[Biometry of the large bones of fetal extremities in the 3d trimester].

Ultrasound measurement of head, thorax and the limb bones femur, tibia, humerus and radius was made of 285 fetuses with known gestational age. In addition measurements of the fibula were obtained in 120 fetuses and of the ulna in 125 fetuses. With the use of these data a normal growth curve for each limb bone was established, which can be helpful in detecting late fetal limb malformations as heterozygous achondroplasia. All limb bones show a nearly linear growth rate so that the length of the bones can alternatively be used to determine gestational age. The linear relationship between growth o

Fetal lung volumetry by three-dimensional ultrasound.

The aim of this study was to establish a method for determination of fetal lung volume by three-dimensional ultrasound. The thoraces of a total of 113 fetuses (singleton pregnancies, 11-41 weeks of gestation without any signs of malformation or oligohydramnios) were examined by three-dimensional ultrasound. Volumetric evaluation of each lung was performed in each of three perpendicular planes (six measurements in total). There were significant differences in all three measurements between the left and right lungs. Especially in the second and third trimesters, measurement of the frontal and th

Fetal pulmonary maturity as determined by particle electrophoresis.

Particle electrophoresis has been used to investigate particles obtained from amniotic fluid. These particles are defined mainly as unilamellar and multilamellar liposomes by electron microscopy. A total of 172 samples of amniotic fluids from 132 pregnancies were included in this study. The electrophoretic results were compared to the total phospholipids and to neonatal outcomes in 42 cases. At least two kinds of particles from amniotic fluids with different electrophoretic mobilities (EPM) are evident. With a ratio of F greater than 1.0, the presence of electrophoretically faster particles wa

The effect of low amniotic pressure without oligohydramnios on fetal lung development in a rabbit model.

OBJECTIVE: Our purpose was to examine fetal lung development in reduced intraamniotic pressure without amniotic fluid loss. STUDY DESIGN: A circular portion of uterine wall measuring 1 cm in diameter was excised while the chorionic and amniotic membranes were left intact at the twenty-third day of gestation in New Zealand White rabbits. The chorionic and amniotic membranes herniated spontaneously through the defect. RESULTS: Amniotic pressure was significantly reduced after herniation. Lung weight/body weight ratios at term were significantly reduced in experimental fetuses compared with con

The morphology of the foetal sigmoid colon in the African population: a possible predisposition to sigmoid volvulus.

AIM: This study aimed to determine the morphological types of the foetal sigmoid colon and establish if the anatomical predisposition to sigmoid volvulus is present in Black African foetuses. Sigmoid volvulus affects Black Africans in our province more frequently than other ethnic groups, and males more than females. Its aetiology remains speculative, with factors being proposed that do not suggest the pathogenetic mechanisms or the gender bias. Previous anatomical studies have suggested that Black Africans have a predisposition to sigmoid volvulus due to an elongated sigmoid colon. We investi

Intraplacental Mature Cystic Teratoma.

INTRODUCTION: Placental teratomas are benign germ cell tumors composed of mature tissues from multiple germ layers. They may mimic a cardiac fetus but lack organized fetal structures and an umbilical cord. CASE PRESENTATION: A 25-year-old G5P1224 woman delivered at 34 weeks of gestation following PPROM. Examination of the placenta revealed a 3.5 cm intraplacental, mixed solid and cystic lesion without a pedicle. Histology showed mature tissues including cartilage, ganglion cells, choroid plexus, and ciliated epithelium, consistent with a mature cystic teratoma. DISCUSSION: The absence of an

Imaging of fetal tumors and other dysplastic lesions: A review with emphasis on MR imaging.

Fetal tumors and other dysplastic masses are relatively rare. They are usually the result of failure of differentiation and maturation during embryonic or fetal life; dysplastic lesions may be the consequence of an obstruction sequence. In this review, we present the most commonly encountered tumors and masses seen during fetal life. Imaging characteristics, tumoral organ of origin, and its effect on the surrounding organs and overall fetal hemodynamics are descriptors that must be relayed to the fetal surgeon and maternal fetal medicine expert, in order to institute most accurate parental cou

Effect of maternal diabetes on the fetal exocrine pancreas.

To test the hypothesis that fetal pancreatic exocrine and endocrine function are stimulated in parallel in the diabetic pregnancy, 68 mothers with gestational and pregestational diabetes who underwent amniocenteses after 34 weeks' for the evaluation of fetal lung maturity were enrolled. Amniotic fluid specimens were analyzed for C-peptide and trypsin content. Amniotic fluid specimens were obtained from 92 non-diabetic women undergoing amniocenteses for lung maturity, preterm labor, or premature rupture of membranes. Groups were compared using the Wilcoxon rank-sum test, Kruskal Wallis rank sum

Prenatal vitamin E treatment improves lung growth in fetal rats with congenital diaphragmatic hernia.

