Parenteral MCT/ω-3 Polyunsaturated Fatty Acid-Enriched Intravenous Fat Emulsion Is Associated With Cytokine and Fatty Acid Profiles Consistent With Attenuated Inflammatory Response in Preterm Neonates: A Randomized, Double-Blind Clinical Trial.

BACKGROUND: Soybean oil-based intravenous fat emulsion (IVFE) administered to preterm neonates can induce oxidative stress and inflammatory response, which are associated with severe complications of prematurity. This study aimed to test the hypothesis that administration of medium-chain triglyceride (MCT)/ω-3 polyunsaturated fatty acid (PUFA)-enriched IVFE in preterm neonates is associated with a cytokine and fatty acid (FA) profile consistent with attenuated inflammatory response. PATIENTS/METHODS: In a double-blind randomized study, 60 preterm neonates (gestational age 26-32 weeks) were ra

Fat emulsion in surgical patients with liver disorders.

Total parenteral nutrition (TPN) with fat emulsion was evaluated in 68 patients with liver disorders who were admitted to the First Department of Surgery, Osaka City University Medical School. All patients received transabdominal direct operation for esophageal varices. Immediately after their operations, 28 received TPN with carbohydrate and fat emulsion as an energy source (FAT group), and 40 received TPN with only carbohydrate as an energy source (non-FAT group). Liver function tests, blood glucose levels, intravenous fat tolerance tests, serum lipid levels, fatty acid composition of serum

Fat overload with a 10% soybean oil emulsion.

A 10% soybean oil emulsion (Intralipid) has been demonstrated to be an effective means of delivering an adequate caloric intake via a peripheral vein. This emulsion has generally been considered safe and free of significant complications. This is a report of a child who initially thrived on this intravenously administered fat emulsion, then suddenly developed a lifethreatening intolerance to the infusion, which appeared to be the "fat overload syndrome." This syndrome, seen frequently with earlier fat emulsions, has not been reported previously as a complication of the 10% soybean oil emulsion

Nutrition, the gastrointestinal tract and the acquired immune deficiency syndrome. Facts and perspectives.

Diarrhoea and malnutrition are common findings in patients with the Acquired Immune Deficiency Syndrome (AIDS). In this disease, enteropathy leads to fat and D-xylose malabsorption and chronic non-specific inflammation of the small bowel. Moreover, gastrointestinal infection can induce severe diarrhoea. Depletion in real body cell mass, body fat content, and weight loss have been observed. Nutritional therapy is mandatory when weight loss is 10% or greater. Enteral feeding is not easily achieved. Parenteral feeding including fat as a nonprotein calorie source improves general condition. The us

Accuracy of plasma turbidity measurement for determining fat intolerance during total parenteral nutrition.

The accuracy of plasma turbidity measurements in predicting ability to metabolise intravenous fat emulsions during total parenteral nutrition was studied in 35 adult surgical patients. Plasma turbidity, expressed as a light scattering index (LSI), was determined by nephelometry and compared with measured triglyceride (TG) concentrations. A poor coefficient of correlation was found between LSI and TG (r = 0.52). The sensitivity and specificity of LSI in predicting TG concentration were 19% and 96% respectively. This indicates that the measurement of LSI is more useful in ruling out hypertriglyc

A Case Report of Flecainide Toxicity With Review of Literature.

Flecainide is an anti-arrhythmic drug with a narrow therapeutic index. Flecainide toxicity is rare, but the mortality is high. This case demonstrates the use of intravenous fat emulsion therapy in conjunction with intravenous sodium bicarbonate treatment for flecainide toxicity. A 50-year-old male with atrial fibrillation and taking flecainide 75 mg twice daily presented to Emergency Department after ingesting 1125 mg of flecainide, in a suicide attempt. An electrocardiogram (ECG) on arrival showed bradycardia, wide QRS complex, prolonged QTc interval. Treatment for flecainide poisoning with i

Intravenous lipid emulsions designed to meet preterm infant requirements increase plasma and tissue levels of docosahexaenoic acid and arachidonic acid in mice.

BACKGROUND & AIMS: Intravenous lipid emulsions used in preterm infants contain insufficient docosahexaenoic acid (DHA) and arachidonic acid (ARA) to support normal development, resulting in deficiencies that contribute to complications of prematurity and cognitive delay. We sought to investigate the effects of new intravenous lipid emulsions designed to contain sufficient DHA and ARA to meet preterm needs, while avoiding liver toxicity. METHODS: Three new lipid emulsions (NLE A-C) were laboratory-generated using high pressure homogenization. First, a long-term experiment evaluated the impact

Essential fatty acid deficiency during total parenteral nutrition.

Essential fatty acid (EFA) deficiency has become a clinical problem since the advent of fat-free total parenteral nutrition (TPN). The following study was done to determine the minimum fat requirements for patients receiving continuous TPN solution. Seventy-seven patients who had 97 courses of TPN of at least 14 days duration were prospectively studied. The following fat supplementation was given: a) none, b) 10% soybean oil emulsion intravenously at fixed dosage, c) fat from an oral diet, or d) intravenous and oral fat. No patient was EFA deficient before the onset of TPN. EFA deficiency was

Effect of parenteral serum plant sterols on liver enzymes and cholesterol metabolism in a patient with short bowel syndrome.

Hepatobiliary complications are common during parenteral nutrition. Lipid moiety in commercially available solutions contains plant sterols. It is not known whether plant sterols in parenteral nutrition interfere with hepatic function in adults. We detected how different amounts of plant sterols in parenteral nutrition solution affected serum plant sterol concentrations and liver enzymes during a 1.5-year follow-up in a patient with short bowel syndrome. Serum lipid, plant sterol, and liver enzyme levels were measured regularly during the transition from Intralipid (100% soy-based intravenous

New method for administration of hydrochloric acid in metabolic alkalosis.

