Ultrasound-based prediction of fetal hyperinsulinemia: development of a fetal metabolic score.

OBJECTIVES: Gestational diabetes mellitus (GDM) is associated with fetal hyperinsulinemia, excessive adiposity, and adverse perinatal outcomes; sonographic markers of fetal adiposity, liver enlargement, and septal thickening may allow direct assessment of fetal metabolic adaptation. This study aimed to develop an ultrasound-based fetal metabolic score (FMS) estimating the risk of fetal hyperinsulinemia. METHODS: The FMS was developed from a prospective cohort of 223 pregnant women (128 with GDM, 95 controls). Sonographic markers of neonatal hyperinsulinemia were identified using umbilical cor

Fetal brain development in fetal growth restriction using MRI: a systematic review.

BACKGROUND: This systematic review investigates potential differences in brain development between growth restricted (FGR)-fetuses compared to appropriate for gestational age (AGA) fetuses using MRI. METHODS: PubMed, Embase, Cochrane Library and Web of Science databases were searched from 1985 to 2023. FGR was defined as an estimated fetal weight (EFW) < p10 and/or an abdominal circumference (AC) < p10, or 20% reduction in EFW or AC using a minimum interval of two weeks. Outcomes included volumetrics, biometrics, apparent diffusion coefficients (ADC), 1H-MRS-metabolites, and oxygenation of th

Fetal Brain Development in Congenital Heart Disease.

Neurodevelopmental impairments are the most common extracardiac morbidities among patients with complex congenital heart disease (CHD) across the lifespan. Robust clinical research in this area has revealed several cardiac, medical, and social factors that can contribute to neurodevelopmental outcome in the context of CHD. Studies using brain magnetic resonance imaging (MRI) have been instrumental in identifying quantitative and qualitative difference in brain structure and maturation in this patient population. Full-term newborns with complex CHD are known to have abnormal microstructural and

Maternal buprenorphine treatment and fetal neurobehavioral development.

BACKGROUND: Gestational opioid use/misuse is escalating in the United States; however, little is understood about the fetal effects of medications used to treat maternal opioid use disorders. OBJECTIVE: The purpose of this study was to determine the effect of maternal buprenorphine administration on longitudinal fetal neurobehavioral development. STUDY DESIGN: Forty-nine buprenorphine-maintained women who attended a substance use disorder treatment facility with generally uncomplicated pregnancies underwent fetal monitoring for 60 minutes at times of trough and peak maternal buprenorphine le

Maternal cadmium, placental PCDHAC1, and fetal development.

Cadmium (Cd) is a ubiquitous environmental contaminant implicated as a developmental toxicant, yet the underlying mechanisms that confer this toxicity are unknown. Mother-infant pairs from a Rhode Island birth cohort were investigated for the potential effects of maternal Cd exposure on fetal growth, and the possible role of the PCDHAC1 gene on this association. Mothers with higher toenail Cd concentrations were at increased odds of giving birth to an infant that was small for gestational age or with a decreased head circumference. These associations were strongest amongst those with low level

Sonographic Assessment of the Fetal Penile Development.

The intrauterine diagnosis of micropenis is an important clue in the discernment of some syndromes and hormonal deficiencies. In this study, we tried to establish reference ranges for the fetal penile length and penile width. This prospective cross-sectional study included 179 healthy singleton male fetus pregnancies that were between 17 and 37 weeks of gestation. Of these pregnancies, the fetal penile length and width were measured using trans-abdominal ultrasound. The correlation coefficients of gestational age with penile measurements were calculated. We observed that as the gestational age

MRI of fetal and neonatal cerebellar development.

In the last few years magnetic resonance imaging (MRI) has expanded its diagnostic capability in the evaluation of fetal brain. Starting from the 18th to 20th gestational weeks, MRI can reliably depict fetal brain anatomy and locate pathology, offering a robust and reliable tool for the assessment of disorders of the fetal central nervous system. In this review quantitative and qualitative in vivo MRI data on fetal cerebellar development are presented. Our normative reference data have been obtained from a database of 580 MR fetal imaging studies. Normal cases were selected within normal MRI f

Influence of fetal environment on kidney development.

Several lines of evidence, mostly derived from animal studies, indicate that changes in fetal environment may affect renal development. Besides maternal hyperglycemia or drug exposure, that were recently found to alter nephrogenesis, changes in vitamin A supply to the fetus may prove to be responsible for most of the variations in nephron number found in the population. A low vitamin A status in the fetus may be a major cause of inborn nephron deficit, either as a feature of intrauterine growth retardation or independently of growth retardation. The possibility that vitamin A status may also i

Inbreeding effects on fetal growth and development.

The effects of inbreeding on the incidence of congenital anomalies and measurement at birth of offspring were explored on the basis of a prospective survey done during 1969 to 1974. The pregnancies of over 20 000 married women from rural and urban samples in North Arcot District were identified and followed up through routine home visiting once every 5 weeks by qualified women investigators. Of all marriages, 47% in the rural areas and 29% in the urban were consanguineous. The average inbreeding coefficients in the rural and urban areas were 0·0371 and 0·0204, respectively; these are significa

Development of autonomic control of fetal circulation.