PURPOSE: Congenital diaphragmatic hernia (CDH) is associated with pulmonary hypoplasia. To discover factors that would accelerate fetal lung growth, the authors developed models of hypoplasia, found that antioxidants improved lung growth in vitro, and then proceeded to in vivo studies. METHODS: Timed-pregnant rats were fed nitrofen (100 mg) on gestational day 9.5 (term, 22), and fetal lungs were harvested at day 13.5 and placed in organ culture in serum-free media with (n = 10) or without (n = 9) additional vitamin E (0.134 IU/mL). Camera lucida tracings were made daily on live, unstained lun

Characterization of a reversible thermally-actuated polymer-valve: A potential dynamic treatment for congenital diaphragmatic hernia.

BACKGROUND: Congenital diaphragmatic hernia (CDH) is a fetal defect comprising an incomplete diaphragm and the herniation of abdominal organs into the chest cavity that interfere with fetal pulmonary development. Though the most promising treatment for CDH is via interventional fetoscopic tracheal occlusion (TO) surgery in-utero, it has produced mixed results due to the static nature of the inserted occlusion. We hypothesize that a suitable noninvasively-actuatable, cyclic-release tracheal occlusion device can be developed to enable dynamic tracheal occlusion (dTO) implementation. OBJECTIVE:

Lung maturation in diabetes in pregnancy: if and when to test.

Poorly controlled maternal diabetes in pregnancy may delay fetal pulmonary maturation. However, diabetic women with good glycemic control have fetal lung maturation at the same gestational age as nondiabetic women. With modern ultrasound technology, gestational dates can be accurately assessed in the first or early second trimester. Respiratory distress syndrome is rare in nondiabetic and well-controlled diabetic pregnancies confirmed by early ultrasound to be at or beyond 37 weeks. Early confirmation of dates thus eliminates the need for amniotic fluid assessment of fetal lung maturity prior

The evaluation of infection and pulmonary maturity in women with premature rupture of the membranes.

Although the etiology of PROM is multifactorial, increasing evidence regarding clinical risk factors, membrane histology, membrane culture, and amniotic fluid microbiology shows a strong association with infection. Recent studies suggest an association between genital tract infection, preterm labor (PTL), and preterm premature rupture of the membranes (pPROM). If correct, this information may be used to target areas for prevention, as well as to develop management protocols. This article reviews both the diagnostic tests for the causes associated with pPROM and the evaluation of intraamniotic

Perinatal pulmonary hyperplasia due to laryngeal atresia.

A premature newborn boy, who died at birth because of cartilaginous laryngeal atresia, showed lung development that was far more advanced than normal for gestational age. The lungs, which were histologically normal, were three times the expected weight and showed a degree of alveolarization appropriate for 3 months' postnatal age. The lungs crowded the chest cavity so that the diaphragm was flattened and immobilized; the massive ascites, documented by ultrasound 5 weeks prior to delivery, appeared to be due to obstructed venous return. Thus, the pulmonary hyperplasia, a new finding in perinata

Mortality rates in low-birth-weight infants born after a positive contraction stress test.

Perinatal mortality rates and the indications for contraction stress tests (CSTs) were studied in 46 patients with positive test results and who gave birth to infants weighing between 500 and 1 500 g. Severe pre-eclampsia and intra-uterine growth retardation were the indications for the CST in the large majority of patients. The perinatal mortality rate for infants weighing between 500 and 1 000 g was 76.9%, for infants between 1 001 and 1 250 g 38.5%, and only 25% when the infants weighed 1 251 - 1 500 g. After conservative treatment because of fetal immaturity only 1 of 7 infants was born al

Fetal lung hypoplasia: biochemical and structural variations and their possible significance.

Quantitative biochemical criteria for lung growth and maturation were compared with the histological appearances in hypoplastic lungs from 20 fetuses and newborn infants. Cases associated with oligohydramnios showed a characteristic series of changes with narrow airways, retardation of epithelial and interstitial growth, delay in development of blood-air barriers, and low concentrations of phospholipid phosphorus, lecithin phosphorus, total palmitate, and lecithin palmitate. The growth and maturation arrest appeared to affect the peripheral part of the acinus. Examples of other types of lung h

Perinatal programming, renal development, and adult renal function.

Substantial data indicate that maternal diet during gestation, as well as perinatal events, affect organogenesis and, furthermore, may affect organ function at maturity, thus determining whether an individual is prone or resistant to a variety of health problems such as hypertension, cardiovascular disease, or renal dysfunction. This concept is called perinatal programming, or the fetal origins of adult disease (FOAD), a concept that arose after the publication of epidemiologic observations by Barker and colleagues that birth weight among apparently normal individuals studied in midlife was in