In a new method for peripheral intravenous infusion of hydrochloric acid the HCl is buffered in an aminoacid solution and infused with a fat emulsion. The aminoacids and the fat emulsions are stable in the presence of HCl, and the transfusion set is resistant to the chemical actin of 0.15 mol/l HCl. Two case-reports show that HCl can be administered safely through a peripheral vein.

Successful treatment of flecainide-induced cardiac arrest with extracorporeal membrane oxygenation in the ED.

Flecainide is a class Ic antidysrhythmic agent used to prevent and treat tachydysrhythmias. Flecainide toxicity primarily causes cardiovascular and neurologic effects through sodium-channel blockade. There is scant evidence to support specific management, and recommended therapies have been extrapolated from management of other sodium-channel blocking drugs. Traditionally, these therapies have consisted of intravenous fluids, sodium bicarbonate, vasopressors, and cardiac pacing. Novel therapies include intravenous fat emulsion and venoarterial extracorporeal membrane oxygenation (VA-ECMO). A 2

[Effect of fat emulsion (intralipid) on essential fatty acid deficiency during total parenteral nutrition in pediatric patients. Part 1. Experimental study].

Experimental studies were performed to confirm the effect of TPN with and without fat emulsion, Intralipid, on fat metabolism in weanling rats and puppies. Long-term fat-free TPN induced essential fatty acid (EFA) deficiency in weanling rats and the intravenous fat emulsion which accounted for 10% of the total caloric intake could prevent EFA deficiency. In the long-term TPN in growing puppies, fatfree TPN and induced EFA deficiency within two weeks, and Intralipid which accounted for 4% of the total caloric intake (2% as linoleate) satisfied the EFA requirement.

Fat overload syndrome after the rapid infusion of SMOFlipid emulsion.

Fat overload syndrome is a well-known complication of intravenous lipid emulsion therapy. It is characterized by headaches, fever, jaundice, hepatosplenomegaly, respiratory distress, and spontaneous hemorrhage. Other symptoms include anemia, leukopenia, thrombocytopenia, low fibrinogen levels, and coagulopathy. Several reports in the literature describe fat overload syndrome caused by rapid infusion of lipid emulsions, all with soybean-based lipid emulsions. We report fat overload syndrome in a 2-year-old girl with short bowel syndrome on home parenteral nutrition. Fat overload syndrome occurr

[Studies on metabolism of fat emulsion in endotoxemic dogs].

Changes of fat metabolism in endotoxemic dogs were studied by means of intravenous fat tolerance test. Twelve dogs subjected to the present studies were divided into two groups; peritonitis was produced in one group (6 dogs) and 1.5 mg/kg of E. coli lipopolysaccharide was injected intravenously in another groups (6 dogs). After production of peritonitis, Limulus test was positive in all of the six dogs, however there were no significant changes in blood sugar levels, lactate/pyruvate ratio, and elimination rate of fat emulsion from the venous blood (K-value) after the induction of endotoxemia

Parenteral nutrition in a patient with familial Type IV hypertriglyceridemia: a dilemma.

Patients with a Type IV familial hypertriglyceridemia should never receive intravenous fat emulsions. In addition, the amount of carbohydrate that can be given is limited. A 33-year-old white female, with Type IV hypertriglyceridemia, began receiving a parenteral nutrition regimen that included fat. A series of lipid studies was planned, coordinated with fat emulsion and glucose administration. The patient had persistently elevated basal triglyceride levels (greater than 250 mg/dL), which rose in response to both fat and carbohydrate administration. Serum cholesterol levels remained within the

Plasma protein adsorption patterns on surfaces of Amphotericin B-containing fat emulsions.

Nephrotoxicity of the conventional Amphotericin B formulation Fungizone is the most common side effect in treatment of systemic fungal infections. Lipid formulations of Amphotericin B including fat emulsions showed a reduced nephrotoxicity. In vivo distribution studies of lipid formulations have shown an accumulation of Amphotericin B in liver and spleen, while concentration in the kidneys is reduced. Blood proteins adsorbed onto particles after intravenous administration are regarded as the key factors determining their in vivo fate. Two-dimensional polyacrylamid gel electrophoresis is a powe

Analysis of phospholipids and triacylglycerols in intravenous lipid emulsions.

Intravenous lipid emulsions (ILEs) are used for parenteral nutrition, providing a vital source of essential fatty acids and concentrated energy for patients who are unable to absorb nutrients via the digestive track. They are commonly used to treat local and non-local anesthetic toxicity, and lipophilic drug overdose. ILE are composed of natural lipids, and the composition of these natural lipids can be varied based on their source. The lipids are susceptible to hydrolytic degradation with time, resulting various lipid degradation products such as Lysophosphatidylcholines (LPs), affecting the

The spectrophotometric absorbance of Intralipid.

The spectral absorbance of Intralipid, a phospholipid emulsion, was investigated to discern its effect, when parenterally administered, on the spectrophotometric measurement of hemoglobin (Hb), oxyhemoglobin (HbO2), and carboxyhemoglobin (HbCO). While accounting for dilutional factors, various concentrations of Intralipid in both water and hemoglobin solutions were analyzed at six wavelengths commonly used to measure Hb, HbO2 and HbCO. Absorbance increased linearly with Intralipid concentration at all wavelengths, and ranged from 0.034 at 505 nm to 0.019 at 626.6 nm per mg of Intralipid. There
