one lower eyelid. Six of the 9 patients were Asian. Each patient had punctate keratopathy. Epiblepharon resolved in all but one patient after treating coexisting manifestations of thyroid-related orbitopathy. One patient required epiblepharon repair. CONCLUSIONS: We describe a new finding of a

Eyelid movements in health and disease. The supranuclear impairment of the palpebral motility.

The eyelid movements are mediated mainly by the orbicularis oculi (OO) and the levator palpebrae superioris (LPS) muscles. Dissociated upper lid functions exhibit different counterbalanced action of these muscles, and in blinking they show a strictly reciprocal innervation. The disturbance of this close LPS-OO relationship likely leads to many of the central lid movement disorders. Three groups of supranuclear motor impairment of lid movements are considered: the disorders of the lid-eye movements' coordination, the disturbances of blinking and lid "postural" maintenance, and the alteration of