Development of parasympathetic and sympathetic reflexes controlling heart rate, vascular pressures, and blood flows was investigated in fetal lambs weighing 300-5,800 g (65-165 days' gestation). Cardiovascular responses to veratridine injections, atrial stretching, bilateral cervical vagotomy, and cholinergic blockade with atropine were used to test parasympathetic activities. Responses to propranolol and phenoxybenzamine were used to test beta- and alpha-adrenergic activities. Autonomic ganglionic blockade and stimulation provided additional information on both cholinergic and adrenergic syst

Interdependence of placenta and fetal cardiac development.

The placenta and fetal heart undergo development concurrently during early pregnancy, and, while human studies have reported associations between placental abnormalities and congenital heart disease (CHD), the nature of this relationship remains incompletely understood. Evidence from animal studies suggests a plausible cause and effect connection between placental abnormalities and fetal CHD. Biomechanical models demonstrate the influence of mechanical forces on cardiac development, whereas genetic models highlight the role of confined placental mutations that can cause some forms of CHD. Fetal Doppler Evaluation to Predict NEC Development.

Antenatal factors play a role in NEC pathogenesis. This study aimed to investigate the predictive value of fetal ductus venosus doppler (DV) for NEC in fetal growth restriction fetuses (FGRF) and to assess the predictive accuracy of IG21 and Fenton curves in NEC development. Data from FGRF, postnatal findings, and Doppler characteristics were collected between 2010 and 2020 at a single center. Patients were then divided into two groups (i.e., with and without NEC). Bivariate and multivariate analyses were performed. We identified 24 cases and 30 controls. Absent or reversed end-diastolic flow

Developmental Programming of Fetal Growth and Development.

Maternal stressors that affect fetal development result in "developmental programming," which is associated with increased risk of various chronic pathologic conditions in the offspring, including metabolic syndrome; growth abnormalities; and reproductive, immune, behavioral, or cognitive dysfunction that can persist throughout their lifetime and even across subsequent generations. Developmental programming thus can lead to poor health, reduced longevity, and reduced productivity. Current research aims to develop management and therapeutic strategies to optimize fetal growth and development an

Hypoxia: From Placental Development to Fetal Programming.

Hypoxia may influence normal and different pathological processes. Low oxygenation activates a variety of responses, many of them regulated by hypoxia-inducible factor 1 complex, which is mostly involved in cellular control of O2 consumption and delivery, inhibition of growth and development, and promotion of anaerobic metabolism. Hypoxia plays a significant physiological role in fetal development; it is involved in different embryonic processes, for example, placentation, angiogenesis, and hematopoiesis. More recently, fetal hypoxia has been associated directly or indirectly with fetal progra

Role of melatonin in embryo fetal development.

Melatonin is an indoleamine produced by the pineal gland and secreted in a circadian manner. In the past few decades, research over this topic has been enhanced. Melatonin has many important roles in the human physiology: regulator of the circadian rhythms, sleep inducer, antioxidant, anticarcinogenic. This paper reviews the involvement of melatonin in embryo fetal development. The pineal gland develops completely postpartum, so both the embryo and the fetus are dependent on the maternal melatonin provided transplacentally. Melatonin appears to be involved in the normal outcome of pregnancy be

Neural crest development in fetal alcohol syndrome.

Fetal alcohol spectrum disorder (FASD) is a leading cause of neurodevelopmental disability. Some affected individuals possess distinctive craniofacial deficits, but many more lack overt facial changes. An understanding of the mechanisms underlying these deficits would inform their diagnostic utility. Our understanding of these mechanisms is challenged because ethanol lacks a single receptor when redirecting cellular activity. This review summarizes our current understanding of how ethanol alters neural crest development. Ample evidence shows that ethanol causes the "classic" fetal alcohol synd

Skeletal development on fetal magnetic resonance imaging.

Prenatal magnetic resonance imaging (MRI) is being increasingly used, in addition to standard ultrasound, for the diagnosis of congenital diseases beyond the central nervous system. Previous studies have demonstrated that MRI may be useful for the in utero visualization of spinal dysraphism and for differentiating between isolated and complex skeletal disorders with associated abnormalities. More recently, attention has focused on the visualization of the human fetal skeleton for the delineation of normal and pathological development of skeletal structures. On 1.5 T, in particular, echoplanar

Magnetic resonance imaging of fetal cerebellar development.

In the last few years fetal magnetic resonance imaging (MRI) has been proposed as a second level technique in the evaluation of fetal brain anomalies. It has been demonstrated that MRI is highly accurate in illustrating the morphologic changes of developing brain and fetal brain abnormalities being a useful procedure when ultrasonography is inconclusive or doubtful. Starting from the 19-20 weeks gestational age (GA), MRI can reliably depict fetal brain anatomy and locating pathology, offering a robust and reliable tool in the assessment of fetal CNS diseases.
