A 72-year-old woman presented with a 3 × 2 × 1-mm right lower eyelid micronodular, whitish solid lesion at the nasal eyelid margin that had caused madarosis. Local excision led to the microscopic discovery of 2 epidermoid cysts, one of which harbored a basal cell carcinoma arising from its orthokeratinizing squamous cell lining. Sebaceous gland carcinomas of the eyelids are rare neoplasms accounting for less than 1% of all eyelid tumors. They usually mimic chalazia and undergo repeated curettage before a definitive diagnosis is made. Although they are relatively slow-growing tumors, they behave in an aggressive manner, frequently metastasizing to regional lymph nodes. OBJECTIVE: To describe a modified H-figure sliding skin flap for treating eyelid central neoplasms and to evaluate the cosmetic and functional results of this reconstructive blepharoplasty.

A 77-year-old woman presented with an ulcerated nodule on her right lower eyelid that was unresponsive to broad-spectrum antibiotics or incision and drainage. Coccidioidomycosis was suspected because of previous serologic testing and concurrent indurated cutaneous nodules. Although skin biopsies were not diagnostic, cultures from the eyelid were positive for Coccidioides immitis. The patient was initially treated with oral fluconazole, with resolution of the eyelid and skin lesions. She ultimately completed a 6-month course of oral azole therapy, and at 18-month follow-up she was symptom free.

A 59-year-old woman on continuous treatment with corticosteroids and immunosuppressive drugs for pemphigus vulgaris of 14 year's duration presented with an enlarging nodular lesion on the right lower eyelid. The neoplasm, diagnosed as basal cell carcinoma, was excised conserving the lid margin, and eyelid reconstruction was done with a rotated cheek flap. Wound healing proceeded without complications in spite of immunosuppressive treatment, and the final cosmetic and functional result was satisfactory.

Syringocystadenoma papilliferum (SCAP) is a rare cutaneous adnexal neoplasm with variable clinical appearance and characteristic histology. It arises in about one third of cases within a sebaceous hamartoma (SH) and in this case, multiple other benign adnexal neoplasms may be associated with it. OBJECTIVE: To report on patients with mucinous eccrine adenocarcinoma of the eyelid.
DESIGN: Retrospective case series.
PARTICIPANTS: Biopsy specimens of 6 patients with eyelid neoplasm were obtained. The pathologic diagnoses were mucinous eccrine adenocarcinomas.
METHODS: The medical records, including demographic, clinical, histopathologic, and follow-up information, were reviewed.
RESULTS: Five patients underwent surgical removal of the lesion by means of Mohs micrographic surgery. Four of these patients were doing well; 1 of them, with positive margins of resection, had 2 recurrences.

Sebaceous carcinoma is a malignant neoplasm that usually arises in the sebaceous glands of the eyelids. Its pathogenesis is unknown; however, irradiation history, immunosuppression, and use of diuretics are known risk factors. The mainstay of treatment for sebaceous carcinoma of the eyelid is wide surgical resection with a safety margin of 5 to 6 mm, which often results in full-thickness defects.
